ABSTRACT

OBJECTIVE: To evaluate the applicability and the psychometric properties of Montreal Cognitive Assessment Brazilian Version (MoCA-BR) in the elderly, as well as comparing its accuracy as a tracking test for mild cognitive impairment (MCI) and mild Alzheimer's disease (AD) with the accuracy of Mini-Mental State Examination (MMSE). METHOD: A transversal study was performed in 4 reference medical centers that care for the elderly. In all, 229 elderly participated in the study. To select the sample, the clinical history of the elderly, Pfeffer Functional Activities Questionnaire, and neuropsychological battery, apart from MMSE and MoCA-BR cognitive tests, were selected. The elderly were classified into control, MCI, and mild AD groups. RESULTS: There was a significant statistical difference between the MoCA-BR scores of the elderly and the control group, MCI, and mild AD (p < 0.001). The Cronbach alpha for MoCA-BR was 0.77, indicating a good internal consistency. The test-retest reliability was elevated, with intraclass correlation coefficient (ICC) 0.91. The inter-examiner reliability was excellent (ICC 0.96). The area under curve of the receiver operating characteristics curve was 0.95, when evaluating the ability of MoCA-BR to discriminate between the elderly with cognitive impairment and cognitively healthy elderly. CONCLUSIONS: The results of the study show that the Brazilian version of MoCA is a reliable cognitive tracking tool and is accurate for the detection of MCI and early stage AD, with good applicability on the elderly with education equal to or more than 4 years and adequate to discriminate between cognitively healthy elderly, and those with MCI and mild, proving to be superior to MMSE in tracking MCI and similar to this test when tracking mild AD.

ABSTRACT

Objetivo: Relatar a experiência vivenciada por uma equipe multiprofissional referente ao gerenciamento de um evento adverso após a passagem de cateter central de inserção periférica (PICC) por retenção do fio guia metálico. Método: Relato de experiência ocorrida em hospital geral no município de Guarulhos, São Paulo, em novembro de 2015. Resultado: Descreve-se o caso de lactente de um ano de idade, hemofílico e neuropata, que, após a passagem de PICC 3F no antebraço sem incidentes, teve seu fio guia retido, impossibilitando sua retirada. A radioscopia revelou enovelamento do fio guia, com necessidade de remoção cirúrgica. Conclusão: O procedimento cirúrgico foi realizado com sucesso. Não houve perda sanguínea de monta no campo operatório. A criança se recuperou bem, sem consequências detectáveis pela intercorrência. Notificou-se o fabricante sobre o evento ocorrido. Este caso serviu como um aprendizado para a equipe multiprofissional.


Objective: To report the experience of a multiprofessional team on the management of an adverse event after the insertion of a peripherally inserted central catheter (PICC) because of the retention of the metallic guidewire. Method: An experience report concerning an occurrence at a general hospital in the city of Guarulhos, São Paulo, Brazil, in November 2015. Results: Description of the case of a 1-year-old infant, with hemophilia and neuropathy, who had its guidewire held after the insertion of a PICC 3F in the forearm without incidents, which made its removal impossible. Radioscopy revealed that the guidewire was curled up and had to be surgically removed. Conclusion: The surgical procedure was successful. There was no blood loss in the operating field. The infant recovered well, without any consequences detected because of the complication. The manufacturer was notified of the event. This case served as a learning experience for the multiprofessional team.


Objetivo: Reportar la experiencia pasada por un equipo multiprofesional en referencia al manejo de un evento adverso luego del paso del catéter central de la inserción periférica (PICC) por retención del alambre metálico. Método: Relato de una experiencia sucedida en un hospital general del municipio de Guarulhos (San Pablo, Brasil), en noviembre de 2015. Resultado: Se describe el caso de un lactante de un año, hemofílico y portador de neuropatía, lo cual, luego del paso de un PICC 3F por su antebrazo sin incidencias, sufrió una retención del alambre, que imposibilitaba retirarlo. La radioscopia mostró un enredado del alambre guía, que debió retirarse por vía quirúrgica. Conclusión: El procedimiento quirúrgico se llevó a cabo con éxito. No hubo pérdida sanguínea relevante en el acto quirúrgico. El niño se recuperó bien, sin consecuencias detectables que fueron motivadas por el evento. Se notificó al fabricante sobre el inconveniente acaecido. Este caso sirve como un aprendizaje para el equipo multiprofesional.

Este estudo teve como objetivo verificar a prevalência de doenças palpebrais nos pacientes atendidos emergencialmente no ambulatório do Serviço de Oftalmologia do Hospital Universitário da Universidade Federal de Santa Catarina (HU/UFSC), associando-a com o ano, mês, procedência, sexo, faixa etária, diagnóstico e estação do ano.Método: Foram estudados, retrospectivamente, os dados de 385 pacientes com diagnóstico de doenças palpebrais, no período de janeiro de 2000 a dezembro de 2004.Resultados: As enfermidades palpebrais foram responsáveis por 24,2% dos atendimentos realizados no Serviço no período analisado. O ano com o maior número de atendimentos foi 2004 (25,7%), e o mês foi setembro (11,2%). Constatou-se que Florianópolis foi a cidade de procedência do maior número de pacientes (80,8%). Houve predomínio do sexo feminino (56,0%) sobre o masculino (44,0%) e a faixa etária mais acometida foi a de 15 a 29 anos (36,9%). Os diagnósticos mais freqüentes foram: blefarite (41,0%), hordéolo (31,4%) e calázio (17,4%). Verificou-se que o número de pacientes com doenças palpebrais foi maior na primavera (29,9%) e no inverno (27,8%). Conclusão: as doenças palpebrais são causas freqüentes de atendimento emergencial no ambulatório do Serviço de Oftalmologia do HU/UFSC, principalmente em mulheres jovens, procedentes de Florianópolis e nas estações frias do ano...


The aim of the study was to verify the prevalence of eyelid diseases at the Ophthalmologic Service of the Santa Catarina Federal University Hospital (HU/UFSC) from january/2000 to december/2004. Method: the data of 385 patients with eyelid diseases which were found through this period, were retrospectively studied and associated with the following variables: year, month, city of residence, sex, age, diagnosis and season. Results: these diseases were responsible for 24.2% of the medical attendances at the Service during the analysed period. The year that had the greatest number of patients with eyelid diseases was 2004 (25.7%), and the month that figured as the one with more cases was september (11.2%). Florianopolis was the hometown of most of the patients (80.8%). There was a preponderance of the feminine sex (56.0%) on masculine sex (44.0%) and the most prevalent age was between 15 and 29 years (36.9%). The most frequent diagnostics were: blepharitis (41.0%), hordeolum (31.4%) and 65 chalazion (17.4%). It was verified that the number of patients with eyelid diseases was higher in spring (29.9%) and winter (27.8%).Conclusion: Eyelid diseases are frequent reasons of medical attendance at the Ophthalmic Service of the Santa Catarina Federal University Hospital (HU/UFSC), mainly in young women, proceeding from Florianopolis and in the cold seasons of the year...
