ABSTRACT

PURPOSE: To evaluate the anxiety and depression levels during five days of stay in a Clinical Cardiological Intensive Care Unit (ICU) and verify the relation to different variables. METHODS: Forty seven patients were submitted to IDATE scale for anxiety and an avaliation for depression. The scores were compared to medical diagnoses, age, local of stay before the ICU, number of hospitalizations. RESULTS: Anxiety and depression scores decrease significant from first to fifth day of ICU stay. No significant correlation was found between age, number of hospitalizations, medical diagnoses and the anxiety and depression scores. Patients admitted from the emergency unit presented a correlation between anxiety and depression scores. CONCLUSION: Anxiety and depression decrease during the five days of ICU stay. Emotional reactions are not influenced by medical diagnoses, age and number of hospitalizations. The emergency unit stay affects emotionally the patient in the first days of ICU admission.

ABSTRACT

Por imagem corporal compreende-se a forma como nosso corpo é organizado em nossa vida psíquica. Baseia-se em contatos que o indivíduo tem consigo mesmo e com o mundo externo, apresentando-se em constante transformaçäo e assumindo caráter dinâmico. Qualquer modificaçäo na vida do indivíduo será fator provocativo de alteraçäo da imagem corporal, tal como ocorre no caso de doença orgânica. O transplante cardíaco é uma situaçäo com gravidade vital para o indivíduo, mobilizadora de afetos e sensaçöes fícas, e desencadeantes de profundas mudanças da auto-imagem. Foram estudados 16 pacientes em fila de espera para transplante cardíaco e 23 transplantados, a fim de verificar as alteraçöes de imagem corporal ocorridas. Para tal, utilizou-se o Desenho da figura Humana de Machover, instrumento que permite avaliar a dinâmica de personalidade e que tem suas bases no conceito de imagem corporal. Observaram-se, como resultados comparativos, mudanças entre os grupos quanto à imagem corporal. A relaçäo indivídu-udno, para o grupo de espera, aponta para sentimentos de gragilidade e vulnerabilidade, associados a dependência emocional e passividade. No pós-operatório, parece haver melhora no que diz respeito à auto-estima e na relaçäo indivíduo-mundo, persistindo dificuldades relacionadas à esfera da sexualidade, porém um pouco mais amenas. Em ambos os grupos foram notados traços característicos que simulam desenhos realizados por psicóticos.

ABSTRACT

PURPOSE: To study the psychological factors influence on the maintenance of smoking in coronary patients. METHODS: Questionnaire was applied in 40 smoking coronary patients, to investigate the tobacco's dependence type and degree, resistance degree, readiness level to abstinence and attitudes/emotions that favors tobacco's use. Male sex was predominant (75%) with age ranging from 25 to 80 (mean 52) years. RESULTS: Regarding the tobacco psychological type dependence, 75% of patients smoke to lower tension, 47.5% presenting a moderate resistance to quit smoking. Concerning the readiness to abstinence, 42.5% of patients were found to be in the contemplative stage. CONCLUSION: The coronary patient quit smoking treatment must include interventions aimed to provide instruments to cope with stressful situations and to increase the motivation to reach an action and maintenance abstinence smoking stage.

ABSTRACT

PURPOSE: To assess quality of life in patients submitted to surgical treatment of congenital heart disease in infancy. Quality of life has been assessed over social sciences backgrounds, but we have focused on the patient self-perception of their disease and related problems too. METHODS: We studied 134 adolescents, age ranged from 12 to 19 year-old, of both sexes, with any congenital heart disease, submitted to surgical treatment in infancy. They answered a questionnaire that covered their clinical, social, emotional, education and professional status. RESULTS: Ninety one percent qualified their lives as good because were symptom-free and living well without and physical restriction. CONCLUSION: The school performance was similar to the overall population engaged in Brazilian public schools. Their feelings, social relations and future plans were equal to the normal adolescents.

ABSTRACT

Cardiomyoplasty has recently been used as a surgical treatment for refractory heart failure, but its results have not been well described in quality-of-life patterns. We studied the quality of life of 14 patients (13 men, with a mean age of 43.3 +/- 7.4 years) submitted to this procedure for treatment of dilated or ischemic cardiomyopathies. They were approached by personal, structured interviews before and 13 +/- 9 months after the procedure, focusing on the following areas: physical activity, food and sleep patterns, working status, social activity, sexual activity, psychologic state, and perceptions and expectations about the treatment. The presence of limitation descriptors (discomfort, disability, and dissatisfaction) was recorded for all patients. The results showed an important decrease in limitation of physical activity, sleep pattern, social activity, and perceptions and expectations about the treatment. These findings suggest that cardiomyoplasty may improve the quality of life of a selected group of patients.
