ABSTRACT

The article analyses the 17-year (1993-2009 yy) experience of surgical treatment of desmoid fibroma and sarcoma of the thoracic and abdominal wall. 46 operations were analyzed. The radical excision according to the principles of surgical oncology remains the mainstay in the treatment of such patients. The combined technique with the use of polymeric implantates allows to cover large wall defects, thus extending the operability borders and being one of the main factors of the better treatment prognosis.

ABSTRACT

On the basis of investigations performed a classification is made systematizing postlactational involution according to different parameters and degree. It facilitates objectivization of indications to different correcting operative interventions on mammary glands with postlactational involution and choice of the optimum complex of surgical measures for correction of the mammary gland. It allows prognosis of its contour and esthetic results.

ABSTRACT

The article analyses the method of topical open treatment of postoperative pyothorax in 17 patients with the use of biologically active helevin-based compositions. The control group was made up of 11 patients with a similar suppurative process who received topical treatment by the same method but with the use of ointments with antibiotics and antiseptics on a fat base. With the use of biologically active compositions (BAC) on a helevin-base, the empyema cavity was completely freed from the pyonecrotic masses and its walls filled with granulation tissue on day 7 of treatment. In the control group the cavity was completely cleansed only on day 19 of treatment. It is shown that BAC possesses a high antibacterial effect not only due to the antibacterial components but because it is based on the laws of physical antiseptics.

ABSTRACT

The efficacy of surgical rehabilitation of patients with chronic osteomyelitis of the sternum is shown. Surgical treatment was divided in most cases into two stages: removal of the involved areas of the sternum and adjoining ribs and fixed muscular or musculocutaneous plastics. The obtained results are reassuring and promote further development of surgery of this region.

ABSTRACT

Generalization of an experience with using the transsternal access for the simultaneous treatment of the bilateral bullous disease of the lungs in 14 patients is presented. Under analysis were the nearest and long-term results (up to 18 months). Technical aspects of the surgery of bullous disease and its bilateral form in particular were considered. Postoperative complications were hemorrhage and paresis of the right diaphragm cupola. There were no lethal outcomes. An experience of other surgeons in this field is analyzed.

ABSTRACT

Silicone implants used in mammaplasty are often associated with constrictive capsule fibrosis, leading to induration and deformation of the prosthetic organs. In connection with this, there has been developed and is currently successfully applied in the clinical practice of the A. V. Vishnevsky Institute of Surgery an original method of mammary gland endoprosthesis with oily implants. The objective of this work was to make a comparative study of the late results of mammaplasty based upon the morphologic study of capsules formed around silicone and oily implants, removed during secondary surgical interventions. The dynamics of capsule formation around temporary organic glass endoprostheses has also been studied. The results showed that implantation of silicone prostheses results in a formation of a rough thick capsule, an active inflammatory reaction in the surrounding tissues and the development of constrictive fibrosis. Implantation of a temporary organic glass endoprosthesis that is filled with oil at the second stage of the operation, is followed by a thin capsule formation without signs of inflammation. The stability of the capsule structure up to 3 years has been noted. On the whole, this prosthesis provides a good cosmetic effect during long time after mammaplasty.
