ABSTRACT

Pairing is a commonly recommended practice used to build rapport and create positive therapeutic environments. However, there are limited evaluations of training procedures to teach pairing skills to direct-care staff. The purpose of the present study was to formalize and improve the efficiency of the training process for the initial stages of pairing using video modeling with embedded voice-over instructions plus performance feedback. Participants included three dyads of behavior technicians and children with autism. The results indicated that the training package increased the consistency of pairing implementation. These outcomes were maintained in a novel setting and for up to 4 weeks following training. We also measured relevant child behaviors such as joint attention, engagement, indices of happiness, and calm. Specific areas for future research are described.

Introduction: Previous studies have reported a correlation between a high-grade CMV-infection and an unfavorable prognosis in glioblastoma (GB). Coversely, epilepsy has been associated with a more favorable outcome in GB patients. Despites epilepsy and CMV share similar molecular mechanisms in GB tumoral microenvironment, the correlation between Tumor-Related-Epilepsy (TRE) and CMVinfection remains unexplored. The aim of our study is to examine the correlation between the dregree of CMV infection and seizure types on the survival of TRE Adult-type-diffuse-glioma. To achieve this objective, we conducted a comprehensive literature review to assess our results regarding previous publications. Methods: We conducted a retrospective-observational study on TRE Adult-type-diffuse-gliomas treated at a single center in Mexico from 2010 to 2018. Tumor tissue and cDNA were analyzed by immunochemistry (IHC) for CMV (IE and LA antigens) at the Karolinska Institute in Sweden, and RT-PCR for CMV-gB in Torreon Mexico, respectively. Bivariate analysis (X2-test) was performed to evaluate the association between subtypes of Adult-type-diffuse-glioma (IDH-mut grade 4 astrocytoma vs. IDH-wt glioblastoma) and the following variables: type of hemispheric involvement (mesial vs. neocortical involvement), degree of CMV infection (<25%vs. >25% infected-tumoral cells) and seizure types [Focal awareness, focal impaired awareness, and FBTCS]. Kaplan Meier and Cox analyses were performed to determine the risk, p < 0.05 was considered statistically significant. Results: Sixty patients with TRE Adult type diffuse gliomas were included (80% IDH-wt glioblastoma and 20% IDH-mut grade 4astrocytomas). The mean age was 61.5 SD ± 18.4, and 57% were male. Fifty percent of the patients presented with mesial involvement of the hemysphere. Seizure types included focal awareness (15%), focal impaired awareness (43.3%), and FBTCS (41.7%). Ninety percent of cases were treated with Levetiracetam and 33.3% presented Engel-IA postoperative seizure control. More than 90% of samples were positive for CMV-immunohistochemistry (IHC). However, all cDNA analyzed by RT-PCR return negative results. The median of overall survival (OS) was 15 months. High-grade CMV-IE infection (14 vs. 25 months, p<0.001), mesial involvement (12 vs. 18 months, p<0.001), and FBTCS were associated with worse OS (9 vs.18 months for non-FBTCS). Multivariate analysis demonstrated that high-grade CMV infection (HR = 3.689, p=0.002) and FBTCS (HR=7.007, p<0.001) were independent unfavorable survival factors. Conclusions: CMV induces a proinflammatory tumoral microenvironment that contributes to the developmet of epilepsy. Tumor progression could be associated not only with a higher degree of CMV infection but also to epileptogenesis, resulting in a seizure phenotype chracterized by FBTCS and poor survival outcomes. This study represents the first survival analysis in Latin America to include a representative sample of TRE Adult-type diffuse gliomas considering CMV-infection-degree and distinguishing features (such as FBTCS) that might have potential clinical relevance in this group of patients. Further prospective studies are required to validate these results.

ABSTRACT

Interteaching is a behavioral teaching method that has demonstrated efficacy in higher education. Of particular interest is the use of a preparation guide (a guided reading assignment), which is designed to promote engagement in the other areas of the interteaching process. The present study compared the use of a preparation guide completed before the start of class with that of a quiz administered at the start of the class. The quiz was hypothesized to serve as a functional alternative to the preparation guide. A total of 38 undergraduate students enrolled in an Introduction to Psychology course participated in this study. The primary dependent measure was student performance on tests following each condition. The analysis revealed no statistically significant difference between the conditions, F(1, 302) = 0.103, p = .748, though qualitative feedback revealed student preference for preparation guides. Future research is necessary to examine the effects of quizzing while addressing the limitations of this study.

ABSTRACT

Given the persistent shift in racial and ethnic demographics in the United States, board certified behavior analysts (BCBAs) will increasingly serve culturally and linguistically diverse families. There has been a recent increase in published resources to help behavior analysis practitioners navigate working with diverse populations. The purpose of this article is to add to these resources and demonstrate how these recommendations can be put into action. We outline five recommendations for working with culturally and linguistically diverse families in the context of a small company that has incorporated these practices in their own work focused on serving a large percentage of immigrant families.

ABSTRACT

There has been increased interest and attention to the need for equity, diversity, and inclusion, in the field of applied behavior analysis in recent years. Several publications have focused on these topics and educational curricula and professional development opportunities have been developed. One aspect that has received less attention is how companies providing behavior analytic services can help to promote and sustain a diverse workforce. The purpose of this article is to provide examples and recommendations for how these overarching goals can be addressed. The examples and recommendations are described in the context of a small company that has made important strides in addressing this topic through its mission to serve members of marginalized communities.

ABSTRACT Objective: To evaluate the response to cabergoline (CBG) treatment in patients with non-functioning pituitary adenomas (NFPA). Subjects and methods: Retrospective, single tertiary care center study. A total of 44 patients were treated with 3 mg/week of CBG, 32 after surgical treatment (transsphenoidal surgery [TSS] in 27 and TC in 5 patients) and 12 as primary therapy. Mean age was 59.2 ± 12 years and 23 (52.2%) were women. Response to therapy was ascertained by serial magnetic resonance imaging. The median duration of CBG therapy was 30 months (IQR 24-48). Response to CBG therapy was defined as a greater than 20% reduction in tumor size and volume. Results: A significant reduction in tumor size was documented in 29 patients (66%), whereas in 11 patients (25%) the tumor increased in size and in 4 (9%), it remained stable. Significant tumor shrinkage was documented in 4 (33.3%) of 12 patients treated primarily and in 23 (71.8%) of those treated secondarily. The three-year progression-free survival was 0.61. Conclusion: Cabergoline therapy is effective in reducing tumor growth in over two thirds of patients with NFPA, however 16% of patients will escape to this beneficial effect and will require alternative forms of treatment to halt tumor progression.

ABSTRACT

Recent statistics indicate children of Latino background are diagnosed with autism spectrum disorder (ASD) at a much later age than their non-Latino White counterparts. Once a diagnosis is made, it is critically important that families get access to evidence-based interventions, including applied behavior analysis (ABA) services. However, disadvantaging factors such as a lack of awareness of available services, poor health care access and health literacy, and language barriers may impact these families' ability to access and maintain ABA services. The purpose of this pilot study was to obtain preliminary information on the experiences of accessing and maintaining ABA services for a sample of Latino American families living in Massachusetts. We conducted structured interviews with 28 families that had a child with ASD aged 8 years or younger. Questions in the interview were related to the background and diagnostic experiences of the child; difficulties in accessing ABA services; and for those children receiving ABA services, parents' perceptions of the services. Results of the interviews are summarized, and implications for future research and service delivery are discussed.

INTRODUCTION: Shear-wave elastography (SWE) has been shown to be predictive of malignancy in thyroid nodules. OBJECTIVE: To determine, by SWE, the stiffness cutoff point with the highest specificity and sensitivity to detect thyroid nodules that require surgery. METHODS: Cross-sectional study of ultrasonographically-evaluated patients for thyroid nodules over a period of three years; the TI-RADS classification system was used, and nodule stiffness was determined by SWE. Histopathological specimens were classified using the Bethesda system, and the stiffness cutoff point with the highest specificity and sensitivity was obtained using ROC curves. RESULTS: Forty-one percent of the nodules were classified as TI-RADS 5, and 59 %, as TI-RADS 1-4. In TI-RADS 5 nodules, median stiffness of those in Bethesda system IV-VI categories was 35.9 kPa; in nodules with TI-RADS 1-4, 21.6 kPa. In TI-RADS 5 nodules, a cutoff point > 32.5 kPa had a specificity of 75 % and sensitivity of 57 % to detect those requiring surgery; in TI-RADS 1 to 4 nodules, a cutoff point of 21.5 kPa had a specificity of 63 % and sensitivity of 51 %. CONCLUSION: SWE-determined stiffness is useful to detect nodules that require surgical evaluation.

ABSTRACT

OBJECTIVE: To report the immunohistochemical and molecular evaluation of a patient with ectopic ACTH syndrome (EAS) from a MCAT which has single cells with features of both 96 medullary and cortical differentiation. Case Description and Methods. A 16-year-old woman presented with severe EAS and a large right MCAT composed of ACTH-secreting cells resembling pheochromocytoma and another lineage similar to adrenal carcinoma. Immunohistochemistry (IHC) showed positivity for medullary (ACTH, chromogranin A, synaptophysin, and PS-100) and epithelial components (inhibin, melan-A, and calretinin). Embryonic stem cell markers were evaluated using RT/PCR and immunofluorescence. After initial surgery, the tumor recurred shifting to rapidly progressive ACTH-independent liver metastasis. RESULTS: Histopathology and IHC revealed two distinct and intermingled cellular patterns, while some cells immunostained for both medullary and cortical markers. Demonstration of all stem cell biomarkers by RT/PCR and immunofluorescence was predominantly localized to the nucleus, whereas SOX2 immunoreactivity was evident in the cytoplasm as well. CONCLUSION: The expression of cancer stem cell biomarkers points towards the involvement of primitive embryonic cells as the origin of this neoplasm and maybe to the clinically aggressive and biochemically changing behavior.

Background: 85% of neuroendocrine tumors (NET) originate in the gastrointestinal tract, which is why their primary hepatic location is very rare; NETs most frequently cause metastases to the liver; so when diagnosed, a hepatic NET is considered initially metastatic. Diagnosis of primary hepatic neuroendocrine tumors (PHNET) should be performed by excluding extrahepatic NETs and through histological confirmation. The objective of this article is to present a case of PHNET. Clinical case: 75-year-old male patient, who presented asthenia, adynamia, abdominal pain in the right hypochondrium, six-month weight loss, with a right subcostal palpable tumor. Imaging studies reported a lesion in the right hepatic lobe, multilobulated, heterogeneous, with poorly defined margins and with cystic areas. It was performed diagnostic laparotomy and then a hepatic tumorectomy, whose product measured 16.0 x 10.0 x 6.5 cm, with two cystic cavities of 13.2 and 11.5 cm of hematic content. Microscopically, cells with neuroendocrine differentiation, with positive immunoreactivity to chromogranin were observed. It was diagnosed well-differentiated neuroendocrine neoplasm, with cystic degeneration. Conclusions: Even though it is excluded a NET from an extrahepatic primary site, the tumor etiology of an important proportion of patients with PHNET will be due to an unknown primary tumor that will become apparent over time; hence, the need to follow up as long as possible.


Introducción: 85% de los tumores neuroendocrinos (TNE) se originan del tracto gastrointestinal, su localización hepática primaria muy rara; los TNE con mayor frecuencia causan metástasis al hígado; por esto, cuando se diagnóstica un TNE hepático es considerado inicialmente metastásico. El diagnóstico de tumor neuroendocrino primario de hígado (TNEPH) debe realizarse mediante la exclusión de tumores neuroendócrinos extrahepáticos y la confirmación histológica. El objetivo de este artículo es presentar un caso con TNEHP. Caso clínico: paciente de sexo masculino, de 75 años de edad, quien presentó astenia, adinamia, dolor abdominal en hipocondrio derecho y pérdida de peso de 6 meses de evolución, con tumor palpable subcostal derecho. Los estudios de imagen reportaron lesión en lóbulo hepático derecho, multilobulada, márgenes mal definidos, heterogénea y con áreas quísticas. Se realizó laparotomía diagnóstica y tumorectomía hepática que midió 16.0 x10.0 x 6.5 cm, con dos cavidades quísticas de 13.2 y 11.5 cm de contenido hemático. Microscópicamente se observaron células con diferenciación neuroendócrina, inmunoreactivas a cromogranina. Se estableció el diagnóstico de neoplasia neuroendócrina bien diferenciada, con degeneración quística. Conclusiones: aunque se excluya un TNE de sitio primario extrahepático, la etiología tumoral de una proporción considerable de pacientes con TNEPH será a causada por un tumor primario no conocido que se hará evidente con el tiempo; de ahí la necesidad de hacer seguimiento el máximo tiempo posible.

Introducción. Las jóvenes tienen su primera experiencia sexual en edades tempranas; si padecen una cardiopatía, el riesgo de morir se eleva cinco veces durante el parto. Objetivo. Describir los conocimientos que tienen las adolescentes sobre su cardiopatía congénita (CC), sexualidad, embarazo y las complicaciones que esto conlleva. Material y métodos. Estudio descriptivo, transversal y prospectivo realizado previo consentimiento informado de los padres. Se aplicó cédula ex profeso a 30 mujeres hospitalizadas de 12 a 18 años de edad con alguna CC. Cédula con información sociodemográfica y sobre conocimientos de su enfermedad, sexualidad y embarazo, con respuestas dicotómicas, opción múltiple y abiertas. Datos analizados con medidas de dispersión y tendencia central. Resultados. Edad promedio 14.6 años; 10% con vida sexual activa iniciada entre 13-17 años. Todas conocen su tratamiento, 90% sabe qué enfermedad padece. 77.3% tiene confianza con su madre para platicar de sexualidad; 60% considera que es por falta de información el embarazo; 73.4% conoce dos o más métodos anticonceptivos, principalmente condón, aunque 29.9% piensa erróneamente que todos protegen de enfermedades de transmisión sexual; 73.3% considera poderse embarazar aún con cardiopatía. Conclusiones. La mayoría de adolescentes está informada, pero es imperativo el desarrollo de estrategias educativas que disminuyan la problemática.


Introduction. Today, young people have their first sexual experience at early ages. In addition, if they suffer from heart disease, the risk of dying raises five times during childbirth. Objective. Describe the knowledge they have about their adolescent congenital heart disease (CHD), sexuality, pregnancy and the complications that entails. Material and methods. We performed a descriptive, transversal and prospective study prior informed consent of the parents. A charter expressly 30 woman 12 to 18 years of age with some kind of CC, hospitalized was applied. The schedule consisted of sociodemographic information and knowledge about his illness, sexuality and pregnancy with dichotomous response, multiple choice and open. We analyzed the data with measures of central tendency and dispersion. Results. Average age 14.6 years; 10% initiated sexual activity between 13 and 17 years. All know their treatment, 90% know what disease you have and what stage. 77.3% have confidence with her mother to talk about sexuality; 60% believe that teenage pregnancy is due to lack of information; 73.4% know two or more birth control. The best known is the condom, while 29.9% mistakenly think that some contraceptive methods protect against sexually transmitted diseases; and 73.3% consider being able to still pregnant with heart disease. Conclusions. Most teens know about their disease, contraception and sexuality; however, it is imperative to develop educational strategies to reduce the problem.

I will address two important highlights of the revised mission statement of The Analysis of Verbal Behavior aimed at actively encouraging diversity of thought in publications and focusing on narrowing the gap between applied research and practice.

OBJECTIVES: The aim of this study was to compare the effectiveness of oral (PO) versus enteral nutrition (EN) medium-chain triglyceride (MCT) containing-formula to prevent malnutrition and growth impairment in infants with biliary atresia (BA) waiting for a liver transplant. METHODS: A total of 15 infants, 3 to 9 months old with BA were included. They were randomly assigned to either PO or EN. For 12 weeks, both groups received an MCT formula fortified with glucose polymers and corn oil to reach a caloric density between 0.8 and 1 kcal/mL. The formula given to the PO group was administered ad libitum and that given via EN was infused through a nasogastric tube to reach 140% of the energy intake recommended by the Dietary Recommended Intake guidelines. Protein intake was adjusted to 4 to 5 g/kg present weight. Outcome variables were growth and nutritional status evaluated periodically by anthropometric indicators. Biochemical and hematological variables were evaluated through the study. RESULTS: Baseline clinical, nutritional, biochemical, and hematological variables showed no differences between the study groups. Baseline length/age was <-2 SD in 10 of the 15 patients; in the PO group, it fell <-3 SD, whereas in the EN group, it remained stable. Head circumference z score dropped 0.6 SD in the PO group, whereas in the EN group it remained stable. Triceps skinfold values improved in the infants taking EN, P < 0.001. The frequency of adverse effects--respiratory infection and diarrhea--was higher in the EN group. No biochemical or hematological differences were observed between the study groups throughout the study. CONCLUSIONS: A 12-week EN trial with an MCT-fortified formula prevented malnutrition and growth impairment in infants with BA waiting for a liver transplant.

ABSTRACT

A multiple baseline design across participants was used to evaluate the effects of video modeling with embedded instructions on training teachers to implement 3 preference assessments. Each assessment was conducted with a confederate learner or a child with autism during generalization probes. All teachers met the predetermined mastery criterion, and 2 of the 3 demonstrated skill maintenance at 1-month follow-up.

ABSTRACT

The prevalences of overweight and obesity have increased dramatically in the last two decades in the adult and children population. The Organization for Cooperation and Economic Development reported in 2010 that Mexico ranks first worldwide in childhood obesity. The 2006 National Health and Nutrition Survey reported that one of every three teenagers are overweight and obese. In the last decades, pediatric hospitals in different parts of the world reported the prevalence of secondary malnutrition, since in those days overweight and obesity did not represent health problems. Currently, the prevalence of overweight and obesity has been scarcely studied in pediatric hospitals. In the Hospital de Pediatría (Children's Hospital) of the Instituto Mexicano del Seguro Social's Centro Médico Nacional de Occidente it is reported a prevalence of overweight of 15.4 % and obesity of 12.2 %, which reflects a nutritional transition.Due to the high prevalence of overweight and obesity in this pediatric hospital of reference, one could conclude that the pediatrician should be able to make a correct evaluation of the nutritional state, because, if he does not detect these problems, we will be condemning children to suffer from a chronic disease for the rest of their lives, and with all the implications in the short, medium and long term.


Las prevalencias de sobrepeso y obesidad se han incrementado alarmantemente en las dos últimas décadas en la población adulta e infantil. La Organización para la Cooperación y el Desarrollo Económicos reportó en el 2010 que México era el primer lugar mundial de obesidad en niños. La Encuesta Nacional de Salud y Nutrición 2006 reporta que uno de cada tres adolescentes presentan sobrepeso y obesidad. En décadas pasadas los hospitales pediátricos en diferentes partes del mundo reportaban la prevalencia de desnutrición secundaria, ya que el sobrepeso y la obesidad no representaban problemas de salud. Actualmente la prevalencia de sobrepeso y obesidad ha sido poco estudiada en hospitales pediátricos. En el Hospital de Pediatría del Centro Médico Nacional de Occidente del Instituto Mexicano del Seguro Social se reporta una prevalencia de sobrepeso de 15.4 % y de obesidad de 12.2 %, lo que refleja una transición nutricia. Debido a esta alta prevalencia de sobrepeso y obesidad en este hospital; pediátrico de referencia, se podría concluir que el pediatra debería estar capacitado para realizar una correcta evaluación del estado nutricio, ya que si él no detecta estos problemas, estaremos condenando a los niños a padecer una enfermedad crónica por el resto de su vida, con todas las implicaciones que tiene a corto, mediano y largo plazo.

ABSTRACT

We investigated the relative effects of simple and complex auditory-visual discrimination training using an adapted alternating treatments design to establish derived stimulus relations in 2 children who had been diagnosed with autism and 1 typically developing peer. Emergence of untrained conditional relations was observed after training in both conditions, with a possible advantage of simple-sample training for 1 participant. Results of generalization and follow-up probes were mixed.

ABSTRACT

We evaluated the effectiveness of a behavioral skills training package to teach implementation of Phases 1, 2, and 3A of the picture exchange communication system (PECS) to teachers employed at a therapeutic center for children with autism. Probes in the natural environment and follow-up were conducted with children who were assigned to work with the teachers in their own classrooms. Results provide additional support for the efficacy of behavioral skills training to teach implementation of PECS.
