ABSTRACT

The dominant SARS-CoV-2 Delta variant (B.1.617.2) became the main circulating variant among countries by mid 2021. Attention was raised to the increased risk of airborne transmission, leading to nosocomial outbreaks even among vaccinated individuals. Considering the increased number of COVID-19 hospital admissions fueled by the spread of the variant, with Spain showing the highest COVID-19 rates in mainland Europe by July 2021, the aim of this study was to assess SARS-CoV-2 environmental contamination in different areas of a University Hospital in the region of Castile-León, Spain, during the peak of the 5th wave of COVID-19 in the country (July 2021). Air samples were collected from sixteen different areas of the Hospital using a Coriolis® µ air sampler. Surface samples were collected in these same areas using sterile flocked plastic swabs. RNA extraction followed by a one-step RT-qPCR were performed for detection of SARS-CoV-2 RNA. Of the 21 air samples, only one was positive for SARS-CoV-2 RNA, from the emergency waiting room. Of the 40 surface samples, 2 were positive for SARS-CoV-2 RNA, both from the microbiology laboratory. These results may be relevant for risk assessment of nosocomial infection within healthcare facilities, thus helping prevent and minimize healthcare staff's exposure to SARS-CoV-2, reinforcing the importance of always wearing appropriate and well-fit masks at all times and proper PPE when in contact with infected patients.

ABSTRACT

En este trabajo presentamos un caso de luxación recidivante de rótula en un paciente joven con dos cirugías previas, al cual se decidió realizarle, dados sus antecedentes quirúrgicos y su edad, una cirugía poco frecuente como es la trocleoplastia + revisión de plástica del ligamento patelofemoral medial (LPFM).Haremos una breve reseña de la inestabilidad de la rótula para entender el evento, su diagnóstico, opciones de tratamiento y la presentación del caso con la descripción de la técnica quirúrgica realizada. We present a case of recurrent patellar dislocation in a young patient with two previous surgeries, which was decided to perform a rare surgery such as trocheoplasty and medial patellofemoral ligament revision.We make a brief review of the patellar instability to understand de case, diagnosis and treatment options and case presentation with chirurgical technique description that we perform. ABSTRACT

Background: Candida parapsilosis is a frequent cause of candidemia worldwide. Its incidence is associated with the use of medical implants, such as central venous catheters or parenteral nutrition. This species has reduced susceptibility to echinocandins, and it is susceptible to polyenes and azoles. Multiple outbreaks caused by fluconazole-nonsusceptible strains have been reported recently. A similar trend has been observed among the C. parapsilosis isolates received in the last 2 years at the Spanish Mycology Reference Laboratory. Methods: Yeast were identified by molecular biology, and antifungal susceptibility testing was performed using the European Committee on Antimicrobial Susceptibility Testing protocol. The ERG11 gene was sequenced to identify resistance mechanisms, and strain typing was carried out by microsatellite analysis. Results: We examined the susceptibility profile of 1315 C. parapsilosis isolates available at our reference laboratory between 2000 and 2021, noticing an increase in the number of isolates with acquired resistance to fluconazole, and voriconazole has increased in at least 8 different Spanish hospitals in 2020-2021. From 121 recorded clones, 3 were identified as the most prevalent in Spain (clone 10 in Catalonia and clone 96 in Castilla-Leon and Madrid, whereas clone 67 was found in 2 geographically unrelated regions, Cantabria and the Balearic Islands). Conclusions: Our data suggest that concurrently with the coronavirus disease 2019 pandemic, a selection of fluconazole-resistant C. parapsilosis isolates has occurred in Spain, and the expansion of specific clones has been noted across centers. Further research is needed to determine the factors that underlie the successful expansion of these clones and their potential genetic relatedness.

ABSTRACT

AIM: To evaluate the serological response against SARS-CoV-2 in a multicenter study representative of the Spanish COVID pandemic. METHODS: IgG and IgM + IgA responses were measured on 1466 samples from 1236 Spanish COVID-19 patients admitted to the hospital, two commercial ELISA kits (Vircell SL, Spain) based on the detection of antibodies against the viral spike protein and nucleoprotein, were used. RESULTS: Approximately half of the patients presented antibodies (56.8% were IgM + IgA positive and 43.0% were IgG positive) as soon as 2 days after the first positive PCR result. Serological test positivity increased with time from the PCR test, and 10 days after the first PCR result, 91.5% and 88.0% of the patients presented IgM + IgA and IgG antibodies, respectively. CONCLUSION: The high values of sensitivity attained in the present study from a relatively early period of time after hospitalization support the use of the evaluated serological assays as supplementary diagnostic tests for the clinical management of COVID-19.

Introduccion: En este trabajo se estudia las relaciones entre el varo/valgo, flexion/extensión y rotación interna/externa de rodilla en los a jugadores de básquet durante movimientos de lanzamiento. Material y Método: Se realizó el registro de siete sujetos masculinos, jugadores amateur de básquet. Cada uno ejecutó cinco lanzamientos de tiro simple y en carrera. Se evaluó la influencia en el movimiento varo/valgo de estos gestos. Resultados: Se encontró que en ambos casos existe una mayor tendencia al valgo, con un 69% para el lanzamiento simple y un 80% para el lanzamiento simple con carrera. Se registró para la pierna izquierda mayor movimiento de valgo de rodilla que para la pierna derecha. Se encontró que el 78% de los registros en lanzamiento simple y el 93 % lanzamiento con carrera realizan un movimiento de valgo para la rodilla izquierda. Conclusión: Al comparar los ángulos máximos de flexión/extensión y de rotación interna/externa con los ángulos máximos varo/valgo se encontró una débil correlación entre el ángulo varo/valgo y el ángulo de rotación interna/externa, como así también entre el ángulo varo/valgo y el ángulo de flexión/extensión en rodilla, para los dos gestos de lanzamiento analizados. Background: The relationship between knee varus-valgus, flexion-extension and internal-external rotation in basketball players and their shot movements is studied in this paper. Methods: Seven male amateur players without injury antecedents were recorded with tridimensional motion capture system for biomechanical analysis. Each volunteer executed/performed 5 free shots and 5 jump shots. Results:The relationship between this gestures and varus-valgus movement was assessed. In both types of shooting, there is a valgus trend, with 69% in free shot and 80% in jump shot. In all cases, registered volunteers presented right limb dominance. In left limb assessment, a greater valgus knee movement was found. In 78% of free shot and 93% of jump shots, amateur players achieved valgus knee movement with their left limb. Conclusion:Achieving a data comparison of maximum angles corresponding to flexion-extension, internal-external rotation and varus-valgus position, it was found a weak correlation between knee varus-valgus angles and knee internal-external rotation angles, and so between knee varus-valgus angles and knee flexion-extension angles for both types of shooting gestures acquired. Introducción: El propósito de este trabajo es describir en forma retrospectiva las lesiones en el compartimiento lateral de la rodi Ila en jugadores profesionales de básquet. Materiales y Métodos: Presentamos 6 (seis) casos de jugadores profesionales de básquet del Centro Juventud Sionista (CJS) en seis temporadas de Liga Nacional de Básquet (LNB). Los pacientes fueron evaluados utilizando los siguientes criterios: examen físico, estudios complementarios (Rx y RMN) y artroscopía. Se utilizó la clasificación de Duterbridge para estadificar las lesiones osteocondrales. Resultados: Tres pacientes fueron diagnosticados y tratados con lesiones meniscales puras y regresaron a la competencia a partir de los 40 días de postoperatorio. Aquellos que presentaron lesiones osteocondrales siguieron el mismo protocolo de rehabilitación llevado a cabo por el mismo equipo de kinesiólogos del club (CJS), retornando a la competencia a partir de los 90 días de postoperatorio. Todos los jugadores regresaron a la competencia al mismo nivel y en la misma categoría (LNB-Liga Nacional de Básquet) que antes de operarse. Conclusión: Nuestros hallazgos son coincidentes con la escasa bibliografía internacional respecto a las lesiones meniscales evaluadas en basquetbolistas profesionales que indican una mayor incidencia de lesiones del menisco externo respecto del interno. Creemos que los movimientos específicos desarrollados en este deporte en particular contribuyen al desarrollo de lesiones del compartimiento externo...

Los conjuntos bacterianos son sistemas dinámicos complejos que interactúan entre sí, y gracias a esa interacción, las bacterias coexisten, colaboran, compiten e intercambian información de forma coordinada. Una forma de comunicación de las bacterias es el quorum sensing. Mediante este mecanismo las bacterias pueden conocer su concentración en un ambiente determinado y decidir el momento en el que se va a poner en marcha la expresión de un determinado conjunto de genes con el fin de desarrollar una respuesta concreta y de forma simultánea. El resultado de estas interconexiones hace que surjan propiedades que no pueden explicarse a partir de una sola célula bacteriana aislada (AU)
