ABSTRACT

Robotic systems are an integral component of today's work place automation, especially in industrial settings. Due to technological advancements, we see new forms of human-robot interaction emerge which are related to different OSH risks and benefits. We present a multifaceted analysis of risks and opportunities regarding robotic systems in the context of task automation in the industrial sector. This includes the scientific perspective through literature review as well as the workers' expectations in form of use case evaluations. Based on the results, with regards to human-centred workplace design and occupational safety and health (OSH), implications for the practical application are derived and presented. For the literature review a selected subset of papers from a systematic review was extracted. Five systematic reviews and meta-analysis (492 primary studies) focused on the topic of task automation via robotic systems and OSH. These were extracted and categorised into physical, psychosocial and organisational factors based on an OSH-factors framework for advanced robotics developed for the European Agency for Safety and Health at Work (EU-OSHA). To assess the workers' perspective, 27 workers from three European manufacturing companies were asked about their expectations regarding benefits and challenges of robotic systems at their workplace. The answers were translated and categorised in accordance with the framework as well. The statements, both from literature and the survey were then analysed according to the qualitative content analysis, to gain additional insight into the underlying structure and trends in them. As a result, new categories were formed deductively. The analysis showed that the framework is capable to help categorise both findings from literature and worker survey into basic categories with good interrater reliability. Regarding the proposed subcategories however, it failed to reflect the complexity of the workers' expectations. The results of the worker evaluation as well as literature findings both predominantly highlight the psychosocial impact these systems may have on workers. Organisational risks or changes are underrepresented in both groups. Workers' initial expectations lean towards a positive impact.

ABSTRACT

The continuous and rapid development of AI-based systems comes along with an increase in automation of tasks and, therewith, a qualitative shift in opportunities and challenges for occupational safety and health. A fundamental aspect of humane working conditions is the ability to exert influence over different aspects of one's own work. Consequently, stakeholders contribute to the prospect of maintaining the workers' autonomy albeit increasing automation and summarize this aspiration with the human in control principle. Job control has been part of multiple theories and models within the field of occupational psychology. However, most of the models do not include specific technical considerations nor focus on task but rather on job level. That is, they are possibly not able to fully explain specific changes regarding the digitalization of tasks. According to the results of a large-scale study on German workers (DiWaBe), this seems to be the case to some extend: the influence of varying degrees of automation, moderated by perceived autonomy, on workers' wellbeing was not consistent. However, automation is a double-edged sword: on a high level, it can be reversely related to the workers' job control while highly autonomous and reliable systems can also create opportunities for more flexible, impactful and diverse working tasks. Consequently, automation can foster and decrease the factor of job control. Models about the optimal level of automation aim to give guidelines on how the former can be achieved. The results of the DiWaBe study indicate that automation in occupational practice does not always happen in line with these models. Instead, a substantial part of automation happens at the decision-making level, while executive actions remain with the human. From an occupational safety and health perspective, it is therefore crucial to closely monitor and anticipate the implementation of AI in working systems. Constellations where employees are too controlled by technology and are left with a high degree of demands and very limited resources should be avoided. Instead, it would be favorable to use AI as an assistance tool for the employees, helping them to gather and process information and assisting them in decision-making.

ABSTRACT

BACKGROUND: Coral snake bites from Micrurus fulvius and Micrurus tener account for < 1% of all snake bites in North America. Coral snake envenomation may cause significant neurotoxicity, including respiratory insufficiency, and its onset may be delayed up to 13 h. CASE REPORT: We present a unique patient encounter of M. tener venom exposure through the ocular mucous membranes and a small cutaneous bite, resulting in neurotoxicity. To our knowledge, this is the first reported case of systemic neurotoxicity associated with ocular contact with coral snake venom. Our patient developed rapid-onset skeletal muscle weakness, which is very uncommon for M. tener, along with cranial nerve deficits. Acquisition of antivenom was challenging, but our patient provides a rare report of resolution of suspected M. tener neurotoxicity after receiving Central American coral snake (Micrurus nigrocinctus) antivenom. Our patient subsequently developed serum sickness, a known delayed complication of antivenom. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: The emergency physician should be aware that coral snake venom may be absorbed through different routes. Neurotoxicity and respiratory insufficiency may be fatal and onset may be delayed up to 13 h. North American Coral Snake Antivenom is in very limited supply, so non-Food and Drug Administration-approved alternative coral snake antivenoms may be used for patients demonstrating neurotoxicity. Emergency physicians should be proactive in contacting a toxicologist to procure antivenom, as well as consideration of adjunctive treatments, such as neostigmine. Furthermore, whole immunoglobulin G products, such as antivenom, may result in immediate and delayed reactions.

ABSTRACT

Continuous labor support offers multiple benefits for mothers and infants. The type of caregiver that is the best support person in labor has not been identified. A critical review of the English language literature was conducted to describe the current state of knowledge on different types of labor support persons. Randomized trials and other published reports were identified from relevant databases and hand searches. Studies were reviewed and assessed by using a structured format. Eight randomized trials met the selection criteria for inclusion in this analysis. These trials investigated untrained and trained lay women, female relatives, nurses, lay midwives, and student lay midwives as labor support persons. Support by untrained lay women starting in early labor and continuing into the postpartum period demonstrates the most consistent beneficial effect on childbirth outcomes. However, more randomized controlled trials are warranted before firm conclusions may be drawn.
