ABSTRACT

OBJECT: Craniovertebral junction malformation (CVJM) or Chiari malformation in adults, with or without syringomyelia and basilar invagination, produces neuronal dysfunction of the brainstem, cerebellum, cranial nerves, and upper spinal cord. The respiratory center and some of its afferent and efferent components can be altered in these diseases. The authors studied patients with and without CVJM to determine whether this physical feature contributed to sleep disturbances. METHODS: Respiratory manifestations during sleep were studied prospectively, by using whole-night polysomnography, in 32 symptomatic patients (CVJM group) and 16 healthy volunteers (control group). Patients with CVJM presented with more sleep disturbances (reports of snoring and apnea) than those in the control group. The apnea/hypopnea index values were higher in patients with CVJMs than in the control group (13 +/- 15 compared with 3 +/- 6; p = 0.007) and the rate of central sleep apneas was higher in the CVJM than in the control group (22 +/- 30 compared with 4 +/- 8%; p = 0.009). The apnea/hypopnea index was highest in the subgroup with basilar invagination than in the other subgroups. The central apneic episodes were more frequent in the patients with basilar invagination (35 +/- 40%; p = 0.001) and in those with syringomyelia (17.6 +/- 24.6%; p = 0.003) than in the control group (4 +/- 8%). Patients with symptomatic CVJM, especially those with basilar invagination, presented with more sleep respiratory compromise than did those in the control group. CONCLUSIONS: The incidence of sleep apnea/hypopnea syndrome is significantly higher in patients with CVJM.

ABSTRACT

Os autores descrevem o caso de um paciente do sexo masculino, de 66 anos de idade, com lesão neoplásica gástrica Borrmann IV, desde 2 cm do cárdia até a região pré-pilórica, cujo exame anatomopatológico revelou adenocarcinoma tubulopapilífero pouco diferenciado. O mesmo paciente era portador de gioblastoma multiforme do lobo temporal esquerdo. Neoplasias malignas orimárias múltipals que incluam neoplasias cerebrais primárias são raras, havendo apenas 17 casos descritos na literatura. Os autores discutem a etiopatogenia dessa infrequente patologia.

ABSTRACT

Os autores relatam o caso de um paciente de 54 anos, do sexo masculino, que procurou o nosso serviço com queixas de sonolência diurna, ronco e pausas respiratórias durante o sono. O estudo com polissonografia revelou a presença de apnéias centrais, com índice de apnéia/hipopnéia (IAH)=47,0. O paciente retornou ao serviço, após um ano sem seguimento, com piora da sintomatologia, fraqueza em membros superiores e dificuldade de deglutição. Foi estudado com ressonância magnética (RM) que mostrou malformação de Chiari tipo I (MCI) com siringomielia C1-C3, tendo sido submetido à cirurgia descompressiva para tratamento da MCI e siringomielia. Houve remissão da apnéia do sono central, com evolução do IAH para 9,6 em polissonografia pós-operatória.

ABSTRACT

Os autores apresentam um caso de puberdade precoce em um menino de sete anos, em que após investigaçäo clínica, radiológica, tomográfica e laboratorial foi feita a hipotese diagnóstica de tumor de pineal. Após realizaçäo da cirurgia para ressecçäo tumoral, diagnosticou-se germinoma com áreas de coriocarcinoma em pineal. Os autores mostram a evoluçäo, exames laboratoriais incluindo dosagens hormonais e marcadores biológico, e também os tratamentos cirúrgico e quimioterápico.
