ABSTRACT

Base excision repair (BER) generates gapped DNA intermediates containing a 5'-terminal 2-deoxyribose-5-phosphate (5'-dRP) group. In mammalian cells, gap filling and dRP removal are catalyzed by Pol ß, which belongs to the X family of DNA polymerases. In higher plants, the only member of the X family of DNA polymerases is Pol λ. Although it is generally believed that plant Pol λ participates in BER, there is limited experimental evidence for this hypothesis. Here we have characterized the biochemical properties of Arabidopsis thaliana Pol λ (AtPol λ) in a BER context, using a variety of DNA repair intermediates. We have found that AtPol λ performs gap filling inserting the correct nucleotide, and that the rate of nucleotide incorporation is higher in substrates containing a C in the template strand. Gap filling catalyzed by AtPol λ is most efficient with a phosphate at the 5'-end of the gap and is not inhibited by the presence of a 5'-dRP mimic. We also show that AtPol λ possesses an intrinsic dRP lyase activity that is reduced by mutations at two lysine residues in its 8-kDa domain, one of which is present in Pol λ exclusively and not in any Pol ß homolog. Importantly, we also found that the dRP lyase activity of AtPol λ allows efficient completion of uracil repair in a reconstituted short-patch BER reaction. These results suggest that AtPol λ plays an important role in plant BER.

ABSTRACT

Active DNA demethylation plays an important role in controlling methylation patterns in eukaryotes. In plants, the DEMETER-LIKE (DML) family of 5-methylcytosine DNA glycosylases initiates DNA demethylation through a base excision repair pathway. However, it is poorly understood how these DNA demethylases are recruited to their target loci and the role that histone marks play in this process. Arabidopsis REPRESSOR OF SILENCING 1 (ROS1) is a representative enzyme of the DML family, whose members are uniquely characterized by a basic amino-terminal domain mediating nonspecific binding to DNA, a discontinuous catalytic domain, and a conserved carboxy-terminal domain of unknown function. Here, we show that ROS1 interacts with the N-terminal tail of H3 through its C-terminal domain. Importantly, phosphorylation at H3 Ser28, but not Ser10, abrogates ROS1 interaction with H3. Conserved residues at the C-terminal domain are not only required for H3 interaction, but also for efficient DNA binding and catalytic activity. Our findings suggest that the C-terminal domain of ROS1 may function as a histone reader module involved in recruitment of the DNA demethylase activity to specific genomic regions.

ABSTRACT

An 8-year-old domestic short hair female cat initially presented with bilateral uveitis with pseudotumoral appearance. The patient tested negative for feline immunodeficiency virus (FIV), feline leukemia virus (FeLV) and Toxoplasma gondii. Histopathology of a granulomatous lesion on the upper left conjunctiva revealed amastigotes compatible with Leishmania spp. Aqueous humor was aspired and the diagnosis was confirmed after isolation of promastigotes cultivated in biphasic NNN medium and by positive polymerase chain reaction (PCR) for Leishmania infantum. Treatment with allopurinol (10mg/kg/ BID/PO) was commenced and a natural insect repellent was prescribed. Six months of treatment with allopurinol associated with the initial topical medications helped to improve ocular signs. Leishmaniasis should be considered as a differential diagnosis in cats presenting uveitis with pseudotumoral appearance. To our knowledge, this is the first report of feline leishmaniasis with ocular manifestation in Brazil, in which diagnosis was confirmed by aqueous humor analysis.(AU)


Uma gata, sem raça definida, de oito anos de idade, foi atendida inicialmente com uveíte bilateral, com aparência pseudotumoral em íris. Foi realizado teste para o vírus da imunodeficiência felina (FIV), da leucemia felina (FeLV) e de Toxoplasma gondii, obtendo-se resultados negativos. O exame histopatológico da conjuntiva superior do olho esquerdo revelou amastigotas compatíveis com Leishmania spp. Foi realizada paracentese, e promastigotas foram isoladas no humor aquoso, cultivadas em meio NNN bifásica. Reação em cadeia da polimerase (PCR) confirmou diagnóstico positivo para Leishmania infantum. Tratamento com alopurinol (10mg/kg/BID/PO) foi iniciado, e um repelente natural de insetos foi prescrito. Seis meses de tratamento com alopurinol associado aos medicamentos tópicos iniciais ajudaram a melhorar os sinais oculares. Leishmaniose deve ser considerada como um diagnóstico diferencial nos gatos que apresentam uveíte com aparência pseudotumoral de íris. Até o presente momento, este é o primeiro relato de leishmaniose felina com manifestação exclusivamente ocular da doença no Brasil cujo diagnóstico foi confirmado por meio de análise de humor aquoso.(AU)

O presente trabalho objetivou avaliar os resultados de 34 olhos submetidos ao enxerto conjuntival pediculado (ECP) em ceratites ulcerativas profundas (n=5), em ceratites ulcerativas com colagenólise (n=2), descemetocele (n=7), perfuração corneal (n=15) e prolapso de íris (n=5). Os impactos do grau de uveíte e da integridade da córnea foram correlacionados com presença e ausência de visão por tabelas de contingência. Raças braquicefálicas foram acometidas em 91,11% dos casos. O número de córneas consideradas perfuradas [20/34 (58,82%)] foi maior que o de córneas íntegras [14/34 (41,17%)]. Ao 50º dia de pós-operatório, o número de olhos visuais que apresentavam córneas íntegras previamente às cirurgias (n=13) não diferiu significativamente dos olhos com córneas perfuradas (n=12) (P=0,05). Avaliações relativas ao grau de uveíte, revelaram que a metade dos casos foi considerada severa [17/34 (50%)] e na outra metade as uveítes foram consideradas discretas. Dos 17 casos que apresentaram uveíte severa, oito recuperaram a visão. Já nos 17 olhos onde a uveíte foi considerada leve, 15 mantiveram a visão ao final do período de avaliação. Apesar de a integridade da córnea não se correlacionar com a severidade da uveíte (P=0,48), constatou-se que o número de olhos visuais com uveíte discreta foi significativamente maior que os olhos com uveíte severa (P=0,006). Neste estudo, a taxa geral de sucesso visual após ECP foi de 73,52% e a integridade da córnea não exerceu impacto significativo sobre a manutenção da visão. Todavia, olhos acometidos por uveítes severas apresentaram menor chance de recuperar a visão.(AU)


The present study aimed to evaluate the results of 34 eyes of dogs presenting deep corneal ulcer (n=5), colagenolytic corneal ulcer (n=2), descemetocele (n=7), corneal perforation (n=15), and iris prolapse (n=5) that were corrected by the bulbar conjuntival pedicle graft (CPG). The uveitis score and the corneal integrity were correlated with the presence or absence of vision by contingency tables. Brachycephalic breeds accounted for 91.11% of all cases. The number of perforated corneas [20/34 (58.82%)] were higher than the non-perforated ones [14/34 (41.17%)]. At post-operative day 50, the number of visual eyes with non-perforated corneas before surgery (n=13) did not differ significantly from the eyes with perforated corneas (n=12) (P = 0.05). In half of the cases, uveitis score was considered severe [17/34 (50%)], and in the other half, mild. Eight out of 17 eyes classified with severe uveitis regained vision. In 17 eyes where uveitis score was considered mild, 16 regained vision at the end of the study. Although corneal integrity was non-correlated with the uveitis score (P = 0.48), the number of visual eyes scored with mild uveitis were significantly larger than the eyes with severe uveitis (P = 0.006). In this study, the overall success visual rate after CPG was 73,52% % and the corneal integrity did not play an important role regarding maintenance of vision. However, eyes presenting severe uveitis score had less chance to regain vision.(AU)

ABSTRACT

Adult articular chondrocytes are surrounded by a pericellular matrix (PCM) to form a chondron. The PCM is rich in hyaluronan, proteoglycans, and collagen II, and it is the exclusive location of collagen VI in articular cartilage. Collagen VI anchors the chondrocyte to the PCM. It has been suggested that co-culture of chondrons with mesenchymal stromal cells (MSCs) might enhance extracellular matrix (ECM) production. This co-culture study investigates whether MSCs help to preserve the PCM and increase ECM production. Primary bovine chondrons or chondrocytes or rat MSCs were cultured alone to establish a baseline level for ECM production. A xenogeneic co-culture monolayer model using rat MSCs (20, 50, and 80%) was established. PCM maintenance and ECM production were assessed by biochemical assays, immunofluorescence, and histological staining. Co-culture of MSCs with chondrons enhanced ECM matrix production, as compared to chondrocyte or chondron only cultures. The ratio 50:50 co-culture of MSCs and chondrons resulted in the highest increase in GAG production (18.5 ± 0.54 pg/cell at day 1 and 11 ± 0.38 pg/cell at day 7 in 50:50 co-culture versus 16.8 ± 0.61 pg/cell at day 1 and 10 ± 0.45 pg/cell at day 7 in chondron monoculture). The co-culture of MSCs with chondrons appeared to decelerate the loss of the PCM as determined by collagen VI expression, whilst the expression of high-temperature requirement serine protease A1 (HtrA1) demonstrated an inverse relationship to that of the collagen VI. Together, this implies that MSCs directly or indirectly inhibited HtrA1 activity and the co-culture of MSCs with chondrons enhanced ECM synthesis and the preservation of the PCM.

BACKGROUND: Music has been used for medicinal purposes throughout history due to its variety of physiological, psychological and social effects. OBJECTIVE: To identify the effects of prenatal music stimulation on the vital signs of pregnant women at full term, on the modification of fetal cardiac status during a fetal monitoring cardiotocograph, and on anthropometric measurements of newborns taken after birth. MATERIAL AND METHOD: A randomized controlled trial was implemented. The four hundred and nine pregnant women coming for routine prenatal care were randomized in the third trimester to receive either music (n = 204) or no music (n = 205) during a fetal monitoring cardiotocograph. All of the pregnant women were evaluated by measuring fetal cardiac status (basal fetal heart rate and fetal reactivity), vital signs before and after a fetal monitoring cardiotocograph (maternal heart rate and systolic and diastolic blood pressure), and anthropometric measurements of the newborns were taken after birth (weight, height, head circumference and chest circumference). RESULTS: The strip charts showed a significantly increased basal fetal heart rate and higher fetal reactivity, with accelerations of fetal heart rate in pregnant women with music stimulation. After the fetal monitoring cardiotocograph, a statistically significant decrease in systolic blood pressure, diastolic blood pressure and heart rate in women receiving music stimulation was observed. CONCLUSION: Music can be used as a tool which improves the vital signs of pregnant women during the third trimester, and can influence the fetus by increasing fetal heart rate and fetal reactivity.

INTRODUCTION: Disorders of the lower urinary tract are frequent in patients with multiple sclerosis throughout the course of the disease and reach variable prevalences close to 75%. It is essential to obtain an early diagnosis in the initial phases and to implement an optimal therapeutic management. Burks et al developed the Actionable Bladder Symptoms Screening Tool (ABSST) as a useful screening test in such disorders. Later, Bates et al developed a short version of the ABSST with the objective of minimising the time required to complete it and making it easier to use. AIMS: To carry out the transcultural validation into Spanish of the short version of the ABSST. PATIENTS AND METHODS: The ABSST was translated into Spanish and then back-translated into English, which confirmed the semantic equivalence. A field test was conducted on 40 patients with multiple sclerosis, with two extra questions being included at the end in order to check the comprehension and acceptability of the tool, together with a final item that asked for the time spent on completing it. RESULTS: Forty patients were selected In accordance with the eligibility and exclusion criteria; 67.5% of them were females and the overall mean age was 46.2 years. The rate of comprehension of the test was 100%, and that of acceptability was 97.5%. Results showed that 57.5% obtained scores >= 3, and an average of 5.33 minutes were spent on completing it. CONCLUSIONS: As a brief screening questionnaire for urinary disorders in multiple sclerosis, the ABSST is a useful tool for detecting them at an early stage and has now been validated for use in Spanish.


TITLE: Validacion transcultural al castellano del Actionable Bladder Symptoms Screening Tool.Introduccion. Los trastornos del tracto urinario inferior son frecuentes en pacientes con esclerosis multiple a lo largo del transcurso de la enfermedad y alcanzan prevalencias variables cercanas al 75%. Es primordial realizar un diagnostico precoz en fases tempranas y un abordaje terapeutico optimo. Burks et al elaboraron el Actionable Bladder Symptoms Screening Tool (ABSST) como herramienta de cribado util de dichos trastornos. Posteriormente, Bates et al desarrollaron una version corta del ABSST con el objetivo de minimizar el tiempo de realizacion y facilitar su manejo. Objetivo. Realizar la validacion transcultural al castellano de la version breve del ABSST. Pacientes y metodos. Se realizo la traduccion al castellano del ABSST y una posterior retrotraduccion al ingles que confirmaba su equivalencia semantica. Se llevo a cabo una prueba de campo en 40 pacientes con esclerosis multiple, incluyendo dos preguntas finales para comprobar la comprension y aceptabilidad de la herramienta y un ultimo item que recogia el tiempo empleado para su realizacion. Resultados. Se seleccionaron 40 pacientes conforme a los criterios de inclusion y exclusion; el 67,5% eran mujeres y la media global de edad era de 46,2 años. La comprension del test fue del 100%, y la aceptabilidad, del 97,5%. El 57,5% obtuvo puntuaciones >= 3, y se emplearon 5,33 minutos de media. Conclusiones. El ABSST como cuestionario de cribado breve de trastornos urinarios en la esclerosis multiple es una herramienta util para su deteccion temprana y queda validado para su uso en castellano.

ABSTRACT

OBJETIVO: Estudiar la incidencia, evolución y manejo del síndrome compartimental abdominal (SCA) en los aneurismas de aorta infrarrenal rotos (AAAr) tratados con endoprótesis en nuestro centro. Material y metodología: Estudio observacional retrospectivo. Analizamos todos los pacientes tratados con endoprótesis urgentes desde octubre de 2001 a diciembre de 2015. Incluimos solo pacientes con aneurismas de aorta infrarrenal rotos, excluyendo los AAA sintomáticos. Estudiamos la incidencia, evolución y tratamiento del SCA. Establecimos diagnóstico de SCA con una presión intraabdominal medida en vejiga > 20 mmHg, más el desarrollo de una nueva disfunción orgánica. Analizamos variables preoperatorias y postoperatorias para SCA. Utilizamos test exacto de Fisher y U de Mann-Whitney para el análisis estadístico. RESULTADOS: Durante el periodo analizado se implantaron 65 endoprótesis urgentes, de ellas 42 fueron por aneurismas de aorta infrarrenal rotos. La edad media fue 77 ± 8 años; el 98% fueron hombres. De los 42 pacientes estudiados, 9 (21,4%) fallecieron en quirófano. La incidencia de SCA en los 33 pacientes restantes fue 18,2% (6/33 pacientes). La mortalidad en SCA fue 66,7 frente 18,5% para los que no desarrollaron SCA (p = 0,034). Se realizó laparotomía descompresiva en 4 pacientes (4/6), siendo la mortalidad en estos pacientes del 50 frente al 100% para los que no se realizó laparotomía descompresiva (p = 0,4). Se asociaron con desarrollo de SCA la acidosis pre- y postoperatoria (57,7 vs. 7,7%, p = 0,01), coagulopatía preoperatoria (41,7 vs. 4,8%, p = 0,01), tensión sistólica <90mmHg preoperatoria (46,2 vs. 0%, p = 0,002) y administración > 3 litros de cristaloides/24 h en postoperatorio (38,5 vs. 5%, p = 0,02). Los pacientes con SCA recibieron intraoperatoriamente 8 ± 3,6 concentrados de hematíes vs. 2 ± 2,7 (p = 0,002). CONCLUSIONES: Estos hallazgos confirman que el SCA es una patología frecuente que asocia una elevada mortalidad en pacientes con AAAr tratados con endoprótesis. El compromiso hemodinámico importante y la administración de grandes cantidades de cristaloides intravenosos y politransfusión durante el manejo asocian mayor incidencia de este síndrome


OBJECTIVE: To study the incidence, evolution and management of abdominal compartment syndrome (ACS) in ruptured abdominal aortic aneurysms (RAAA) after endovascular aneurysm repair. MATERIAL AND METHODS: Retrospective observational study. We analyzed all emergency EVAR performed from October 2001 to December 2015. Only RAAA were included. Symptomatic aortic aneurysms were excluded. We studied the incidence, evolution and management of ACS. Diagnosis of ACS was established with intraabdominal pressure more than 20 mmHg plus the development of a new organic dysfunction. Preoperative and postoperative variables were analyzed for ACS. Fisher exact test and Mann-Whitney U-test were used for the statistical analysis. RESULTS: Sixty-five emergency EVAR were performed during the analyzed period, 42 were RAAA. The mean age was 77 ± 8 years; 98% were men. Nine of 42 patients (21.4%) died in the operating room. The incidence of ACS in the 33 remaining patients was 18.2% (6/33 patients). The mortality for ACS was 66.7% vs 18.5% (p = 0.034). Decompressive laparotomy was performed in 4 patients (4/6). The mortality after decompressive laparotomy was 50% vs 100% (p = 0.4). The incidence of ACS for postoperative acidosis was 57.7% vs 7.7% (p = 0.01), for preoperative coagulopathy 41.7% vs 4.8% (p = 0.01), for preoperative systolic blood pressure < 90 mmHg 46.2% vs 0% (p = 0.002). The incidence of ACS after overload of more than 3 liters of crystalloids per 24 hours in the postoperative period was 38.5% vs 5% (p = 0.02). Patients with ACS received intraoperatively 8 ± 3.6 packed erythrocytes vs 2 ± 2.7 (p = 0.002). CONCLUSIONS: These findings confirm that ACS is a common disease that is associated with a high mortality in patients with RAAA after EVAR. Patients with a compromise hemodynamic status, multiple transfusions and those with overload of intravenous crystalloids during management were associated with higher incidence of ACS

BACKGROUND: African swine fever (ASF) is a viral infectious disease of domestic and wild suids of all breeds and ages, causing a wide range of hemorrhagic syndromes and frequently characterized by high mortality. The disease is endemic in Sub-Saharan Africa and Sardinia. Since 2007, it has also been present in different countries of Eastern Europe, where control measures have not been effective so far. The continued spread poses a serious threat to the swine industry worldwide. In the absence of vaccine, early detection of infected animals is of paramount importance for control of the outbreak, to prevent the transmission of the virus to healthy animals and subsequent spreading of the disease. Current laboratory diagnosis is mainly based on virological methods (antigen and genome detection) and serodiagnosis. RESULTS: In the present work, a Lateral Flow Assay (LFA) for antigen detection has been developed and evaluated. The test is based on the use of a MAb against VP72 protein of ASFV, the major viral capsid protein and highly immunogenic. First experiments using VP72 viral and recombinant protein or inactivated culture virus showed promising results with a sensitivity similar to that of a commercially available Antigen-ELISA. Moreover, these strips were tested with blood from experimentally infected pigs and field animals and the results compared with those of PCR and Antigen-ELISA. For the experimentally infected samples, there was an excellent correlation between the LFA and the ELISA, while the PCR always showed to be more sensitive (38 % positive samples by PCR versus 27 % by LFA). The LFA was demonstrated to be positive for animals with circulating virus levels exceeding 10(4) HAU. With the field samples, once again, the PCR detected more positives than either the Antigen-ELISA or LFA, although here the number of positive samples scored by the LFA exceeded the values obtained with the Antigen-ELISA, showing 60 % positivity vs 48 % for the ELISA. For the two groups of sera, the specificity was close to 100 % indicating that hardly any false positive samples were found. CONCLUSIONS: The newly developed LFA allows rapid and reliable detection of ASFV, at field and laboratory level, providing a new useful tool for control programs and in situations where laboratory support and skilled personnel are limited.

ABSTRACT

Morphogenesis protein C (MorC) of Aggregatibacter actinomycetemcomitans is important for maintaining the membrane morphology and integrity of the cell envelope of this oral pathogen. The MorC sequence and operon organization were found to be conserved in Gammaproteobacteria, based on a bioinformatic analysis of 435 sequences from representative organisms. Functional conservation of MorC was investigated using an A. actinomycetemcomitans morC mutant as a model system to express MorC homologs from four phylogenetically diverse representatives of the Gammaproteobacteria: Haemophilus influenzae, Escherichia coli, Pseudomonas aeruginosa, and Moraxella catarrhalis. The A. actinomycetemcomitans strains expressing the homologous proteins were assessed for sensitivity to bile salts, leukotoxin secretion, autoaggregation and membrane morphology. MorC from the most closely related organism (H. influenzae) was functionally identical to MorC from A. actinomycetemcomitans. However, the genes from more distantly related organisms restored some but not all A. actinomycetemcomitans mutant phenotypes. In addition, deletion mutagenesis indicated that the most conserved portion of the protein, the C-terminus DUF490 domain, was necessary to maintain the integrity of the membrane. Deletion of the last 10 amino acids of this domain of the A. actinomycetemcomitans MorC protein was sufficient to disrupt membrane stability and leukotoxin secretion. The data suggest that the MorC sequence is functionally conserved across Gammaproteobacteria and the C-terminus of the protein is essential for maintaining membrane physiology.

ABSTRACT

La transferencia de colgajos libres es una herramienta fundamental dentro del arsenal reconstructivo para el tratamiento quirúrgico del gran quemado. Durante años ha existido controversia sobre la influencia del momento de la cirugía sobre la tasa de complicaciones, así como el alto riesgo de complicaciones al realizar reconstrucciones con colgajos libres en pacientes quemados. Diseñamos un estudio retrospectivo de las reconstrucciones quirúrgicas mediante colgajos libres realizadas en nuestra Unidad de Quemados del Hospital Universitari y Politecnic La Fe, de Valencia (España) en el periodo comprendido entre 2001 y 2013, recogiendo todos los datos relativos al paciente, a la lesión, al procedimiento reconstructivo, a los resultados y relacionando todo ello con el momento en el cual se realizó la cirugía. Para el análisis de los datos y asociaciones entre distintas variables usamos tablas de contingencia y aplicamos el test de independencia de Chi-cuadrado. Exponemos además 2 casos clínicos representativos de la cirugía reconstructiva practicada en pacientes quemados. Recogimos en total datos de 21 procedimientos de reconstrucción mediante colgajo libre en pacientes quemados. En ninguno se realizó reconstrucción primaria inmediata (en los primeros 5 días), mientras que la reconstrucción primaria temprana (entre el día 5 y el 21) se realizó en el 28,57% de los casos, la primaria intermedia (entre el día 21 y las 6 semanas) en el 28,57%, la secundaria (más de 6 semanas) en el 9,5%, y la cirugía de secuelas en el 33,33% de los casos. Las tasas de complicaciones mayores y menores fueron respectivamente 19,05% y 9,5%. El 50% de complicaciones mayores ocurrieron en el periodo de reconstrucción primaria temprana, mientras que no registramos ninguna en el periodo primario intermedio. El resto de complicaciones se repartió a partes iguales entre el periodo secundario y la cirugía de secuelas (ambas con el 25%). No encontramos asociación estadísticamente significativa entre las variables en ninguna de las comparaciones realizadas. En conclusión, creemos que la reconstrucción mediante colgajos libres en pacientes quemados presenta un riesgo más elevado de complicaciones mayores cuando se realiza en el periodo primario temprano (entre los días 5 y 21 tras la quemadura) (AU)


Free flap transfer is an essential tool within the reconstructive repertoire for the surgical treatment of major burn patients. For a long time there has been controversy about the influence of the timing over the complication rate, as well as the increased complication risk when we perform reconstruction by means of free flaps in burn patients. We designed a retrospective study about free flap reconstructions performed in our Burns Unit at the Hospital Universitari y Politecnic La Fe, Valencia (Spain), in the period from 2001 to 2013, collecting data related to the patient, the lesion, the reconstructive procedure, the results, and all this data related to timing. For the data analysis and associations between the different variables we used contingency tables and applied the Chi-square independence test. In addition we present 2 representative clinical reports about burn reconstructive surgery. Data from 21 reconstructive procedures with free flaps in burn patients were collected. Immediate primary reconstruction (in the first 5 days) is not performed in any patient, while early primary reconstruction (from day 5 to 21) was performed in 28,57% of cases; the intermediate primary reconstruction (from day 21 to 6th week) was performed in 28,57%, the secondary reconstruction (more than 6 weeks) in 9,5% and sequelae surgery in 33,33% of the cases. Major and minor complication rate were 19,05% and 9,5% respectively; 50% of major complications happened in early primary reconstruction period, whereas no complication is registered in intermediate primary period, the rest of complications are equally divided between secondary reconstruction period and sequelae surgery (both of them 25%). No statistically significant association between the variables is found in any of the comparisons performed. In conclusion, we think that reconstruction with free flaps applied to burn patients presents a higher major complication rate when performed in the early primary period (from day 5 to day 21 after the burn) (AU)

ABSTRACT

Objetivos. El objetivo principal de este estudio es evaluar la efectividad y seguridad de vareniclina como tratamiento para dejar de fumar en un grupo de fumadores con enfermedad cardiovascular (EVC), en la práctica clínica. Material y métodos. Se ha revisado nuestra base de datos y se han incluido las historias clínicas que consignan ECV estable (>2 meses). Se trata de un estudio descriptivo, longitudinal con recogida retrospectiva de los datos. Se han incorporado a 180 pacientes todos tratados con vareniclina, a dosis estándar de 1 mg cada 12 h, durante 12 semanas, más apoyo psicológico y material de autoayuda. En total se llevaron a cabo 7 visitas (Basal, 2ª y 4ª semanas; 2º, 3º y 6º mes, y, finalmente, a la 52ª semana). Tambien se realizaron llamadas telefónicas, la manifestación verbal de abstinencia se confirmó con la medición del CO en aire espirado (<10 ppm). Resultados. La edad media de los participantes fue de 58 años, con predominio de varones (78%). Edad media de inicio 15,5 años, consumo medio de cigarrillos 25/día. Media en el test de Richmond 8,1; y en el test Fagerström 7,5 puntos. El 83% encendía el 1er cigarrillo en menos de media hora. Las tasas de abstinencia a las 12 s (52%); a las 24 s (44%) y a las 52 s (37%). La tasa de abandonos debido a la presencia de efectos adversos fue del 7,6%. Los efectos indeseados más frecuentes fueron, respectivamente: náuseas, cefalea, insomnio, vómitos y sueños anormales. Conclusiones. Los participantes presentan un consumo elevado de cigarrillos/día y una severa dependencia. Alta motivación, con elevada puntuación en el test de Richmond. Vareniclina se ha mostrado como un tratamiento farmacológico efectivo para conseguir el cese en fumadores con EVC estable, y el fármaco es, generalmente, bien tolerado, presentando un aceptable perfil de seguridad en el paciente con ECV estable (AU)


Objectives. The principal objective of this study is to evaluate effectiveness and safety of Varenicline as a smoking cessation treatment in a group of smokers with cardiovascular disease (CVD) in the clinical practice. Material and methods. A review was made of our database and the clinical records that achieved stable CVD (>2 months) were included. This is a descriptive, longitudinal study with retrospective collection of the data. A total of 180 patients, all treated with varenicline, at standard dose of 1 mg every 12 h, for 12 weeks, plus psychological support and self-help material were incorporated. In all, 7 visits (baseline, 2nd and 4th weeks; 2nd, 3rd and 6th month and finally at 52 weeks) were carried out. Telephone calls were also made; the oral manifestation of abstinence was confirmed with measurement of CO in expired air (<10 ppm). Results. Mean age of the participants was 58 years, with predominance of males (78%). Mean age at onset was 15.5 years, mean consumption of cigarettes 25/day. Mean on the Richmond test 8.1 and on the Fagerstrom test 7.5 points. A total of 83% smoked the 1st cigarette in less than half an hour. Abstinence rates at 12 w (52%); at 24 w (44%) and at 52 w (37%). Drop-out rate due to presence of adverse effects was 7.6%. The most frequent undesired effects were, respectively: nauseas, headache, insomnia, vomits and unusual dreams. Conclusions. The participants had an elevated use of cigarettes/day and severe dependence. High motivation, with high score on the Richmond test. Varenicline has proven to be an effective drug treatment to achieve smoking cessation in smokers with stable CVD, and the drug is generally well tolerated, with an acceptable safety profile in patients with stable CVD (AU)

ABSTRACT

OBJECTIVE: The objective of this study was to compare hand-transcribed oxygen saturation (SpO2) with electronic oximeter data in very low birth weight infants (VLBWI, <1500 g). STUDY DESIGN: Oximeter data were downloaded from birth through 36 weeks postmenstrual age (PMA) for VLBWI before and after interventions to improve neonatal intensive care unit oxygen management. Transcribed SpO2 values were obtained by chart review. Proportions of transcribed and oximetry data in target (85 to 93%), hypoxemic (80 to 84%), and hyperoxemic (≥98%) ranges before and after intervention were compared. RESULT: There were 30,441 oximetry hours before intervention and 54,538 oximetry hours after intervention. Transcribed SpO2 values correlated strongly with oximeter overall. However, during hours on supplemental oxygen, transcribed values significantly overdocumented target range and underdocumented values 80 to 84 and ≥98%. This finding varied by respiratory support and PMA, and increased after intervention. CONCLUSION: Transcribed SpO2 values overdocumented target range and underdocumented hyperoxemic and hypoxemic ranges compared with electronic oximeter data. These results support incorporating electronic oximeter data into medical records.

ABSTRACT

El Cáncer de próstata (CaP) constituye una importante causa de muerte en nuestro país. La razón Antígeno Prostático Específico libre/ Antígeno Prostático Específico total (APE-L/T) es una de las medidas descritas para mejorar la pesquisa de la enfermedad. MATERIAL Y MÉTODOS: Se realizó un estudio retrospectivo analítico en el Hospital Naval de Viña del Mar entre enero 2007 a diciembre 2011 que incluyó 588 pacientes con biopsias prostáticas que presentaban APE- T entre 2-10 ng/ml. Se evaluó la edad al diagnóstico, el valor de APE-T y la razón APE-L/T en relación al resultado histológico de las biopsias. Se realizaron curvas de rendimiento diagnóstico (ROC) para APE- T y para la razón APE-L/T. Se calculó sensibilidad y especificidad para diferentes puntos de corte para la razón APE-L/T. RESULTADOS: 33 por ciento de las biopsias fueron positivas para CaP. Los valores de la razón APE-L/T fueron significativamente más bajos en pacientes con CaP (p<0.001), alcanzando un área bajo la curva ROC 0,615. El mejor punto de corte para la razón APE-L/T fue de 15 por ciento con una sensibilidad de 60 por ciento y una especificidad de 58 por ciento. Para la razón APE-L/T > 25 por ciento la sensibilidad es 20 por ciento y la especificidad 91 por ciento. En cambio, el APE-T no mostró diferencias estadísticamente significativas (p=0,1) con un AUC 0,55. CONCLUSIONES: El APE-T por sí solo no parece tener capacidad discriminante para detectar CaP cuando los valores se encuentran entre 2-10 ng/ml. La razón APE-L/T tiene una utilidad limitada frente al paciente para decidir efectuar o no una biopsia de próstata.


Prostate cancer (PCa) is one of the major causes of death in our country. The Free/Total Prostate specific antigen ratio (f/t PSA) is one of the measurements that have been used to improve the diagnosis of this disease. MATERIAL AND METHODS: A retrospective analytic study in the Almirante Nef Hospital in Viña del Mar between January 2007 and December 2011 was made, which included 588 patients with prostate biopsies that had tPSA between 2-10 ng/ml. The age at diagnosis was evaluated and the value of tPSA and the f/t PSA ratio were compared with the histological results of the biopsies. Curves were performed for the diagnostic yield (ROC) for tPSA and the f/t PSA ratio. The sensitivity and specificity for different cut-off points for the f/t PSA ratio in the diagnosis of PCa were also calculated. RESULTS: 33 percent of the biopsies were positive for PCa. The f/t PSA ratio values were significantly lower in patients with CaP (p<0.001), reaching an area under the curve of 0,615. The best cutoff for f/t PSA ratio was 15 percent with 60 percent sensitivity and 58 percent specificity. For f/t PSA ratio > 25 percent sensitivity was 20 percent and specificity was 91 percent. PSA showed no statistically significant differences (p = 0.1) with AUC: 0.55. CONCLUSIONS: tPSA alone does not seem to have discriminatory power to detect PCa in patients when values are between 2-10 ng/ml. The f/t PSA ratio has a limited utility to decide to perform a prostate biopsy or not.

ABSTRACT

Food powders are used in everyday life in many ways and offer technological solutions to the problem of food production. The natural origin of food powders, diversity in their chemical composition, variability of the raw materials, heterogeneity of the native structures, and physicochemical reactivity under hydrothermal stresses contribute to the complexity in their behavior. Food powder agglomeration has recently been considered according to a multiscale approach, which is followed in the chapter layout: (i) at the particle scale, by a presentation of particle properties and surface reactivity in connection with the agglomeration mechanisms, (ii) at the mechanisms scale, by describing the structuration dynamics of agglomerates, (iii) at the process scale, by a presentation of agglomeration technologies and sensors and by studying the stress transmission mode in the powder bed, and finally (iv) by an integration of the acquired knowledge, thanks to a dimensional analysis carried out at each scale.
