ABSTRACT

BACKGROUND: Critically ill patients are more vulnerable to medical adhesive-related skin injuries (MARSI), whose prevention is a constant challenge and one of the main quality indicators of nursing care. MARSI associated with indwelling urinary catheter (IUC) fixation is a relevant adverse event, mainly because of the constant involuntary traction and high skin vulnerability of the fixation site. Silicone adhesive tape has appreciable qualities for fragile skin among the range of adhesives, leading to the inference that it reduces the risk of MARSI. AIM: To compare silicone adhesive tape for IUC fixation with acrylate tape regarding its safety and efficacy. STUDY DESIGN: This was a randomized controlled trial blinded to the patients and evaluator. Data were collected from an intensive care unit (ICU) of a tertiary university hospital in Brazil. Patients with IUC and no MARSI at the fixation site were considered eligible. The omega (Ω) fixation technique was used for IUC fixation. A total of 132 participants were enrolled and divided into two research groups: 66 patients in the intervention group (silicone tape) and 66 in the control group (acrylate tape). Outcomes were the incidence of MARSI, patient outcome in the ICU and hospital and partial, total and overall spontaneous detachment of the tapes. RESULTS: The overall incidence of MARSI was 28%, with 21% in the silicone group and 35% in the acrylate group, with no statistically significant difference (p = .121), including the severity of the lesions (p = .902). However, partial (p = .003) and overall (p < .001) detachment of the tapes were more frequent in the silicone group. CONCLUSIONS: Silicone tape is no safer than acrylate tape for IUC fixation and is less adhesively effective. RELEVANCE TO CLINICAL PRACTICE: There is no evidence to support the extensive use of silicone tape in this context.

ABSTRACT

Research into consumer attitudes toward food products is important to help people adopt healthier, more sustainable diets. A positive attitude regarding an object is a prerequisite for its adoption. This study compares French consumers' implicit attitudes toward pulses and cereals. Many studies have measured attitudes by explicit methodologies (e.g., questionnaires). Such methods are often biased by social desirability, and consumers may not be consciously aware of their attitudes toward food. A Sorting Paired Feature Task measures the strength of automatic associations, pairing images of pulses or cereals and adjectives with positive or negative valence. Participants sorted 120 paired stimuli as fast as possible. Pairs composed of pulses and negative adjectives were sorted faster than pairs composed of cereals and negative adjectives. Cereals with positive adjectives were sorted more rapidly than pulses with positive adjectives. Mistaken associations were more frequent for pairs composed of cereals and negative adjectives than for pairs composed of pulses and negative adjectives. These results highlight more negative implicit attitudes toward pulses than cereals. This study provides the first potential evidence of negative implicit attitudes toward pulses, which could explain the low consumption of these products.

ABSTRACT

BACKGROUND: Reducing the blood-culture contamination rate is a constant challenge for health services. This study aimed to analyze whether blood-culture (BC) collection using sterile gloves reduces the contamination rate when compared to the non-sterile gloves, and to compare baseline and intervention periods. METHODS: A randomized clinical trial, performed in an intensive care unit in Brazil and paired in two groups: sterile (BCs obtained with modified sterile technique: only sterile gloves; no fenestrated drape or dedicated sterile collection kit) and clean (clean technique: usual care with non-sterile gloves). Two paired blood samples were obtained from each patient by trained and calibrated nurses. BCs were processed by conventional microbiological methods and the results were issued by blinded microbiologists. RESULTS: There was no difference (P = 1.00) in the contamination rate of BC between the sterile (1%) and clean (1%) groups. However, there was a significant difference (P = 0.05; relative risk: 0.17; 95% confidence interval: 0.04-0.70) in the contamination rate between baseline (6.1%; 20/330) and intervention (1%; 2/200). CONCLUSIONS: This study suggests that the aseptic care provided in obtaining samples is more important than the sterile technique itself, and highlights the value of standardizing the practices, qualification and calibration of phlebotomists.

ABSTRACT

Consumption of pulses plays a key role in achieving a healthier and more sustainable diet. Despite the increased availability of pulse-based foods on the French market, pulse consumption in France remains below recommended levels. The objective of this study was to capture French consumers' mental representations of various pulse-based foods with different levels of processing, using a Check-All-That-Apply (CATA) questionnaire with food images. We used CATA questionnaires, in two separate studies, to capture two aspects: consumers' beliefs about the products presented in the study, and the characteristics of those products, according to consumers. In Study 1, twelve images of pulses were presented (unprocessed, processed, and ready-to-eat) to investigate whether and how consumers differentiate between pulse-based products with different recipes and levels of processing. In Study 2, parallel CATA questionnaires were used: one with images of pulse-based products, and one with images of vegetables. We wanted to discover whether the factors underlying different representations for pulse-based products were specific to pulses, or were also observed with another family of food products. Results showed that products were differentiated more by their level of processing than by their main ingredient. Participants had more positive attitudes toward pulse products that were unprocessed, processed, or "new ready-to-eat", than toward other ready-to-eat pulse products (whether canned or frozen). Our results also highlighted that, in consumers' mental representations of pulse-based products, packaging format is a negligible factor in comparison with the nature of the product itself.

ABSTRACT

Spermatozoa are highly specialized cells whose fertilizing and motility functions highly depend on intracellular Ca2+ -mediated events and protein posttranslational modifications like phosphorylation. Our group previously identified PPEF1, the Ser/Thr phosphatase with EF-hand domain 1, among calmodulin-affinity pulled down sperm proteins. As the mammalian ortholog of the Drosophila phosphatase rdgC that dephosphorylates rhodopsin, PPEF1 has been studied mostly in the retina. The presence and importance of this Ca2+ /calmodulin-binding protein phosphatase has not been studied in sperm or testicular functions despite its high expression level. In this study, we show that PPEF1 is present in testicular germ cells, and in mouse, human and bull spermatozoa where it is localized predominantly in the neck and acrosome areas. Different transcript variants encoding four predicted isoforms were detected by reverse transcription polymerase chain reaction in bull testis, spermatocytes and spermatids. Phosphatase activity of immunoprecipitated sperm PPEF1 was detected using the substrate pNPP and analysis of the coimmunoprecipitated proteins reveal an enrichment in the biological processes of sperm capacitation, binding to the zona pellucida and motility. Although this is the first demonstration of the presence of PPEF1 in sperm and testicular germ cells, its involvement in sperm fertilizing ability and motility, and the mechanisms regulating its activity remain to be further investigated.

ABSTRACT

Abstract This study aimed to understand the Discursive Formations about gender related to health care of women hospitalized in gynecology and obstetrics wards in a hospital. Thirteen women participated in this study and their interviews were analyzed in the light of Michel Pêcheux discourse analysis. The main Discursive Formations identified showed the naturalization of the way women should relate to the health system based on what is traditionally delegated to this gender (mother and caregiver), prioritizing reproductive health interventions, and the hegemonic matrix of the biomedical model in the organization of these services, pushing them away from the models of integrality proposed in public health policies. The health system was considered a discursive space for (re)production of gender stereotypes and inequalities based on Ideological and Imaginary Formations that are constituents of the ways in which these subjects relate in this scenario. It is proposed to understand gender issues as a crosscutting content in all health areas and contexts, starting from the perspective that a better understanding of the female universe regarding health care, based on gender issues, can improve the offer of programs and services more consistent with the real demands and needs of this population.(AU)

ABSTRACT

The scope of this research was to characterize and quantify the perforating waste produced domestically by people with diabetes mellitus (DM), as well as to analyze the discard and disposal practices for these materials. The data collection was performed through a questionnaire applied to primary care patients. The sample consisted of 149 people, the majority of whom were elderly and with a low level of schooling. Regarding the generation of waste, 88.64% of the deponents declared the use of one syringe per day, performing 1 to 4 applications. The reuse of the material was reported by 66.67% of the declarants. After being used, the waste was stored - with higher prevalence - in plastic bags (20.8%), plastic bottles, clothes softener and mayonnaise containers, etc. (46.98%). Ordinary household waste was the main disposal site for the perforating waste. Furthermore, it was revealed that those people who did not receive instructions regarding appropriate waste disposal discarded their waste incorrectly. Based on the results, the conclusion drawn is that the diabetics who participated in this study fail to discard and dispose of the perforating waste appropriately due to the lack of adequate guidelines and public programs.

ABSTRACT

The aim of this research was to understand the discursive formations (DF) about gender and health care of hospitalized men (in the urology sector). Seven men participated in this study and the collected data was analyzed in the light of Michel Pêcheux Discourse Analysis. Two DF were identified (hegemonic masculinity; Unsaid about men health) and highlighted: traditionalism and gender stereotypes as guiding the participants' discourses and practices based on an Ideological Formation that prescribes different rules of conduct for the genders, among them health care. Thus, it is emphasized the urgency to mobilize the exercise of self-care through prevention and promotion of health. It is also important to invest in institutional spaces for discussion and professional training, aiming to guarantee the rights and the recognition of this segment of the population as a locus of care.

ABSTRACT

Pulses present many advantages for human health, nutrition, sustainability, and the environment. Despite efforts in recent years by the pulse industry and national authorities to favor pulses, consumption in France remains relatively low, at 1.7 kg/per person in 2016, compared to 1920 when it was around 7.2 kg/per person. To understand social representations of pulses in France, 80 French nonvegetarian consumers and 35 professionals from the pulse industry were asked to say five words spontaneously evoked by the inductor "pulses". They then had to rank these five words in order of importance and rate their valence. The structural approach was used to analyze social representations for each group independently. Our results highlight differences in the structure and content of social representations for pulses. Consumer responses suggested only vague impressions of pulses, but taste evocations were nevertheless rated positively. By contrast, professionals tended to focus specifically on protein content and culinary preparation. These differences could explain some barriers to pulse consumption, and improved communication should be a key target. Efficient communication must consider the concepts most frequently used by consumers when referring to pulses, and those ranked as most important.

ABSTRACT

Calmodulin is a small, highly conserved acidic protein present at high levels in spermatozoa that mediates numerous intracellular Ca2+ -dependent events. Sperm motility and fertilizing ability results from an array of biochemical pathways under Ca2+ control, in which the importance of calmodulin is not fully understood. The role of calmodulin in sperm function has been mostly assessed using antagonists. Nevertheless, few known calmodulin-regulated enzymes have been described in spermatozoa regarding their involvement in sperm function. To further understand the role of this important Ca2+ mediator in spermatozoa, different studies were also undertaken to investigate and to identify sperm calmodulin-binding proteins and determine their localization and subcellular distribution as an attempt to elucidate the role of this important Ca2+ mediator. In the present study, sperm calmodulin-binding proteins were identified by mass spectrometry after Ca2+ -dependent biotinylated-calmodulin binding on sperm head proteins subjected to 2D electrophoresis and transferred on a polyvinylidene difluoride membrane. Calmodulin binding protein identification was also done on detergent extracted whole sperm proteins pulled down in a Ca2+ -dependent manner by calmodulin-conjugated sepharose beads. In this latter group, 300 proteins were identified in at least two experiments out of three, and those identified in the three independent experiments were analyzed for overrepresented biological processes using the Bos taurus Gene Ontology database. Proteins with known function in reproductive processes, fertilization, sperm-egg recognition, sperm binding to the zona pellucida, regulation of sperm capacitation, and sperm motility were identified and further emphasize the importance of calmodulin in sperm function.

ABSTRACT

Over time, meat has acquired a central place in French gastronomy. The role played by plant-based proteins, such as pulses, is less clear. In order to better understand it, this study seeks to identify how French non-vegetarian consumers structure their main dish, using an indirect approach. A total of 120 participants had to compose dishes, following six different scenarios, by selecting three images of food items out of the twenty proposed. Results provided information about: (1) the first food product chosen, and (2) how foods from different food-groups were associated. Our results indicated that (1) French non-vegetarian consumers generally constructed their main dishes using animal-based food products first. Some differences were nevertheless identified, in relation to the scenario presented, and the profile of the participants. Results also showed that (2) the food-groups most often associated within a dish were meat, starch, and vegetable. The pulse group, when used, was in general associated with the meat and vegetable food-groups. These findings provide new evidence of the structure of the French main dish. They also highlight possible levers that could be used to promote pulses to French consumers.

ABSTRACT

Pulses present nutritional advantages for human health, and also contribute to food security and environmental sustainability. Despite these beneficial properties, the consumption of pulses in France has decreased over the past century. This study explores the representation of pulses among French non-vegetarian consumers, using both an indirect approach, with scenarios evoking real-life situations, and a direct approach, with an online questionnaire. For the indirect approach, based on six different scenarios, the 120 participants composed dishes by selecting three food images out of the twenty provided. One week later, for the direct approach, the same participants completed a questionnaire, with ten questions about food products, using the same images. Results showed that spontaneous selection of pulses was low for all dish-composition situations, coherent with the low consumption of pulses declared in the questionnaire. Among possible barriers to pulse consumption were dislike of pulses, preparation difficulty, and perception of pulses as a food for vegetarians. Participants possessed theoretical knowledge about the benefits of pulses, but this knowledge seemed not to be applied to dish-composition situations. Results also highlighted the complementarity of these two methods, providing deeper understanding of consumer food habits in relation to pulses. Based on these findings, several levers are proposed to increase pulse consumption: improving familiarity with pulses, using the context of consumption, formulating trendy and innovative products, and promoting pulses as a "food for all".

ABSTRACT

Abstract The present study aimed to understand how the gender category emerges in studies about adoption in the Brazilian context. The guiding question was: How do researches in the context of adoption, regarding children, parents and professionals, address gender issues? An integrative literature review was conducted on the databases LILACS, SciELO and PePISC with the descriptors adoption, gender and parentality, published between 2007 and 2017. Seventeen articles had been recovered and three categories were constituted: (a) Exercise of maternity in the context of adoption; (b) (In)visibility of paternity in the discussion about adoptive parenting; (c) Homoparentality: meanings of reproductive technology and adoption. The studies brought to light reflections about adoptive parenting and adoptive homoparentality, as well as highlighting the way these couples exercise their parental roles. It is concluded that understanding how parental roles are exercised in these different realities makes it possible to understand different family arrangements, opening more space for families by adoption. It is recommended to expand the gender discussions in order to provide questions and tensions for this scenario, dialoguing with different developmental and clinical perspectives.

ABSTRACT

ABSTRACT

Total proteins and esterases from silk gland extracts of Bombyx mori silkworm were characterized and compared with electrophoretic profiles of prepared extracts with silkworm glands infected with nucleopolyhedrovirus (BmNPV). SDS-PAGE (7%) gels was used for the total proteins, and 10% native PAGE for esterases. In the silk glands extracts of healthy silkworms, it was observed seven protein zones with molecular weight varying between 10 kDa (P1) and 60 kDa (P7). In the infected silkworms, a new zone named P8 (90 kDa) was also detected. Esterases activity at 5th instar larvae underwent changes after the infection with BmNPV, since there was a reduction (EST-6 and EST-7) and an increase (EST-8) in the intensity of the regions of esterases activity, and specificity of EST-9 to ß-naphthyl acetate. Those alterations observed in the expression of genes after the infection with the nucleoplyhedrovirus can be used as markers to detect infections in B. mori.

ABSTRACT

Objective: collect the experience of patients hospitalized for heart failure in order to develop an educational proposal to promote healthy lifestyles and self-care styles in this population. Methods: phenomenological type, qualitative study addressing 19 patients through an in depth interview, which included patients hospitalized in general wards at a third complexity level, with the diagnosis of congestive heart failure. Results: 19 patients interviewed, 68% (13) were women, average age was of 67.8 years. The most representative risk factor in the population was arterial hypertension. It was noted that patients clearly stated to be heart-diseased and largely attributed its origin, to social causes, and some, to inheritance. They also recognized healthy lifestyles; however healthy lifestyles werenot practiced. Conclusions: Heart failure changes the dynamics of life in people suffering from it, while limiting daily activities and has repercussions on the emotional state and family life.

ABSTRACT

This article discusses the the emergence of shared custody in Brazil, establishing a parallel with the uni-parental custody and custody alternating. It also highlight the consequences for the child, arising from alternation. To examine the alternation situations the child experiences (whether as a result of alternating custody, or in shared custody, but regulated with alternating housing), D.W. Winnicott’s concept of continuity of being is used.
