ABSTRACT

This is a case report of a papillary thyroid carcinoma metastatic to the parapharyngeal space, that presented with local obstructive symptoms (dysphagia and displacement of the ipsilateral tonsil). The diagnosis was suspected by imaging studies (CT and MRI) but not confirmed until histological examination. It is not common for such tumors to metastasize to the parapharyngeal space. This is the reason why, to our knowledge, there have been described only five similar cases previously.

ABSTRACT

Se presenta un caso de metástasis de carcinoma papilar de tiroides de localización en el espacio parafaríngeo con clínica inicial de características obstructivas a nivel locorregional (disfagia progresiva y desviación contralateral de la amígdala correspondiente). En el estudio de imagen (TAC y RNM), la masa adoptaba una morfología especial, pero el diagnóstico no se confirmó hasta que se realizó el estudio anatomopatológico. Este espacio no es un lugar frecuente de asiento de metástasis ni de carcinomas tiroideos ni del resto de tumores de cabeza y cuello, por lo que, hasta la fecha, sólo se han descrito 5 casos similares en la literatura médica

ABSTRACT

The Rendu-Osler-Weber disease is an hereditary hemorrhagic dysfunction with muco-cutaneous telangiectasia of diverse localization, being able to internal organs and committing the life in some cases, not alone for the derived systemic complications, but for the quantity of the recurrent nosebleed that are the most frequent feature and sometimes restrictive that these patients present. We report two family groups with multidisciplinary diagnosis, and analyze them most important ENT aspects for this syndrome.

ABSTRACT

The internal jugular vein thrombosis is an infrequent vascular complication associated in the past to severe infections of the head and neck. We report a case with unknown etiology and make a review on the subject discussing on etiology, clinical features, diagnosis and treatment.

ABSTRACT

La trombosis de la vena yugular interna es un proceso grave e infrecuente. La infección de los espacios profundos de cabeza y cuello ha desaparecido como factor predisponente desde la llegada de la terapéutica antibiótica, dando paso a una pluralidad causal, entre las que destacan las neoplasias ORL y la etiología yatrógena. Su clínica solapada y sus complicaciones mortales, hacen necesario un diagnóstico y tratamiento precoz, con lo que mejora sustancialmente el pronóstico. El motivo de esta revisión es destacar la importancia de unos criterios terapéuticos claros en el abordaje de esta rara entidad (AU)


The internal jugular vein thrombosis is a rare and critical disease. The infection of the head and neck deep spaces it is not the main cause since the antibiotic era. The ENT neoplasms and iatrogenic etiology are more frequent. Anodyne symptoms and vital complications are important to obtain an early diagnose and therapy, to improve the prognosis. The objective of this review is to obtain a certainly therapeutic criteria in the approach to this entity (AU)

ABSTRACT

El divertículo de Zenker es una lesión esofágica causante de disfagia alta originada por la protusión de la mucosa faríngea a través de una solución de continuidad en el conocido triángulo de Lainert. Existen múltiples teorías etiopatogénicas que coinciden en el carácter adquirido del mismo, atribuyendo a una disnergia faringo esfinteriana el origen del saco. Presentamos el caso de un paciente intervenido de laringuectomía total con disfagia progresiva a sólidos en el que se descubre un divertículo hipofaríngeo. Se analizan los factores implicados en su aparición y se exponen las características definitorias de la enfermedad. Se realiza una revisión bibliográfica, siendo excepcional el hallazgo de casos de divertículos con manifestaciones clínicas en pacientes laringuectomizados (AU)


Zenker's diverticulum is a mucosal lined outpouching of pharynx through Lainert's space that causes dysphagia of the upper digestive tract. Multiples theories try to explain the acquired etiology of this entity, attributing its origin to a disfunction of pharynx-esophageal sphincter. A case of total larynguectomy with hypopharyngeal diverticulum and progressive dysphagia to solid food is presented. We analyze the etiopathogenic mechanisms and the definitive characteristics of this entity. We review mundial literature, being exceptional the fact of finding clinical manifestations in diverticulum of larynguectomized patients (AU)

El adenoma pleomorfo es el tumor benigno más común de todas las neoplasias de glándulas salivares, representando el 65 por ciento de todos los tumores de estas localizaciones, afectando sobre todo a la parótida. Puede aparecer también en otras glándulas salivares mayores y menores del tracto aerodigestivo superior.Presentamos dos casos de adenomas localizados en tabique nasal y paladar blando, señalando las características clínicas y diagnósticas de nuestros pacientes, así como la terapéutica aplicada.Exponemos los aspectos más importantes de esta entidad en el momento actual (AU)
