ABSTRACT

Based on theoretical calculations, biomechanical tests, and clinical trials, new types of dynamic distractors are proposed to treat scoliosis in children. The specific feature of the designed devices is that they can transform spinal rotatory motions of the spine and move apart the reference elements of the distractor, which rules out the need for supplementary surgical interventions and reduces the incidence of complications.

ABSTRACT

Clinical and experimental researches were conducted to study the possibilities for enhancing the efficiency of treatment of open composite comminuted and crushed fractures of long bones with the help of repercussive action of electrets. The results of roentgenomorphometric, clinical and biomechanical researches have shown the efficiency of electrets that could shorten the healing time of the infected wounds, stop the inflammatory reaction and promote the repairing process of bones. The application of electrets using the repercussive methods in complex treatment of complicated open fractures in cases with multiorgan and concomitant injuries creates favourable conditions for the healing process. This method is simple, atraumatic and economical.

ABSTRACT

970 laboratory experiments, 202 chronic experiments on animals and 102 clinic observations were performed to study the possibilities of sorbents to enhance the efficiency of surgical treatment in gunshot wounds of the extremities. Gravimetric, climatic, morphometrical, microbiological, spectrofluorometric, clinical and roentgenological methods of studies were used for this matter. It was ascertained that the sorbent application contributed to the optimization of the wound process. The studies have proved the advantages of composite sorbent with biologically active supplements: it has a high sorption capacity, pronounced bacteriostatic and local-anaesthetic features.

ABSTRACT

It was found out during the study of multichannel electromyostimulation (EMS) influence upon human organism and the adequacy of its physical load to adoptive and compensative possibilities of cardiovascular system that that method was effective for prophylaxis of disorders and rehabilitation of nervous and muscular system, and locomotor apparatus after a short period of hypodynamia. The EMS increases the working capacity of nervous and muscular apparatus, mass of muscles and their contraction strength due to improvement of nervous and muscular conductivity and blood supply of the extremity. The efficiency of EMS decreases considerably when degenerative changes are taking place.

ABSTRACT

The schemes were worked out for optimization of complex physiotherapy in rehabilitation system of patients with complicated fractures of extremities. This work was carried out on the basis of 178 dynamic clinic observations with application of biomechanical and roentgenological methods, as well as methods of mathematical modelling and prognostication. That made it possible to increase the efficiency of rehabilitation programs by 3.2--4.1 times. The article deals with the perspective trends in the development of this problem.

ABSTRACT

With the goal of improvement of intramedullary osteosynthesis efficiency in treatment of composite and associated fractures of long bones by way of improvement of fixatives according to their electrical properties and optimization of reparative electrogenesis conditions in 72 experiments with animals the effect of electro-conductive properties and electrostatic field of intra-bone implanted rods on the osteo-reparation following transverse osteotomy has been studied, the method has been elaborated and on the basis of dynamic analysis of 147 clinical observations has been given comparative evaluation of the results of polytetrafluorethylene intramedullary osteosynthesis. The advantages ethylene-based electrets application in case of intramedullary osteosynthesis. The advantages of electret application for activation of osteo-reparation, improvement of the results of treatment and reduction of terms of rehabilitation of victims with critical traumata have been demonstrated.

ABSTRACT

The authors present their results of treatment of coxarthrosis of various etiology (dysplastic, idiopathic, posttraumatic and aseptic necrosis of the femoral head) by means of an operation of denervation, decompression and fenestration on 35 hip joints in 31 patients with the follow-up period up to 6 years. The proposed operation is especially effective at the early stages of development of coxarthrosis; in the patients with aseptic necrosis of the femoral head the prognosticated remission may reach 10 years. Low traumatism and short duration of the operation with the preservation of the anatomic unity of the femur and the possibility of early activization allows to recommend this method for the treatment of idiopathic coxarthrosis in the elderly people with associated diseases when conservative treatment is hopeless and an extension of surgical intervention is impossible.

ABSTRACT

The authors substantiate the expediency of improving osteosynthesis with apparatuses of perosseous external fixation on the basis of electromechanical principles making use of statistically reliable results of 58 chronic experiments on animals, 27 stand studies on the tibia of cadavers and 207 dynamic clinical observations employing quantitative results of clinical, roentgenomorphometric, electrophysiologic, biomechanical and histomorphological studies. Optimal parameters of the rigidity of fixation for early restoration of the properties of elasticity and strength of the injured bone are determined. Dampered external osteosynthesis in the treatment of open comminuted fractures of the bones of the crus reduced the mean periods of in-patient treatment and consolidation of the fractures by 27-34. 5% and the frequency of complications and unsatisfactory outcomes by 75%.

ABSTRACT

The authors carried out dynamic observation of 180 children aged 3-14 with infantile cerebral paralysis (ICP) who were treated for pronation flexion contractures of the forearm and of the hand, studied up to 45 values connected with the age of the patients, terms of appearance of the symptoms of the disease, subsequently processed the results of their investigations employing methods of analysis of variance, non-linear correlation and regression analysis and constructed analytical models of development of the pathologic process, which allowed to draw the following conclusions. Pathomechanogenesis of disturbances of the upper extremity in ICP includes primary muscular disbalance, successive formation of faulty postures, pathologic locomotor stereotype, vegetotrophic disturbances, contractures of the joint and anatomical changes of the skeleton. These changes are closely correlated with the age of the patients and according to the degree of the disturbances may be regarded as stages of the disease.

ABSTRACT

Experimental studies in 60 rabbits with defects of the radius have shown that exposure to oxygenobarotherapy results in the greatest activization of osteoreparation in the early period after trauma with the formation of osseous matrix. The intensity of OBT sessions should be reduced along with the "maturation" of the osseous regenerate. The clinical application of OBT in 33 patients with defects of long tubular bones contributed to shortening the terms of rehabilitation of such patients.
