ABSTRACT

BACKGROUND: The Communication Skills Scale (CSS) is an instrument for evaluating communication skills in different health professionals; however, a differential analysis of their psychometric properties by professions and specific scales has not been performed. Therefore, the aim of the present study was to analyze the psychometric properties of the CSS and develop T-scores for the scale for nursing professionals. MATERIAL AND METHODS: We developed an instrumental study to analyze the psychometric properties of the CSS, evaluating its items, reliability, evidence of validity and its internal structure. For this purpose, a battery of questionnaires was administered, which included the CSS, the General Self-efficacy Scale and the Specific Self-efficacy Scale for communication in difficult situations. RESULTS: The questionnaires were applied to 692 nurses. Discrimination indices >0,30 were obtained in all items. The confirmatory factor analysis of the two models (with four oblique factors and adding a second-order factor) revealed that all items had adequate factor loadings and the fit indices of both models were good. The internal consistency was 0,88 for the total scale and between 0,70 and 0,77 for the dimensions. A positive relationship between communication skills and general and specific self-efficacy was obtained. CONCLUSION: The CSS displayed appropriate psychometric properties (item analysis, internal structure, reliability and evidence of validity), relating the communication skills of nurses with their general and specific self-efficacy in communication in difficult situations.

FUNDAMENTO: La Escala sobre Habilidades de Comunicación (EHC) es un instrumento para evaluar las habilidades de comunicación en diferentes profesionales sanitarios; sin embargo, no se ha realizado ni un análisis diferencial de sus propiedades psicométricas por profesiones, ni baremos específicos. Por ello, el objetivo del presente estudio fue analizar las propiedades psicométricas de la EHC y elaborar un baremo en profesionales de Enfermería. MATERIAL Y MÉTODOS: Estudio instrumental para analizar las propiedades psicométricas de la EHC, evaluándose los ítems, la fiabilidad, las evidencias de validez y su estructura interna. Para ello, se administró una batería de cuestionarios que incluía la EHC, la Escala de Autoeficacia General y la Escala de Autoeficacia Específica para la comunicación en situaciones difíciles. RESULTADOS: Los cuestionarios se aplicaron a 692 enfermeras. Se obtuvieron índices de discriminación >0,30 en todos los ítems. El análisis factorial confirmatorio de los dos modelos (de cuatro factores oblicuos y añadiendo un factor de segundo orden) reveló que todos los ítems tenían cargas factoriales adecuadas y que los índices de ajuste eran buenos. La consistencia interna fue de 0,88 para la escala total y entre 0,70 y 0,77 para las dimensiones. Se obtuvo una relación positiva entre las habilidades comunicativas y la autoeficacia general y específica. CONCLUSIONES: La EHC mostró unas adecuadas propiedades psicométricas en cuanto a análisis de ítems, estructura interna, fiabilidad y evidencias de validez, relacionando las habilidades de comunicación de las enfermeras con la autoeficacia general y específica para la comunicación en situaciones difíciles

ABSTRACT

OBJETIVO: Calcular la efectividad de la vacuna de la varicela en brotes escolares durante el curso académico 2009-2010. MATERIAL Y MÉTODO: Estudio de cohortes retrospectivo realizado en guarderías y/o colegios de educación infantil o primaria públicos de un municipio de la Región de Murcia, España. Los participantes fueron el alumnado de 1 a 12 años que acudía a la misma aula donde se produjo un brote de varicela. Las mediciones principales fueron: datos sociodemográficos, antecedentes clínicos y de vacunación y variables relacionadas con la enfermedad varicelosa obtenidos a través de un cuestionario autocumplimentado por padres y/o tutores y del registro regional informatizado de vacunas (VACUSAN). RESULTADOS: Se detectaron 51 brotes de varicela, con una mediana de 3 casos por brote en ambos niveles educativos. La cobertura vacunal global fue del 10,7% (IC del 95%, 8,63-13,18), siendo siempre muy superior en el alumnado de nacionalidad española frente al de nacionalidad extranjera (OR = 21,21; IC del 95%, 2,92-153,92; p < 0,001). Encontramos discrepancias entre los datos vacunales del cuestionario y el programa informatizado de vacunaciones (kappa=0,50 [IC del 95%, 0,43-0,58), p < 0,001). La tasa de ataque global según VACUSAN fue de 59,7 (IC del 95%, 55,82-63,43) en no vacunados y de 6,5 (IC del 95%, 2,54-15,45) en vacunados. Obtuvimos una efectividad global del 89,1% (IC del 95%, 74,55-95,35) y 100% para una y 2 dosis de vacuna, respectivamente. CONCLUSIONES: Elevada efectividad de la vacuna de la varicela y muy especialmente en un esquema de vacunación de 2 dosis. Se han detectado discrepancias entre la información aportada por los padres y los registros informatizados regionales, así como una diferencia en la cobertura de vacunación en función de la nacionalidad

CONCLUSIONS: There is a high effectiveness of varicella vaccine, emphasizing that the administration of two doses of vaccine produces an adequate and optimal protection against varicella disease. A discrepancy was found between the information provided by parents and official records. Finally, there was a lower vaccination coverage in the immigrant community

ABSTRACT

Objetivo. Presentar una innovación docente desarrollada en la asignatura de Radiología y Medicina Física Especial durante el curso académico 2006-2007. Sujetos y métodos. Los alumnos realizaron una rotación durante dos semanas en los servicios de Radiodiagnóstico y Medicina Nuclear, en grupos pequeños. La metodología docente utilizada fue la carpeta de aprendizaje o portafolio. Se describen los aspectos fundamentales para la elaboración de la carpeta y de los casos clínicos, así como del sistema de evaluación. Dentro del ciclo de mejora continuada se llevó a cabo una encuesta de satisfacción. Resultados. Los resultados académicos obtenidos fueron excelentes. La mayoría de los alumnos consideraron que la asignatura fue interesante. También señalaron una valoración positiva de la rotación, de los seminarios y del sistema de evaluación. Asimismo, la opinión de los alumnos acerca de la experiencia docente realizada fue muy favorable. Conclusión. La satisfacción global de los alumnos ha sido muy alta con la metodología docente implantada en la asignatura (AU)


Aim. To introduce a learning innovation on the subject of Radiology and Special Physical Medicine along the academic year 2006/2007. Subjects and methods. Working in small groups, students carried out a two-week rotation in the departments of Radiology and Nuclear Medicine. The portfolio was the educational methodology used. The main aspects to elaborate the portfolio, the case reports, aswell as the evaluation system are described. A satisfaction survey within the continuous improvement course was performed. Results. The academic results achieved were excellent. The subject was considered interesting by most of the students. A positive evaluation about the rotation made by the students, the seminaries and the evaluation system was made. Students’ opinion about the educational experience performed was also, very favourable. Conclusion. The educational methodology introduced in the subject has produced a high level of satisfaction among the students (AU)
