ABSTRACT

The study revealed that the Antarctic territory in the area of the Ukrainian Antarctic station "Academician Vernadsky" can be referred to the zone of the "super high", risk in developing ophthalmological pathology, caused by ultraviolet radiation. The results of the comprehensive dynamic study of 24 winterers of the XVI and XVII Ukrainian Antarctic expeditions (2011-2013) showed the developing of specific changes in the organ of vision under the exposure to the excessive natural ultraviolet radiation (UVR), such as parenchymatous-endothelial keratopathy, nuclear-posteriorcapsular phacopathy, maculopathy. Morphological changes were recorded using the cornea densitometry method: a decrease of the central cornea thickness by 14.3%, a decrease of the density and the number of endothelial cells of hexagonal type by 10.1% and 16.6%, respectively (P < 0.05). Using densitometry method, we detected morphological changes in the lens. Specifically, we found an increase of the optic lens density: in median zone by 10.1%, in maximal zone by 21.6%, in posterior part by 13.1%, P < 0.05. The results of densitometry recordings point to the development of local opacity in the posterior lens parts (in the nucleus and in the capsule). The data of the computed quantitative macula perimetry showed a significant decrease of light sensitivity of retina conicalis cells by 6 times (P < 0.05). The morphological signs of the endured acute solar retinopathy were found in more than 50% winterers. The revealed pathological changes can be considered as pre-, subclinical manifestations of the cornea dystrophy, cataracts and maculodystrophy, caused by specific action of the natural UVR, thus demanding the elaboration of the combined preventive measures.

ABSTRACT

Peculiarities of anatomy, histology and physiology of eyes, do not allow to create an effective concentration of a medication in damaged tissues by traditional methods. Therefore, effective remedy delivery to a pathological focus is an current issue for ophthalmologists, pharmacologists and pharmacists. One of the perspective ways to solve this problem is nanotechnology. The use of nanomaterials (nanoparticles, dendrimers, cyclodextrins, liposomes, niosomes, diskomes), medicine formulations (microemulsions, hydrogel systems, nanosuspenisons) and ways of medicine delivery and nanomaterials (contact lenses, microneedles, intraocular implants, iontophoresis), open new possibilities in the treatment of serious eye diseases, such as glaucoma, diabetic retinopathy, macular degeneration, uveitises of different etiology. It will establish new high level therapy standards in ophthalmology in future. Aspects of possible complications and long-term effects of application of these technologies are still poorly explored and need further development and improvement. Despite of it, this new direction of the treatment in ophthalmology is very important and perspective.

ABSTRACT

The efficacy of the organizational system has been studied by rendering ophthalmologic care to 246,866 patients. Statistical methods such as parametric and evaluative coefficient technique were used. Data verification has been evaluated with Student coefficient. Implemented organizational system of ophthalmologic care turned out to be highly effective due to the following reasons: differentiation of the patients flow between out-patients and in-patients departments; increase and differentiation of ophthalmologic assistance rendering in out-patient department; development of in-patients substitution forms of patients treatment in conditions of outpatient surgery department; usage of high technological methods of treatment in eye laser treatment department; more effective usage of in-patient beds by reducing in-clinic stay period, application of up to date surgical treatment, referring to in-patients department those who need highly qualified care.

The methodology of the prophylaxis of ophthalmologic pathology among different age groups of the population at primary medical care level, with the initial eyes screening was worked out. Standards and efficiency indices to provide quality prophylaxis help have been developed.
