ABSTRACT

The number of T- and B-lymphocytes, O cells, the IgA, IgG, IgM content, the lymphocyte transformation in vitro and also the haematological parameters of the white blood were studied in 33 patients with rheumatic heart diseases during the inactive phase of the illness and in 25 operated on patients on the 1st-10th post-operative days. On the 1st-3rd day after the operation marked (3-4-fold) decrease of the absolute number of T- and B-lymphocytes was recorded. The immunological parameters recovered by the 5th-7th days after surgery. On the 10th day they exceed their initial levels. The authors believe it possible to use these data in elaborating the program of immunological control in cardiosurgical patients.

ABSTRACT

The remote results were studied in 194 patients operated on for secondary atrial septal defects at ages of 18 months to 41 years. The follow-up period ranged from 5 months to 11 years. The ECG was normal in 74.7% of patients, its improvement was noted in 8.7% of patients. Disorders of rhythm were encountered in 10.3% of patients, their incidence being 2 1/2 times higher among adults than among children. The systolic murmur disappeared in 29.8% and diminished in 50.8% of patients. The diastolic murmur was corrected in 92.2% of cases. Splitting of the second sound disappeared in 66% and diminished in 6% of patients. The size of the heart on a radiogram became normal in 32.2% and its enlargement became less in 41% of the patients examined. The heart grows smaller within 2--3 years and the pulmonary pattern becomes normal in 1--2 years. The process is more rapid in children. The postoperative result was good in 90% of patients.

ABSTRACT

Experimentally on 116 dogs it has been found that intravascular shunts of a big diameter or those controlled by means of extracorporal shunts ensure stable hemodynamic indices, approximately the initial ones. The isolated hypothermal perfusion protects, in its turn, the liver from an ischemic trauma and prolongs the safety period of the liver blood flow break to 40 minutes and more.

ABSTRACT

In 39 patients aged from 2 1/2 to 35 years the results of surgical treatment of congenital aortic stenosis are analyzed. Electro-, phonocardiographic and roentgenological findings along with those obtained through catheterization of the left cardiac chambers prior to surgery are described in detail. All these patients were divided into 3 groups, depending upon extent of deranged hemodynamics. The 1st group included 10 persons with a moderate aortic stenosis. The systolic gradient between the left ventricle and the aorta did not exceed in them 30 mm Hg. The 2nd group counted 20 patients with a well-marked aortic stenosis and a systolic pressure gradient between 30 and 80 mm HG. The 3d group embraced 9 patients with an acute aortic stenosis. Here the systolic gradient was in excess of 80 mm Hg. Surgery was performed in 35 patients under a moderate hypothermia and with extracorporeal circulation. Thirteen (33.3 per cent) patients died during surgery and in the post-operative period.
