ABSTRACT

The effect of 10 mg nifedipine on cerebral oxygenation was studied in healthy individuals. The duration of investigation was 30 min or 3 h. Nifedipine appreciably increased cerebral oxygenation (by 5.9%, p<0.01) against the background of significant increase in heart rate and negligible decrease in systolic arterial pressure. Drowsiness was the most serious side effect of nifedipine (observed in 52.8% examined subjects).

ABSTRACT

Efficacy of intravenous drop Nitro Mac (alcoholic solution of nitroglycerin) in acute left ventricular failure and angina of functional class III was investigated in 18 patients aged 42-68 years. 1 ml Nitro Mac solution was diluted in 100 ml isotonic solution once a day (in severe condition twice a day) for 6 days. Nitro Mac relieved anginal attack on infusion minute 3-5 and anginal pain on minute 2. In angina functional class III Nitro Mac completely eliminated anginal attacks in 8 of 10 patients and reduced its number in 2 patients. Exercise tolerance has grown. Oral nitrates' doses could be decreased twice. Side effects were mild. Nitro Mac is recommended for treatment of acute cardiac failure and angina of functional class III as an effective medication.

ABSTRACT

Intravenous isosorbide dinitrate and nitroglycerin administered within early hours of myocardial infarction have a marked antianginal effect, stabilize the size of necrotic focus and promptly control acute left-ventricular insufficiency. Unlike nitroglycerin, isosorbide dinitrate has a more lasting action and does not affect heart rate and arterial blood pressure. It can therefore be used more extensively in patients with acute myocardial infarction.

ABSTRACT

The authors presented the results of a radioimmunoassay of myoglobin (MG), MV-fraction of creatine phosphokinase (CPK), hypophyseal thyrotropic hormone, peripheral thyroid hormones and testosterone in 150 patients with acute myocardial infarction. A high informative value of the radioimmunoassay was shown. The MG level and MV-CPK activity as well as the functioning of the hypophyseal-thyroid system were shown to be directly correlated with the severity of disease and were of prognostic value. A considerable suppression of hypophyseal and thyroid function coinciding with the maximum values of myoglobinemia in the most severe cases was noted. Three variants of the testosterone content were singled out in patients with acute myocardial infarction. Paroxysmal rhythm disorders in the 1st variant (up to 300 ng/100 ml) and A-V blockades (over 1000 ng/100 ml) in the 3rd variant were frequently noted.

ABSTRACT

The authors present comparative characteristics of different methods of assessing the size of myocardial infarction. They included among others radioimmunoassay of myoglobin and the activity of MB-CPK in the blood serum, ECG charting with the calculation of the ST-coefficient and 99mTc-pyrophosphate scanning of the myocardium. The most informative methods were determination of the serum content of myoglobin and of its hourly increment, monitoring of the total rise of the ST segment and ST coefficient on the cartogram and the measurement of the area of accumulation of radiopharmacologic preparations during myocardial scanning.

ABSTRACT

Visualization of the myocardial necrotic focus with the aid of 99mTc -pyrophosphate is an informative method of determining the infarction degree and localization in the cardiac muscle. Scanning enables evaluation of the area of the necrotic focus in patients with acute myocardial infarction within two weeks of the disease onset. Scintigraphy with both the diffuse and the focal variants of isotope accumulation is shown to be of high diagnostic value in acute myocardial infarction. The specificity and sensitivity of scintigraphy are 87% and 96%, respectively.
