ABSTRACT

OBJECTIVE: The objective of this study was to assess the tolerability, safety and effectiveness of an oral sodium phosphate solution as a colon preparation method prior to colonoscopy, compared to a standard polyethylene glycol preparation. EXPERIMENTAL DESIGN: A cohort-based multicenter, prospective, observational clinical study was conducted. PATIENTS: Twenty-nine centers from all over Spain participated in this study, including a total of 638 patients who met all the inclusion criteria established. RESULTS: The oral sodium phosphate solution has an effectiveness and safety profile similar to the standard poliethylene glycol preparation; however, due to its dosage, it is better tolerated by patients, with a statistically significant difference between both preparations. CONCLUSIONS: Based on the effectiveness, safety and tolerability profile of oral sodium phosphate solution in patients, it can be considered the preparation of choice before colonoscopy, since it allows a fast, effective and safe bowel cleansing in patients without contraindications.

ABSTRACT

Objetivo: el objetivo de este estudio ha sido evaluar la tolerancia, seguridad y eficacia de una solución de fosfato sódico oral como método de preparación del colon previo a una colonoscopia, frente a una preparación estándar de polietilenglicol. Diseño experimental: planteamos un estudio clínico multicéntrico, prospectivo observacional de cohortes. Pacientes: en el presente estudio han participado 29 centros de todo el territorio nacional y un total de 638 pacientes incluidos en el estudio, siempre y cuando cumplieran todos los criterios de inclusión descritos en el mismo. Resultados: la solución de fosfato sódico oral presenta una eficacia y un perfil de seguridad equiparables a la preparación estándar de polietilenglicol; sin embargo, debido a su posología, es mejor tolerada por los pacientes con una diferencia estadísticamente significativa entre ambas preparaciones. Conclusiones: debido al perfil de eficacia, seguridad y tolerancia de los pacientes a la solución de fosfato sódico oral, éste puede ser considerado como la preparación de elección previa a la realización de colonoscopias, ya que permite obtener un lavado del colon rápido, eficaz y seguro en aquellos pacientes sin contraindicaciones (AU)

ABSTRACT

The blue rubber bleb nevus syndrome is a rare entity characterized by the presence of cavernous hemangiomas in the skin and gastrointestinal tract with frequent digestive hemorrhages. Different therapeutic modalities exist: medical treatment, surgical resection; and most recently, endoscopic therapy has been described. We present a patient with blue rubber bleb nevus syndrome treated with combined endoscopic therapy: sclerosis and band ligation.

ABSTRACT

Acute post traumatic pancreatitis is an infrequent disease representing 0.4% of the acute pancreatitis with pseudocyst formation. Few data have been reported in the literature with regard to response to treatment, particularly in cases of small or multiple pseudocysts. Internal surgical drainage is the usual treatment. Different therapeutic alternatives have been proposed among which conservative treatment with total parenteral nutrition, somatostatin or octreotide, or more recently, endoscopy may be included. We herein present one case of acute post traumatic pancreatitis initially treated with conservative treatment which evolved to the formation of pseudocysts which were satisfactorily drained by endoscopic cystogastrostomy.

ABSTRACT

The prognosis of 189 patients with upper gastrointestinal bleeding was studied with the assistance of a computer. The safety of the prognosis given by the doctor was partially improved with the computer. However, when both judgements were simultaneously considered, the best results were obtained. A scoring-system constructed with simple clinical data proved useful to obtain prognostic judgements with high sensitivity and specificity. It is concluded that the reliability of the prognosis given in patients with upper gastrointestinal bleeding can be improved with objective methods. The improvement is maximal if objective information is evaluated together with clinical information.
