ABSTRACT

Blood supply to the human thalami is complex and multiple variants exist. The artery of Percheron is one of those variants and is characterized by a solitary arterial trunk that branches from one of the proximal segments of either posterior cerebral artery and supplies blood to the paramedian thalami. Its occlusion results in bilateral paramedian thalamic infarction sometimes extending to the midbrain. We report six cases of bithalamic infarction secondary to occlusion of the artery of Percheron. We will illustrate the complex clinical symptomatology and underscore the role of imaging, especially MRI, for diagnosis.

ABSTRACT

The superior mesenteric artery (SMA) syndrome is an atypical cause of high intestinal obstruction in adults. Formerly considered controversial, this syndrome has now been recognized as a real clinical entity which results from extrinsic compression of the third portion of the duodenum by reduction of the angle formed between the SMA and the aorta, usually favoured by rapid and dramatic weight loss. We report a case observed in a 25-year-old female. The abdominal scan provided the diagnosis. Laparoscopic duodenojejunostomy provided cure after failure of initial conservative treatment.

ABSTRACT

This is a new case of Susac syndrome in a 27-year-old woman with polymorphic neurological disorders, her brain MRI showed multifocal hyperintense signals on T2-weighted images with possible effects on the corpus callosum. However, visualization of an occlusion in the retinal arterial branch of the right eye and hypoacusia on the right side allowed confirmation of the diagnosis. In this case report, we describe the imaging aspects of Susac syndrome and demonstrate that brain MRI allows the syndrome to be diagnosed at an early stage.

ABSTRACT

Hydatid disease is the most common human disease caused by helminths, but primary skeletal involvement is uncommon. Diagnosis is usually obtained late after considerable extension, half of which concerns the spine. The dissemination mode leads to local malignancy with severe prognosis. Medical imaging is essential. Plain x-ray findings are polymorphous and nonspecific, but computed tomography and magnetic resonance imaging are particularly useful for exhaustive pretherapeutic assessment of extension and for patient follow-up.

ABSTRACT

Bone and joint localizations of treponematosis vary greatly although there are many common features. Osteal and periosteal lesions are common. We report radiographic descriptions of bone and joint treponematosis, which must not go unrecognized due to the increasing incidence of these diseases.

ABSTRACT

Magnetic resonance imaging (MRI) is a high performance tool for morphological assessment and tissue characterization in the presurgical work-up of scrotal tumors. A wait-and-see attitude is now possible for benign lesions identified as epidermoid cysts. Pathology examination remains more pertinent for an exhaustive assessment of local extension, but MRI can provide a convincing, if not definitive, distinction between seminomatous and non-seminomatous germ cell tumors.

ABSTRACT

The authors report a case of spontaneous intramural haematoma of the oesophagus. This is a rare observation which usually occurs in association with oesophageal hyperpressure and sometimes with impaired haemostasis. The strategy for diagnosis is based on tomodensitography and also endosonography and magnetic resonance imagery.

ABSTRACT

The authors report the case of a 28-year-old patient presenting with successive ischaemic cerebral vascular accidents, preceded by the appearance of a livedo reticulare, without any laboratory signs of an inflammatory syndrome. Since Sneddon, and before him Divry and Von Bogaert, patients with this dermatosis are known to have an increased frequency of ischaemic cerebral vascular accidents. This syndrome, considered for a long time to be minimally aggressive, appears to be a very serious disease, leading to a bedridden state and dementia. The prognosis for these patients, all young (30-40 years), therefore appears to be very poor. Any case of livedo reticulare requires the search for cerebral neurological lesions. After a complete neurological examination, imaging therefore has an important major in the assessment of cerebral lesions due to this corticomeningeal angiomatosis. CT scan and MRI are useful in this initial assessment and in the subsequent follow-up of patients. In cases with frank neurological lesions, cerebral arterial exploration is essential with demonstration of lesions of the midcerebral arteries and activation of collateral vascular anastomoses and their helicine vessels. The prognosis is based on the extent of these lesions. Cerebral biopsy may demonstrate a non-inflammatory endothelial cellular proliferation of midcerebral vessels.

ABSTRACT

The authors report us about an unusual hydatid cyst of the liver. Ultrasound, CT and MRI exams were used, and showed an unusual fatty component inside the lesion. The development of the disease in steatosic liver is not enough to explain the unusual density.
