ABSTRACT

Evidence remains unclear on how intergenerational social mobility is associated with body mass index (BMI) and its long-term changes. Our study identified BMI trajectories from middle to older age by intergenerational social mobility groups and stratified the analyses by gender and two birth cohorts (birth years 1940‒1947 and 1950-1962). We used questionnaire-based cohort data that consists of four survey phases: 2000-2002, 2007, 2012, and 2017. In Phase 1, participants were 40-60-year-old employees of the City of Helsinki, Finland. Our analytical sample consisted of 6,971 women and 1,752 men. Intergenerational social mobility was constructed based on self-reported parental and own education-both divided into high and low-yielding four groups: stable high socioeconomic position (SEP) (high-high), upward social mobility (low-high), downward social mobility (high-low), and stable low SEP (low-low). BMI was calculated from self-reported height and weight from all four phases. Using mixed-effects linear regression, we found increasing BMI trajectories in all four social mobility groups until the age of 65. Women and men with stable high SEP had lower BMI trajectories compared to those with stable low SEP. In the younger birth cohort, women with upward social mobility had a lower BMI trajectory than women with stable low SEP. Additionally, women and men with downward social mobility had higher BMI trajectories than those with stable high SEP. In the older birth cohort, however, the BMI trajectories of upward and downward social mobility groups were somewhat similar and settled between the BMI trajectories of stable high and stable low SEP groups. Our results indicate that the associations between intergenerational social mobility and BMI may depend on gender and birth cohort. Nevertheless, to reduce socioeconomic inequalities in unhealthy weight gain, obesity prevention actions that focus on people who are likely to remain in low SEP might be worthwhile.

ABSTRACT

DNA sequences offer powerful tools for describing the members and interactions of natural communities. In this study, we establish the to-date most comprehensive library of DNA barcodes for a terrestrial site, including all known macroscopic animals and vascular plants of an intensively studied area of the High Arctic, the Zackenberg Valley in Northeast Greenland. To demonstrate its utility, we apply the library to identify nearly 20 000 arthropod individuals from two Malaise traps, each operated for two summers. Drawing on this material, we estimate the coverage of previous morphology-based species inventories, derive a snapshot of faunal turnover in space and time and describe the abundance and phenology of species in the rapidly changing arctic environment. Overall, 403 terrestrial animal and 160 vascular plant species were recorded by morphology-based techniques. DNA barcodes (CO1) offered high resolution in discriminating among the local animal taxa, with 92% of morphologically distinguishable taxa assigned to unique Barcode Index Numbers (BINs) and 93% to monophyletic clusters. For vascular plants, resolution was lower, with 54% of species forming monophyletic clusters based on barcode regions rbcLa and ITS2. Malaise catches revealed 122 BINs not detected by previous sampling and DNA barcoding. The insect community was dominated by a few highly abundant taxa. Even closely related taxa differed in phenology, emphasizing the need for species-level resolution when describing ongoing shifts in arctic communities and ecosystems. The DNA barcode library now established for Zackenberg offers new scope for such explorations, and for the detailed dissection of interspecific interactions throughout the community.

ABSTRACT

OBJECTIVE: To evaluate the characteristics of patients treated at a field hospital in the first month after a major earthquake. METHODS: Age, sex, diagnosis, and operations performed on patients admitted to the field hospital of the International Committee of the Red Cross in Pakistani Kashmir between 21 October and 10 November 2005 were recorded and the data analysed. RESULTS: During the three week period of this study, 316 patients were treated at the hospital; 246 were women and children (77.9%). Two thirds were hospitalised, over 90% because of the need for surgery or surgical consultation. Altogether 345 operations were performed on 157 patients. The majority of patients had infected wounds with or without fractures. CONCLUSIONS: Most patients need medical evaluation in consequence of earthquake-related trauma even weeks after the catastrophe, especially in areas difficult of access.

ABSTRACT

OBJETIVO: Com base em estudo qualitativo, utilizando a história oral e a análise de documentos, procurou-se identificar as raízes da fisioterapia em Minas Gerais, a fim de discutir o processo de profissionalização e o desenvolvimento da identidade do fisioterapeuta neste Estado. METODOLOGIA: O foco do estudo foi dirigido principalmente a Belo Horizonte, no período compreendido entre 1950 e 1980. O referencial teórico utilizado foi a sociologia das profissões, e teve como guia os cinco passos de profissionalização descritos por Wilensky¹ que são a necessidade da ocupação tornar-se de dedicação exclusiva, o estabelecimento de procedimentos de instrução e seleção, a formação de uma associação profissional, a regulamentação da profissão e a adoção do código de ética. Na pesquisa de campo, foram utilizadas entrevistas do tipo semi-estruturada para colher depoimentos de pessoas consideradas chave, mediante amostragem em série, e o método escolhido foi a análise de conteúdo. RESULTADOS: Avalia-se que o processo de profissionalização da fisioterapia em Minas Gerais é marcado pela hegemonia médica na saúde e que a indefinição das atribuições e competências foram os fatores que motivaram os conflitos internos e externos. CONCLUSÕES: Conclui-se que, para a afirmação efetiva da fisioterapia como profissão, torna-se necessária a delimitação das especificidades do seu saber e do seu fazer (teoria e prática), identificando o papel do fisioterapeuta na equipe multidisciplinar de saúde.

ABSTRACT

To replicate 1986 work of Salmela and Ndoye on whether physical arousal from riding a stationary bicycle led to narrowing of attentional focus 17 subjects responded to a verbal 5-choice RT task while pedalling to exhaustion. Increased heart rates with increased physical stress (bicycle resistance increased) was not associated with narrowing of attention. When heart rates were 160 and 180 bpm, RTs to stimuli peripherally located to the right were slower than central ones. Further evaluation is required.

ABSTRACT

The efficacy of midazolam vs lorazepam and placebo was studied in 60 patients in a comparative, double-blind study. Midazolam 15 mg and lorazepam 2.5 mg were given orally in a single dose the night before operation. Sleep latency was shortest on midazolam but the mean duration of sleep longest on lorazepam. Four patients on lorazepam complained of headache and one reported tinnitus. Three patients receiving midazolam complained of headache. Headache was observed also in one patient given placebo and another experienced tachycardia. Midazolam administered the night before surgery is a preferable hypnotic for transient insomnia caused by preoperative anxiety.

ABSTRACT

In the present investigation, an elite group of competitive female French-Canadian gymnasts (N = 7) were evaluated using 35 standardized morphological measures at an interval of 475 days. At the two evaluatione, the average age of the gymnasts was 13.9 and 15.3 years and at the termination 4 of the 7 girls had not yet reached menarche. Standard body lengths, widths, skinfolds and proportions were measured. Almost everybody dependent variable increased significantly while the body proportions, for the most part, stayed the same. Skinfold measures of body fat tended to remain the same or to decrease over this period. In comparison to other reference groups of female gymnasts tested over the last 10 years, these elite competiters were younger, smaller, and lighter reflecting the present day trends. Implications of this decreased body size were discussed in terms of the potential mechanical advantages that accompany these characteristics.

ABSTRACT

The effect of both the availability and the quality of visual cues preceding stick-puck contact was investigated for an ice hockey goaltending task. Thirty-four young (15.8 years) goaltenders observed filmed sequences of a hockey player approaching the net and directing a shot to one of the four corners. The film sequences were edited so that 2, 4 or 8 images were occluded prior to the impact of the stick with the puck, reflecting durations of 1/12, 1/6, 1/3 of a second, respectively. The subjects' performances for both the wrist and the slap shot were measured as well as the level of response confidence in both the horizontal and vertical planes. The results indicated that performance precision was aided by the increased availability of pre-shot visual cues. It was also evident that the wrist shot provided more cues for the goaltender than did the slap shot. Performances were found to be superior in the horizontal as compared to the vertical plane. In all dimensions, the subjects' perceptions of their success closely followed the actual performance patterns. It appeared that information that could facilitate anticipation in this goaltending task was both available and useable during the period that precedes the shot.

ABSTRACT

A 75-year-old woman was subjected to biliary surgery 38 years after partial gastrectomy for ulcer. There was a history of gallstones of 10 years duration, pentagastrin-resistant achylia, cholecystolithiasis and choledocholithiasis complicated by stenosis of papilla of vater, cholecystitis and pancreatitis. Peroperative cholangiography and biliary tract surgery were performed. On the third postoperative day heavy jaundice and hemolysis developed, leading to death of the patient. Culture of bile taken at operation revealed strains of Clostridium perfringens and Escherichia coli. Autopsy showed a picture of gas gangraena of the liver and Clostridium septicemia. The role of achylia, blind loop, and biliary obstruction in bile surgery is stressed.
