ABSTRACT

Alternatives to traditional organ donor usage has allowed expansion of the organ donor pool to help compensate for the increasing disparity between recipients and donors. The use of bilateral adult renal transplants is a novel idea to salvage older donor kidneys with suboptimal nephron mass that would otherwise be destined for discard. Ten renal transplants were performed utilizing both kidneys from adult cadaver donors with diminished nephron mass determined by calculated glomerular filtration rate or biopsy evidence of significant glomerulosclerosis (>10%). Nine of ten (90%) recipients have satisfactory renal function at a mean follow-up of 7 months. The single case of graft failure was due to documented medical non-compliance. Mean serum creatinine at 6 months was 1.5 mg/dl. Mean measured creatinine clearance was 43.2+/-3.4. These preliminary findings suggest that the use of bilateral renal transplants provide satisfactory early function and allows salvage of older donor kidneys with suboptimal nephron mass.

ABSTRACT

La pielonefritis enfisematosa es una infección grave caracterizada por la presencia de gas en el espacio perirrenal, el parénquima o el sistema pieloureteral. Es un cuadro poco frecuente y de alta mortalidad. Se asocia en un 70 a 90 por ciento de los casos a pacientes diabéticos no controlados con o sin uropatía obstructiva (1-6). El objetivo de esta comunicación es presentar dos casos clínicos tratados con buen resultado y revisar la literatura actual al respecto

ABSTRACT

This study represents the first investigation demonstrating the contractile response to exogenous acetylcholine (ACh) in the isolated human vas deferens. Pharmacological characterization of cholinergic receptors was achieved using selective antagonists to define receptor subtypes. In the HVD the effect of exogenous ACh is revealed as a dose-dependent sudden increase in the basal tension of the vasa. The ACh receptors of the HVD were competitively antagonized by atropine (ATR) with a high pA2 value (8.78). The main finding of this study is the presence of cholinergic receptors of the pharmacologically defined M1-ACh subtype in the isolated HVD, according to the pA2 values obtained with pirenzepine (PRZ) 7.39, AF-DX 116 (AF) 5.92 and 4-DAMP 5.65, M1-ACh, M2-ACh and M3-ACh selective antagonists, respectively. Prazosin (PZ), a selective alpha 1-adrenergic antagonist, displayed a similar competitive antagonism for the contractile response evoked both by ACh (pA2 = 8.69) and NE (pA2 = 8.58) in the HVD. The antagonism exerted by PZ on the ACh-induced contractile response of the HVD, suggests that ACh probably acts at a presynaptic level stimulating the release of NE from an adrenergic neuron. According to these findings, the receptor involved in this action, located in the proximity of the nerve terminals, seems to be of the M1-ACh subtype.

ABSTRACT

Se revisa la sobrevida de 23 pacientes portadores de Cáncer de Próstata D* que inician tratamiento con Flutamida y Castración Subalbugínea entre abril 1986 y agosto 1987. Seis pacientes fallecen durante el período de observación (abril 1986- septiembre 1989), cuatro de ellos por progresión de la enfermedad. En 19 pacientes el dolor permanece estable o disminuye. Se evidencia regresión radiológica y/o cintigráfica en 7/17 casos, permaneciendo estables en otros 10. La sobrevida actuarial es 87% y 66% a 2 y 3 años respectivamente

ABSTRACT

Se analizan las caracteristicas de una muestra representativa de 1.557 pacientes colecistectomizados por patologia benigna en el Hospital de San Carlos en el decenio comprendido entre 1972 y 1981. Se destaca el predominio de las mujeres sobre los hombres en proporcion de 5 a 1. Se describen algunos detalles de tecnica quirurgica Se hace hincapie en el hecho que la patologia vesicular aguda represento el 35,4% de los casos; que se hizo coledocostomia en el 15,8% de ellos; que la litiasis coledociana se comprobo en el 10,4% del grupo total y en el 65,6% de los coledocostomizados. Se enumeran las principales complicaciones postoperatorias inmediatas y alejadas. La mortalidad global fue de 0,9% y la de los casos de colecistitis aguda de 3%

ABSTRACT

El melanoma maligno es uno de los tumores con peor pronostico, por ser de gran malignidad y capacidad de metastasis. Tiene una incidencia de 1.1% de todos los carcinoma y en la revision hecha por Stewart de 1.677 pacientes con melanoma solo 6.9% presentaron metastasis oseas. Estas metastasis cursan clinicamente como fracturas patologicas. Presentamos el primer caso de metastasis osea por melanoma hallado en el Hospital Mexico bien documentado clinica, radiologica e histopatologicamente. Ademas con tratamiento quirurgico, usando osteosintesis con pin-placa cementado (methilmetacrylate), siendo una tecnica salvadora en este caso
