ABSTRACT

Background: With the second wave of coronavirus disease (COVID-19) Rhino Orbito Cerebral Mucormycosis (ROCM) has emerged as an epidemic in India. Early suspicion and management can reduce disfigurement and mortality. Objectives: To identify the demographic and clinical profile of ROCM, its association with COVID-19 and other predisposing factors. Methods: Hospital Based Cross Sectional Observational study on first hundred consecutive patients with signs and symptoms and radiological findings suggestive of ROCM. Data collected from patient records and analyzed. Results: Out of hundred, 72 patients were Males, 74 from rural areas, all were above eighteen years age, 35 presented with orbital and 27 with facial manifestations of ROCM. Main predisposing factors were history of COVID-19 (71%), previous hospitalization (48%), in Intensive Care Unit (5%) systemic corticosteroid therapy (48%), Diabetes, uncontrolled (45%) and new (21%). Conclusions: ROCM presented with orbital and facial manifestations. Main predisposing factors were Diabetes, COVID-19 and systemic steroids.

ABSTRACT

A 70-year old female presented with complaints of fever with chills, headache for 15 days and altered sensorium for 7 days. Peripheral blood smear showed ring form of P. falciparum and CT brain revealed bilateral subdural hematoma. Patient received Artesunate based combination therapy and recovered completely without surgical intervention within 8 days of admission.

INTRODUCTION: Cerebrovascular disorders (CVD) are increasing in prevalence and incidence in Indian population. Global burden of disease study shows that of the 9.4 million deaths in India, 619,000 were due to stroke. A matter of concern is that in the last two decades there is a significant increase in prevalence rate of stroke. AIM: To find the demographic characteristics along with clinico-radiological profile and of patients presenting with stroke in a tertiary care hospital in Rajasthan. METHODOLOGY: This cross sectional study was conducted for a period of 6 months from Aug 2015 to Jan 2016 in a tertiary care hospital .A sample size of 360 patients with aged >18 years were recruited after obtaining written consent from those who attended the medical OPD or were admitted to the government hospital with the newly diagnosed stroke and confirmed by neuroimaging. RESULT: The mean age of stroke in present study was 60.46 ± 14.84 years. The stroke in the young age group defined as 40 years or less comprised only 6%. Hypertension as a risk factor was present in 52.5% of our patients followed by Dyslipidaemia (25.8%). 79.4% had ischemic stroke and 19.4 % haemorrhagic stroke. Supratentorial lesions were seen in 86.4% patients, infratentorial lesions in only 10% anterior circulation strokes (MCA>ACA) are more common than posterior circulation strokes. Motor and sensory symptoms are common in acute strokes rather than change in consciousness or speech abnormalities. 70.2% patients had moderate disability at the end of 28 days. Early presentation to hospital (<3hrs) is associated with better outcome and less morbidity in a stroke patient.

ABSTRACT

Cysticercosis, a parasitic disease caused by larval form of Taenia solium, is a major health concern in the developing world. The encysted larval stage can affect any part of the body, but are most frequently detected in brain, eye, skeletal muscle and subcutaneous tissue. Most muscular cysticercosis are almost always associated with central nervous system involvement or with multiple intramuscular cysts or both. Here we report an unusual case of cysticercosis in right rectus muscle which was an isolated muscle involvement without any other systemic manifestation.

ABSTRACT

The syndrome of familial hypoparathyroidism, sensorineural deafness, and renal dysplasia (HDR syndrome) is inherited as an autosomal dominant trait, caused by haploinsufficiency of the GATA3 gene in chromosome 10p. Although first described years ago, but the disease is considered to be very rare. Patients usually present with hypocalcemia, tetany, or afebrile convulsions at any age. Hearing loss is usually bilateral, range from mild to profound impairment. Renal disease includes dysplasia, hypoplasia or aplasia.
