ABSTRACT

The paper outlined the experience of surgical treatment of 103 patients with intra-articular fractures of the proximal tibia and refined indications to adequate osteosynthesis depending on the nature of the fracture on the classification of Schatzker. Shows two groups of patients (control and basic), which compares traditional and minimally invasive osteosynthesis techniques depending on the type of fracture. Long-term anatomical and functional results were analyzed by 1 year after surgery in 103 patients - the main and control group patients according to the scale Lyscholm.

ABSTRACT

The article presents developed technique of assessment of hand thermograms in healthy volunteers and patients with disorders of hand articulations. The authors have also presented analysis of the results of thermography of hands of 94 healthy person. Methods of practical use of obtained data in medicine to diagnose complicated hand articulations were presented.

ABSTRACT

The article presents obtained results on using remote infrared thermograph in medical practice in Ukraine. It was shown that appearance of domestic thermographs with matrix photoreceiver enhances considerably an area of their use. New results related to diagnosis of a patient's state were obtained. Taking into account that this method is harmless, not difficult to perform, allows carrying out diagnostics many times during treatment and appearance of a new type of thermographs with high temperature sensitivity, it is necessary to renew the use of remote infrared thermography in medical practice in Ukraine.

ABSTRACT

Metalloosteosynthesis of neck and trochantarian region of the femur, endoprosthesis of the hip joint were performed in the treatment of the femur proximal end fractures. Significant frequency of early and late complications in patients, operated on using posterior approach, is caused by its traumaticity due to transsection of the muscles expanse, creation in them hematomas, significant quantity of the wound exudate. The perspectives of application of anterolateral approach in performing operations for fracture of the femur proximal end are proved.

ABSTRACT

We carried out researches with the purpose of comparison of operative treatment results of femur neck fractures in elderly and senile patients with those in other countries. Total hip replacement technique was used over the period of 1991-2002 yy. for 123 patients with femur neck fractures. We have used "Smith & Nephews" or "Zimmer" prostheses with "Polacos" or "Osteobond" acrylcements. Surgical approach tactics is very important for prophylaxis of early and late postoperative complications. Active and of sound mind patients with femur neck fractures should undergo total hip replacement procedure. Rehabilitation is also an important factor in preventive early and late postoperative complications.

ABSTRACT

Frequency is studied of adequate, redundant, inert, and reduced types of thermoreactivity in healthy subjects and patients with fractures. Definition of type of thermoreactivity to cooling in patients with fractures of the thighbone permits prognosticating the course of the bone fracture healing process. The symptom of distal hyperthermia/hypothermia is unspecific but is regarded as a supplementary index of the type of thermoreactivity and character of the course of the fracture healing process.

ABSTRACT

Investigations to compare the efficacy of existing metalloosteosynthesis methods, which are used in primary treatment of fracture of the femoral bone proximal segment, were performed by the author in patients older than 65 years. Results of operative treatment of fracture of the femoral bone neck, acetabular and subacetabular portions as well, were analyzed, optimal methods of operative treatment of these kinds of fractures were established.

ABSTRACT

The purpose of this study is to elucidate and analyze causes of difficulties encountered in dealing with fractures of the proximal segment of the femur and to set out our idea of solving these problems. An analysis is performed of classifications, an assessment is given of studying the adequate choice of a treatment modality, an attempt is made at formulating a doctrine of delivering qualified and specialized emergency medical service to those persons having been disabled for work, which could take account of all aspects of the injury under study. Indications for hemiarthroplasty are outlined, with the policy of primary sparing the joint elements by atraumatic osteosynthesis techniques being chosen in preference to any other policy.

ABSTRACT

Experience gained in different countries of the world with the use of existing classifications of the femur proximal end fractures (Muller, Pauwels, Garden) is discussed together with observations by Klenerman, Linton, Dorne, Erandsen, Kocher, and Nils Carlquist's scheme. Also submitted in the paper is the author's variant of classification based on characterization of the patient with regard for his/her concomitant afflictions, characteristics of the fracture sustained, the site of the fracture-related changes in blood supply.

ABSTRACT

Experience of operative treatment of the diaphysis femoris fracture in 673 patients was presented. Majority of operations were performed in 6-24 hours after the trauma occurrence. In 517 observations generally accepted method of osteosynthesis, and in 156--miniinvasive one, were applied. Results of intervention in 1-4 years in 391 patients were studied. Good result in 87.5%, satisfactory--in 9.9%, poor--in 3.3% were noted. Authors recommend to conduct operative treatment of the diaphysis femoris fracture as early as possible. Miniinvasive osteosynthesis is the preferable method of osteosynthesis in injured persons with severe polytrauma, multicomminuted and segmental fracture.

ABSTRACT

At the centre of traumatology, the Central District Clinical Hospital N 12 of Kiev, the authors have conducted investigations in order that on the basis of clinical and roentgenologic findings they might assess both early and long-term results of osteosynthesis of the femoral neck fractures with the aid of the elaborated device made of titanium. The studies were made in 43 patients aged 30 to 70 years over the period 1997-2000. Complications were recorded in 9.5 percent of cases. Osteosynthesis of the femoral neck fractures by making use of the elaborated device will, we believe, come to be efficiently used by traumatologists.

ABSTRACT

The article contains a description of a thermographic pattern of a healthy inferior limb and elucidates the question of thermographic characterization of healthy people. Reasons are provided for a limited application of the method of remote infrared thermography in the orthopedic and traumatological practice. Analyzing the results obtained the authors come to the conclusion that it is justifiable to use the method in the orthopedic and traumatological practice, in the diagnosis of inflammatory diseases of the locomotor system in particular. In addition, remote infrared thermography permits carrying out an early assessment of effectiveness of the treatments administered and allows its effects to be prognosticated.

ABSTRACT

Patent inventive discoveries in the USSR over the period 1949-1989 are analyzed together with the world patent discoveries, inventions, designs (Ukraine since 1989 included) over the period 1978-1998, which are divided into several groups different in principle, criteria of analysis are singled out, new notions of "cruel", dosage, dynamic compression are determined. By making use of principles of biological low-invasive metallo-osteosynthesis and new notions of "cruel", dosage, and dynamic compression optimal options were found for fixation of fractures of the femoral neck: low-traumatic nail set in the neck at an angle of 130 degrees fit with a small-sized diaphyseal put-in lamina and a local stimulation of union or spongiosa for the neck of the femur screw with an attachment preventing caput femoris rotation, and a springy element combined with a small-sized diaphyseal put-in lamina (that is, a modified system of the dynamic screw for the neck of the femur).

ABSTRACT

Patients with febrile attacks of schizophrenia showed clinical and laboratory correlations between the severity of the clinical picture on the one hand and parameters of the leukocyte index of intoxication, ESR, the NaK ratio and 17-CS secretion, on the other. The authors consider the possible causes of elevated levels of dopamine in the urine and of beta-lipoproteins and creatine phosphokinase in the cerebrospinal fluid. The study has revealed anti-brain antibodies and sensitization of leukocytes to neuroleptics. A possible role of neural viral infection in the etiology and pathogenesis of the studies conditions is discussed.
