ABSTRACT

Extreme ultraviolet spectra of highly-charged ytterbium ions produced in an electron beam ion trap at the National Institute of Standards and Technology were observed with a flat-field grazing incidence spectrometer in the wavelength region of about 4 nm-20 nm. The measured spectra were interpreted through detailed analysis by collisional-radiative modeling of the non-Maxwellian EBIT plasma. Seventy-nine new spectral lines due to intrashell (Δn = 0, n = 4) electric-dipole, magnetic-dipole, and electric-quadrupole transitions were identified in Rb-like Yb33+ through Ni-like Yb42+ ions. The effects of strong configuration interaction within the n = 4 complex on the measured spectra are discussed for a number of ionization stages.

ABSTRACT

The purpose of the study was to evaluate the number of inappropriate admissions to a smaller city hospital and find possible alternatives. Physicians and surgeons from three units (abdominal surgery, internal medicine and orthopaedic surgery) together with general practitioners, doctors on homecall duty and health personnel from the region contributed to the study. In all 421 consecutive patients were included during a three-week period. The patients' median age was 60.5 years. It was found that at least 13.6% of all patients seemed to have been admitted for an inappropriate reason. According to the admitting doctor 3.4% of the patients were not ill at all. The investigation implies that one out of seven acute admissions could be replaced by alternatives such as immediate care in residential homes, more flexible contact to the outpatient's clinic, better access to geriatric evaluation and improved laboratory service. We conclude that acute admission to hospital can be replaced by other alternatives, thereby achieving greater efficiency and better economics.

ABSTRACT

Traditionally, patients suffering from dementia are treated in psychiatric wards. In many geriatric patients, confusion/dementia is one of several problems prohibiting an early return to the previous way of living. To provide the best treatment, one of our geriatric wards was used for these patients. Based on our experiences from the first 58 patients discharged during the period 09.11.89-31.01.91, it was possible to create and secure a stable environment, with suitable activities for the satisfaction of the patients, their families and the staff. A structured procedure, including rating scales, was a great help in disclosing the patients' problems. One quarter of the patients were not demented, but temporarily confused due to other reasons. One third of the demented patients had no public assistance. It was necessary to discharge 60% of the demented and only 10% of the non-demented patients to nursing homes. Sheltered home-like living could be an alternative to the expensive nursing homes.
