ABSTRACT

BACKGROUND: This study explores Transcranial Pulse Stimulation (TPS) as a potential non-invasive treatment for Alzheimer's disease (AD), focusing on its impact on cognitive functions and behavioral symptoms. METHODS: In a prospective, one-arm open-label trial, ten patients with mild to moderate dementia due to AD were assessed using the Alzheimer's Disease Assessment Scale (ADAS-Cog), Neuropsychiatric Inventory (NPI), Pfeffer Functional Activities Questionnaire, and Zarit Caregiver Burden Interview. Assessments occurred at 30- and 90-days post-treatment. The TPS protocol consisted of 10 sessions over five weeks, using the Neurolith® device to deliver 6000 focused shockwave pulses at 0.25 mJ/mm2 and a frequency of 4 Hz. RESULTS: TPS significantly reduced neuropsychiatric symptoms, with NPI scores decreasing by 23.9 points (95% CI: -39.19 to -8.61, p = 0.0042) after 30 days, and by 18.9 points (95% CI: -33.49 to -2.91, p = 0.022) after 90 days. These changes had large effect sizes (Cohen's dz = 1.43 and dz = 0.94, respectively). A decreasing trend was observed in the ADAS-Cog score (-3.6, 95% CI: -7.18 to 0.00, p = 0.05) after 90 days, indicating a potential reduction in cognitive impairment, though not statistically significant. CONCLUSION: The preliminary results indicate that TPS treatment leads to significant improvement in neuropsychiatric symptoms in AD patients, showing promise as a therapeutic approach for AD. Further research is needed to fully establish its effectiveness, especially concerning cognitive functions.

ABSTRACT

Não há dúvidas sobre os prejuízos na vida humana, extensivos à família e sociedade como um todo, abrangendo todos os aspectos funcionais das pessoas envolvidas, além do indivíduo, originados dos traumatismos cranioencefálico, agudos e crônicos, externos ou internos: acidentes, atropelamentos, quedas, crimes, acidente vascular cerebral, doenças com neurodegeneração progressiva, resultando em estados demenciais. Ao longo de meio século, houve a introdução contínua de medicamentos, com resultados usualmente contraditórios e frustrantes, exigindo novas tentativas com outras classes farmacológicas. No todo, a limitação se faz sentir na impossibilidade de reversão ou mesmo de mera estabilização dos danos neurológicos, e inocuidade em termos de estimulação da neuroplasticidade. Uma exceção parece ser uma nova abordagem: a estimulação cerebral profunda por pulsos sonoros de baixa frequência (Transcranial Pulse Stimulation, ou TPS). Ainda pouco conhecida, a não ser em alguns centros de tratamento, tem se mostrado ser um acréscimo válido, por complementar os programas multidisciplinares de reabilitação


There is no doubt about the damage to human life, also extended to family and society as a whole, regarding all the functional aspects of those involved, not only the patient itself, which originates from traumatic brain injury, acute or chronic, for external or internal reasons, such as accidents, run overs, falls, crimes, stroke, progressive neurodegenerative diseases that result in dementia states. Over half a century, drugs have been continuously introduced, however their results have constantly been contradictory and frustrating, requiring new attempts with other pharmacological classes. Overall, the limitation is felt in the impossibility of reversing or even merely stabilizing the neurological damage and inefficacy regarding neuroplasticity stimulation. One exception seems to be a new approach, the non-invasive brain stimulation by low-frequency sound pulses (Transcranial Pulse Stimulation, or TPS). Except for some treatment centers, TPS is still unknown, however it has shown to be a valid adjunct in multidisciplinary rehabilitation programs

ABSTRACT

PURPOSE:: To analysis the effects of passive smoking on the microstructure of tissues of the abdominal wall regarding microcirculation, using histopathological study of the tobacco exposed rats. METHODS:: Twenty four male Wistar rats were divided in Control Group (CG = 8 animals) and Exposition Groups (EG = 16 animals). EG was exposed to cigarette smoke 4x/day for 120 days, while CG was preserved from exposure. Food, water and housing were similar for both groups. After 120 days, urine samples were collected before necropsy to analyze cotinine levels (ng/mL) in urine and blinded histopathological analysis of the abdominal wall performed to count arteries and veins in dermal and muscular fascia layer. RESULTS:: No difference in weight was observed between both groups (P>0.05). Cotinine concentration was significantly higher in EG (P<0.05). In dermal layer, the average of vessels per animal was 8.72 (IC95%: 8.31-9.13) for CG and 11,23 (IC95%: 10.09-12.38) for EG. In muscular fascia layer the average of vessels per animal was 17.97 (IC95%: 15.79-20.15) for CG, whereas the average for EG was of 14,85 (IC95%: 12.71-17.01) (P<0.05). CONCLUSION:: Exposition to passive smoking may cause increase in the number of vessels in dermal layer, with the opposite effects at the muscular fascia layer.

Este texto, sob a forma de verbete, feito sob gentil solicitação dos editores deste periódico, enfoca agrupamentos de pessoas observadas segundo o vértice psicanalítico, que pode ser visto como integração transdisciplinar entre, pelo menos, dois âmbitos de observação. Para finalidades de comunicação, esses âmbitos são definíveis por "níveis" ou "dimensões": macro e micro. Mais especificamente, trata-se do estudo das experiências emocionais que podem ocorrer no inter-relacionamento de pessoas que compõem um determinado grupo e também, conjuntamente, das características conscientes e inconscientes que podem agir. O estudo ressalta alguns antecedentes históricos e consequências em termos de comportamento; tanto o comportamento concreto, aparente, expresso (fenomenologicamente observável), como o comportamento implícito, imaterial, sempre atuante (denominado por Bion como "ultra" e "infra" sensorial, que não pode ser totalmente apreendido pelo aparato sensorial humano). Este estudo sobre grupos de pessoas identifica duas fantasias inconscientes, de natureza onisciente e onipotente, que compartilham da mesma aparência - a de postulados autoritários. Essas fantasias, compondo ideologias (ou Weltanschauungen), postulam duas tendências no pensar, segundo um vértice da história das ideias (ou, mais precisamente, duas tendências em que ocorre ausência do pensar). Para fins didáticos e operacionais, denominaram-se essas tendências de "quantitativa" e "qualitativa"; a partir daí, sempre sob o vértice psicanalítico, descreve-se o conceito de grupos de trabalho, e especificam-se métodos de estudo para grupos de pessoas; o estudo se finaliza com um breve elenco de experiências práticas em clínica que embasam este texto.(AU)


Organized under the form of an entry of a dictionary, this paper was written in response to the kind invitation from the editors of this journal. It focuses on groupings of people, observed under the psychoanalytic vertex. This vertex can be seen as a transdisciplinary integration between, at least, two fields of observation. Roughly speaking, just for communicative purposes, one may define those fields according to their "levels" and/or "dimensions": macro and micro (levels). Being more specific, this is a study of emotional experiences, which may happen in certain human group, and of conscious and unconscious features that may act on interrelationships among individuals who are parts of a group. The study emphasizes some historical antecedents as well as some effects, in terms of group behavior - both the concrete, evident, express behavior (phenomenally observable), and the implicit, immaterial and ever present behavior (which Bion named "ultrasensuous" and "infrasensuous", and cannot be fully apprehended by human sense apparatus). This study about groups of people identifies two omnipotent and omniscient phantasies that share the same appearance - the appearance of an authoritarian postulate. Composing ideologies (or Weltanshauungen), those phantasies propose two tendencies of thinking, according to a vertex of the history of ideas (or, more precisely, two tendencies in which mindlessness - or lack of real thinking - does prevail). For didactic, operational purposes, the author names these tendencies "quantitative" and "qualitative". From this point, always under the psychoanalytic vertex, he describes the concept of working group and specifies few methods for the study of groups. The author finishes this study with a brief list of experiences in clinical practice (cases) that support his theory.(AU)

ABSTRACT

The term gossypiboma denotes a cotton foreign body retained inside the patient during surgery, a rare surgical complication. The symptoms following this entity are non-specific, such as pain, palpable mass and fever, which make clinical diagnosis difficult. The computerized tomography (CT) scan is the most useful method for diagnosis; however, sometimes the preoperative diagnosis remains uncertain even after the imaging exam. In that case, laparoscopy arises as a valuable diagnostic tool, as well as a prompt treatment option. However, when diagnosis is made years after the original surgery, the laparoscopic approach becomes harder. Our patient presented without clear symptoms, remaining asymptomatic for 34 years. The CT scan presumptive diagnosis was a gastrointestinal stromal tumour, and laparoscopy was performed providing an accurate diagnosis and treatment in the same surgical time.

Senso comum, formulado por Locke, base da ciência moderna e da psicanálise, difere de lugar comum, ambas as concepções popularizadas e banalizadas no meio leigo, propagando-se nos movimentos de origem científica por dificuldades de aprendizado das contribuições de autores clássicos. O presente estudo examina criticamente, segundo o vértice psicanalítico e a formulação da história das ideias (Vico e Berlin), a concepção de tempo enquanto verdade absoluta, independente de espaço.(AU)


Kant’s criticism, a critic description of methodologies, was one of the roots which gave birth to Freud’s psycho-analysis as well as of all modern science. Common sense, as formulated by John Locke, is the main bearing of modern science as well as of psycho-analysis. It differs from common place, the popularized concepts obtained by banality which permeate the laymen’s environment and, alas, propagated to movements whose origin was scientific, due to difficulties of learning what the classical authors wrote. The present study scrutinizes critically, under a twofold method, the psycho-analytic vertex and the history of idea’s vertex (Vico and Berlin), the concept of “Time”, specially when it is regarded as an absolute truth, quite independent of “Space”.(AU)


El método crítico (“criticismo”) que aísla y describe metodologías de estudio formulado por Kant, fue una raíz del psicoanálisis de Freud - y de toda ciencia moderna. Sentido común, formulado por Locke, base de la ciencia moderna y del psicoanálisis, difiere de lugar común, concepción popularizada y banalizada en el medio lego, propagándose en los movimientos de origen científico, por dificultades de aprendizaje de las contribuciones de autores clásicos. This study offers a possibly complete, commented and comprehensive analytical index, as far as the intentions and information of this author were available to him, out fromWilfred Ruprecht Bion’s poetical and literary inspirations that were used in his theoretical and technical contributions both to psycho-analysis and observation of the psychoanalyticact. Those inspirations were explicitly and implicitly used by him. The paper follows the lead initiated by previous studies, available in book form, about psycho-analytical roots, andinspirations stemming theories of knowledge, including philosophy of mathematics used bythe same author. Un índice analítico comentado, compactado y también comprehensivo, tal vez completo, de las inspiraciones usualmente consideradas como poéticas y también literarias que son detectables implícita o explícitamente en las contribuciones de Wilfred Ruprecht Bionsegún fueron formuladas en algunas de sus teorías y técnica en psicoanálisis y observación psicoanalítica. This study offers a possibly complete, commented and comprehensive analytical index, as far as the intentions and information of this author were available to him, out fromWilfred Ruprecht Bion’s poetical and literary inspirations that were used in his theoretical and technical contributions both to psycho-analysis and observation of the psychoanalyticact. Those inspirations were explicitly and implicitly used by him. The paper follows the lead initiated by previous studies, available in book form, about psycho-analytical roots, andinspirations stemming theories of knowledge, including philosophy of mathematics used bythe same author.


Un índice analítico comentado, compactado y también comprehensivo, tal vez completo, de las inspiraciones usualmente consideradas como poéticas y también literarias que son detectables implícita o explícitamente en las contribuciones de Wilfred Ruprecht Bionsegún fueron formuladas en algunas de sus teorías y técnica en psicoanálisis y observación psicoanalítica. Este pequeño trabajo se ubica en el ciclo más amplio de las descripciones ydelimitaciones de los orígenes psicoanalíticos y de la teoría del conocimiento, incluso de la filosofía de la matemática, detectables en la obra del mismo autor, objeto de publicaciónanterior. El redactor de este texto agradece anticipadamente las contribuciones tendientes a complementar el índice, debido al fracaso involuntario o a la inevitable ignorancia.

Considerações analíticas e críticas a respeito de concepções e pré-concepções sobre o que se denomina tempo e espaço, originadas de investigação psicanalítica e de algumas ciências que tem se dedicado a desenvolver tais concepções. As considerações brangem uma história das idéias (segundo Berlin), interessado à teoria do conhecimento, e incluem o método crítico (segundo Kant, depois desenvolvido por freud). Limitações ligadas a concepções exclusivas, que pretendem alcançar uma verdade única ou absoluta, são explicitadas, além de alternativas a elas (AU)


Analytical and critical considerations about conceptions and pre-conceptions of that which is commonly named as time and space, stemmed from psycho-analytic research as well as from some sciences that had tradicionally dealt with those conceptions forms the core of this paper. The considerations encompass a history of ideas (after Berlin), and are linked to epistemology; they try to apply Kant's criticism, later developed by Freud. The paper points out some unobserved limitations linked to conceptions which pretend to reach an exclusive and only absolute truth as well as some alternatives to them (AU)
