ABSTRACT

La arteritis de Takayasu (AT) es una enfermedad inflamatoria infrecuente de vasos grandes. A menudo, el daño crónico originado en las grandes arterias requiere de revascularización mediante prótesis vasculares. Excepcionalmente la implantación de las prótesis origina lesiones en otros órganos, como los uréteres, los cuales pueden obstruirse por compresión extrínseca por el tejido fibroso retroperitoneal originado como reacción a la presencia de la prótesis vascular.Se comunica el caso de una mujer que presentó hidronefrosis bilateral en forma asincrónica después de los 2 años de la instalación de una prótesis aorto-ilíaca con insuficiencia renal aguda, la cual pudo revertirse mediante. Takayasu's arteritis is a rare inflammatory disease of large vessels. Often, chronic damage resulting in large arteries stenosis requires revascularization with vascular prostheses. Exceptionally, the presence of prostheses causes lesions in other organs, such as ureters, which are subject to extrinsic compression caused by retroperitoneal fibrous tissue originated as a reaction to the presence of vascular prostheses. We report the case of a woman who presented bilateral asynchronous hydronephrosis after 2 years of the installation of an aorto- iliac graft with acute renal failure, which could be reversed by releasing the ureter from the periprosthetic fibrous retroperitoneal tissue.(AU)

ABSTRACT

AIM OF THE STUDY: Weight in kilograms is a required parameter in the emergency medical care of children. In emergent situations, obtaining an accurate weight is often not possible. In such situations, weight can be estimated by using an age-dependent formula such as the EPLS-formula (age in years+4)×2. As recently recognized for emergency tapes, the habitus of the child has a major influence on weight estimation. In this study, the performance of various age-dependent formulas is to be investigated, with special regard to children demonstrating non-normal growth. METHODS: The performance of various formulas for weight estimation in children growing along the 5th, 50th, and 95th percentile is investigated based on a mathematical model compared to the WHO and CDC reference percentiles using ICC and Bland-Altman methods. Additionally, a new formula for children demonstrating non-normal growth is derived by regression analysis and tested: f×age in years+6 with the factor f being 2 for "tall n' thin", 3 for normal and 4 for "tiny n' thick" children. RESULTS: All previously published formulas lack precision when applied to children outside the 50th percentile. The new habitus-adapted formula shows a better performance for children growing along the 5th or 95th percentile. CONCLUSIONS: The new formula provides enhanced precision in weight estimation and can help in reducing, e.g. drug dosing errors. It should be used for weight estimation in children demonstrating non-normal weight development and in situations when superior methods such as weighing or habitus-adapted emergency tapes are not applicable.

ABSTRACT

Se describe el caso de un paciente operado por embolia pulmonar masiva en el Instituto de Cardiología y Cirugía Cardiovascular. Este constituye el primer paciente que sobrevive a esta operación en nuestro país. Por los antecedentes que presentaba el enfermo, se realizó el diagnóstico presuntivo de tromboembolismo pulmonar, el cual fue confirmado al ser estudiado angiográficamente de urgencia en nuestro centro. Fue sometido a una operación donde se utilizó derivación cardiopulmonar, se efectuó la extracción de los trombos intrapulmonares y en el posoperatorio evolucionó sin complicaciones.
