ABSTRACT

AIM: This study evaluated the transdentinal cytotoxic effects of enzymatic agents (EA) for chemomechanical carious tissue removal on human dental pulp cells. METHODOLOGY: The groups were based on the performed dentine treatments (n = 8): G1: Positive Control (PC - no treatment); G2: Negative Control (NC - 35% H2 O2 for 2 min); G3: Brix 3000™ (BX) for 30 s; G4: BX for 2 min; G5: Papacarie Duo™ (PD) for 30 s; G6: PD for 2 min. The cells were evaluated for viability (VB; MTT assay) and production of reactive oxygen species (ROS; DCFH-DA assay) and nitric oxide (NO; Griess reagent). A scanning electron microscope provided morphological chemical analyses and energy-dispersive X-ray spectroscopy. The data were submitted to the one-way anova statistical test complemented by Tukey (p < .05). RESULTS: Cell viability decreased by 21.1% and 58.4% in G5 and G6, respectively. ROS production in G3 and G4 maintained basal levels but increased by 171.2% and 75.1% in G5 and G6, respectively. CONCLUSIONS: The Brix3000™ enzymatic agent did not cause indirect cytotoxic effects on pulp cells, regardless of the application time. Conversely, Papacarie Duo™ reduced viability and increased ROS production by pulp cells.

ABSTRACT

Uma das tendências da odontologia é a área da estética que consiste na harmonização entre dentes, gengivas, lábios e face. O objetivo deste trabalho é relatar um caso clínico de reanatomização dentária pela técnica restauradora direta com resina composta, onde a queixa principal do paciente se baseava nos espaços interdentais e formato dos dentes na região anterior. Foi elaborado um plano de tratamento que consistiu inicialmente na realização da moldagem de estudo, enceramento diagnóstico e mock-up, onde foi detectada a necessidade de uma gengivoplastia. Em seguida foi realizado um tratamento clareador e então o procedimento restaurador pela técnica direta com resina composta. Sendo assim, obteve-se um resultado satisfatório, suprindo as expectativas do paciente, conservador, pois não houve necessidade de desgaste dentário e com grande previsibilidade técnica.


One of the trends of dentistry, is the area of aesthetics, consists of harmonization between teeth, gums, lips and face. The objective of this study is to report a clinical case of dental reanatomization using the direct composite resin restorative technique, where the patient's main complaint was based on interdental spaces and tooth format in the anterior region. For the solution of the case the following procedures were performed: study molding, diagnostic waxing, mock-up, through this procedure the need for a gingivoplasty was detected, followed by a bleaching treatment and then the restorative procedure by the direct resin technique composed. Thus, a satisfactory result was obtained, supplying the patient's expectations, conservative because there was no need for dental wear and with great technical predictability.

ABSTRACT

Objetivo: o objetivo deste trabalho é relatar um caso clínico de hipersensibilidade severa associada a múltiplas lesões cervicais não cariosas, entender o mecanismo de ação e a eficácia dos agentes dessensibilizantes. Relato de caso: paciente do gênero masculino, 38 anos de idade, compareceu à clínica da Faculdade Morgana Potrich com um quadro de hipersensibilidade em vários elementos, bem como, a presença de lesão cervical não cariosa em alguns desses elementos. Durante a anamnese o paciente relatou o hábito de escovar os dentes várias vezes ao dia logo após se alimentar, exercendo muita força durante o ato, relatou também fazer consumo diário de cerveja que possui um pH ácido. Ao realizar análise da oclusão notou-se um desequilíbrio oclusal e contato prematuro nos dentes 24 e 34. O tratamento de escolha para esse caso foi o ajuste oclusal seguido do uso de dessensibilizantes e restaurações em resina composta nos dentes que havia a presença de lesões cervicais não cariosas com perda de estrutura dentária. Considerações finais: após esse tratamento, realizado em várias sessões, foi possível devolver ao paciente a função e eliminar a dor provocada pela hipersensibilidade, proporcionando uma melhor qualidade de vida para esse paciente.


Objective: This study aims to report a clinical case of severe hypersensitivity associated with multiple non-carious cervical lesions and to understand the mechanism of action and the efficacy of desensitizing agents. Case report: A 38-year-old male patient attended the clinic at Faculdade Morgana Potrich with the hypersensitivity of several elements and the presence of non-carious cervical lesion in some of these elements. During anamnesis, the patient reported the habit of toothbrushing several times a day soon after eating, exerting a lot of force in such activity; he also reported the daily consumption of beer, which has an acidic pH. The occlusal analysis showed occlusal disorder and premature contact in teeth 24 and 34. The treatment of choice for this case was the occlusal adjustment followed by the use of desensitizers and composite resin restorations on the teeth with non-carious cervical lesions with loss of dental structure. Final considerations: After this treatment, which was performed in several sessions, it was possible to return function to the patient and eliminate the pain caused by hypersensitivity, providing a better quality of life for this patient.(au)

ABSTRACT

Unsatisfactory anatomical shape of anterior tooth promotes disharmony in the smile esthetics. The conservative dental esthetic reestablishment treatments has been improved and evaluated with the development of adhesive materials. The adhesive dentistry allowed minimally invasive preparation through direct treatments with composite resin and indirect ceramic laminates veneers. These two treatments methods present advantages and disadvantages. The aim of this manuscript is to describe two clinical reports and compare alternatives to rehabilitate the smile esthetic appearance by direct and indirect minimally invasive treatments. In the first case, a 24-years-old patient, female, complained of unsatisfactory anterior maxillary size and incisal wear. After the functional and esthetic planning, nanoparticle composite resin by free hand technique was indicated to rehabilitation. In the second case, a 24-years-old patient, male, reported dissatisfied with his anterior central incisive shape. On clinical examination, maxillary anterior conoid teeth were verified. Based on this evaluation, a conservative treatment using minimally invasive ceramic veneers in maxillary central and lateral incisor and ceramic fragments on canine was planed to the smile rehabilitation. It can be concluded that conservative treatment using direct composite resin restorations and minimum thickness ceramic veneers laminate, even with their specific limitations and advantages, provide satisfactory esthetic and functional treatment results, preserving the dental structure.


Dentes com formato anatômico insatisfatório podem causar desarmonia na estética do sorriso. Os tratamentos para restabelecer a estética do sorriso vêm evoluindo concomitantemente com o desenvolvimento dos materiais dentários. A odontologia estética permite um preparo minimamente invasivo através de resinas compostas e laminados cerâmicos. Estes dois métodos de tratamento apresentam algumas vantagens e desvantagens. O objetivo deste artigo é descrever dois relatos de casos clínicos e comparar alternativas para a reabilitação da estética do sorriso pelos tratamentos minimamente invasivos diretos e indiretos. No primeiro caso, paciente de 24 anos do gênero feminino, estava descontente com o tamanho e com o desgaste incisal dos dentes maxilares anteriores. Após o planejamento funcional e estético foi indicado restauração direta com uma resina nanopartículada pela técnica da mão livre. No segundo caso, paciente de 24 anos do gênero masculino relatou insatisfação com a forma dos incisivos centrais superiores. No exame clínico foi observado incisivos conoídes. Baseado no exame clínico foi planejado um tratamento conservador com laminados cerâmicos nos incisivos centrais e laterais, e fragmentos cerâmicos nos caninos. Pode-se concluir que os tratamentos conservadores utilizando resina composta e laminados cerâmicos, mesmo com suas limitações, promove uma estética satisfatória e resultados funcionais preservando a estrutura dental.

ABSTRACT

O traumatismo dental acomete frequentemente crianças e adolescentes, comprometendo, principalmente, os dentes permanentes. Dentre as injúrias traumáticas, as fraturas coronárias não complicadas ocorrem mais comumente. Nesses casos, o tratamento ideal é a colagem do fragmento dental para restabelecer função e estética, devolvendo qualidade de vida ao paciente. Diante disso, o objetivo desse trabalho foi apresentar relato de caso clínico, de paciente de gênero feminino, envolvendo a colagem de fragmento após fratura coronária do dente 11. O fragmento foi reposicionado e colado adesivamente com resina composta ao remanescente dental, resgatando a estética, função e autoestima da paciente. Portanto, a colagem de fragmentos dentários representa uma técnica eficaz, pois é rápida, fácil, de excelente resultado estético e funcional (AU).


Dental trauma often affects children and adolescents, compromising mainly permanent teeth. Among traumatic injuries, uncomplicated coronary fractures are the most common. In such cases, the ideal treatment is the bonding of the dental fragment to reestablish function and esthetics, restoring patients' quality of life. The aim of this study was to present a clinical case report of bonding of a fragment after coronary fracture of incisor central tooth in a female patient. The fragment was repositioned and adhesively bonded using composite resin to the dental structure, restoring aesthetics, function and self-esteem. Therefore, the bonding of dental fragments represents an effective technique, since it is fast and simple, with excellent esthetic and functional result (AU).

ABSTRACT

Amelogenesis imperfecta is a hereditary condition that can alter the thickness, color, and shape of tooth enamel. Recent adhesive materials and techniques have provided less invasive treatment options. This case report presents the treatment of a patient whose anterior teeth had color alterations, white spots, pits, and shape defects. Using a more conservative technique, the mandibular and maxillary anterior teeth were restored using veneer direct composite restorations. After 6 years, the restorations demonstrated no deterioration, and no pathology was seen in association with the rehabilitation.

ABSTRACT

With the advent of new adhesive technologies, fiber-reinforced composite fixed partial dentures (FRC-FPDs) have become an inexpensive, esthetic and useful restorative option for patients missing a single tooth. This article describes a case involving an inlay-retained FRC-FPD with a prefabricated fiber-reinforced pontic substructure. In addition, a 6-year follow-up is presented. The FRC-FPD fully restored the single missing molar with simple and conservative intracoronal preparations in the proximal teeth and was deemed clinically acceptable after the evaluation period. The clinical procedures to build a FRC-FPD with a prefabricated fiber-reinforced pontic substructure typically are finished by the clinician in 1 or 2 visits without laboratory assistance, making it a viable alternative for patients who cannot afford implant-based treatment or a conventional metal-ceramic FPD.

ABSTRACT

Restoring flared endodontically treated teeth continues to be a challenge for clinicians. This study evaluated the effect of post types and restorative techniques on the strain, fracture resistance, and fracture mode of incisors with weakened roots. One hundred five endodontically treated bovine incisors roots (15 mm) were divided into 7 groups (n=15). The two control groups were (C) intact roots restored with Cpc (cast posts and core) or Gfp (glass fiber posts). The five experimental groups were (F) flared roots restored with GfpAp (Gfp associated with accessory glass fiber posts), GfpRc (anatomic Gfp, relined with composite resin), and GfpRcAp (anatomized Gfp with resin and accessory glass fiber posts). All teeth were restored with metal crowns. Mechanical fatigue was performed with 3x10(5)/50 N. Specimens were loaded at 45º, and the strain values (µS) were obtained on root buccal and proximal surfaces. Following that, the fracture resistance (N) was measured. One-way ANOVA and Tukey's HSD tests (α=0.05) were applied, and failure mode was checked. No significant difference in strain values among the groups was found. Cpc presented lower fracture resistance and more catastrophic failures in flared roots. Gfp associated with composite resin or accessory glass fiber posts seems to be an effective method to improve the biomechanical behavior of flared roots.

Restaurar dentes tratados endodonticamente continua sendo desafio para clínicos. Este estudo avaliou o efeito de pinos e técnicas na deformação, resistência à fratura e padrão de fratura de incisivos com canal radicular alargado. Cento e cinco raízes bovinas, tratadas endodonticamente (15 mm) foram divididas em 7 grupos (n=15). Os grupos controle (C), constituídos de raízes não alargadas, foram restauradas com Cpc (núcleo metálico fundido) ou Gfp (pino de fibra de vidro). Nos grupos experimentais os canais foram alargados (F) e restaurados com: GfpAp (Gfp associado com pinos de fibra de vidro acessórios); GfpRc (pino anatômico, reembasado com resina composta) e GfpRcAp (pino anatomizado com resina composta e pinos acessórios). Os dentes foram restaurados com coroas metálicas. Fadiga mecânica foi simulada com 3x10(5)/50 N ciclos. O teste foi realizado a 45º e a deformação (μS) obtida nas superfícies vestibular e proximal. Em seguida, a resistência à fratura (N) e o padrão de fratura foram verificados. Aplicou-se ANOVA e Teste de Tukey (α=0,05). Não houve diferença na deformação. Cpc resultou em menor resistência à fratura e com mais fraturas catastróficas em raízes fragilizadas. As técnicas de reembasamento do pino com resina composta ou o uso de pinos acessórios parecem ser efetivos para melhorar o comportamento biomecânico de raízes fragilizadas.

AIM: The aim of this article is to describe a step-by-step protocol for emergency care of a patient with a dentoalveolar injury in the anterior region of the mouth as well as the fabrication of a mouthguard to prevent future trauma. BACKGROUND: Dental trauma is one of the most serious oral health problems in active children and adolescents. Care of traumatized patients requires immediate initial emergency treatment followed by integrated procedures to restore damaged oral structures along with a subsequent trauma prevention strategy. Dentoalveolar injuries in the anterior region of the mouth are often characterized by tooth avulsion and coronal fracture. They are managed using procedures such as dental splinting, endodontic therapy with its unique characteristics, and restorative techniques to re-establish function and esthetics as well as protective mouthguards. REPORT: A 16-year-old male presented with avulsion of his maxillary central incisors as a result of a direct, unintentional impact with an opponent during a basketball game. The teeth had been stored in physiological serum immediately following the injury and the patient received immediate care. On clinical examination, the right central incisor was fractured at the incisal third of the crown but no bone fractures were found. The teeth were reimplanted and splinted. The fractured right central incisor was restored following endodontic treatment and a mouthguard was fabricated for the patient. SUMMARY: The dentist must be knowledgeable about the most efficient and suitable treatment for each traumatic scenario in order to provide appropriate care for dental injuries. Coordinated multi-disciplinary action is fundamental in the successful treatment of these injuries. The dental mouthguard is an effective device for protecting the teeth and supportive structures during physical activities and must be part of the protective equipment used by athletes. It is the responsibility of the dental professional to make parents, trainers, and athletic associations aware of the risks associated with physical activities without orofacial protection; this should encourage the proper use of all protective devices to prevent dentoalveolar injuries that compromise oral functions, esthetics, and increase the cost of healthcare.

Este trabalho avaliou o efeito de dessensibilizante e agente reidratante na resistência adesiva ao cisalhamento à dentina. Cento e trinta cinco incisivos bovinos tiveram suas superfícies vestibulares lixadas produzindo uma superfície plana de dentina que receberam os seguintes tratamentos: G1- One Step Plus (OSP) e cimentação adesiva do cilindro de resina (CR); G2- Gluma One Bond (GOB) e CR; G3- Single Bond (SB) e o CR; G4- Aqua-Prep (AP) + OSP e CR; G5- Gluma Desensitizer (GD) + GOB e CR; G6- GD + SB e CR; G7- GD + OSP e CR; G8- AP + GOB e CR; G9- AP + SB e CR. Os espécimes foram armazenados em 370C e umidade de 100% por 24 horas e o teste de resistência adesiva foi executado com máquina de ensaio mecânico, em uma velocidade de 0.5 mm/min. Os dados foram submetidos a análise de variância seguido pelo teste de Tukey (p<0.05). Os resultados (MPa) foram: G1: 10,75(2,64)a; G2: 10,28(2,58)a; G3: 11,63(4,59)a; G4: 10,93(4,88)a; G5: 10,15(3,95)a; G6: 11,82(4,14)a; G7: 9,85(2,15)a; G8: 5,48(1,94)b; G9: 10,62(2,83)a. A resistência adesiva do sistema adesivo GOB foi negativamente afetada pela aplicação do AP. O uso de dessensibilizante e agente reidratante não compromete a resistência adesiva quando são compatíveis com o cimento resinoso adesivo utilizado.

ABSTRACT

Os autores apresentam caso clínico que envolve escurecimento dental com presença de incisivos conóides e restaurações anteriores insatisfatórias. O objetivo deste trabalho é demonstrar, passo a passo, a técnica de clareamento dental caseiro com peróxido de carbamida 10 por cento (Whiteness FGM) associado ao emprego de resina composta na tranformação dental. Serão abordados a interferência e os cuidados a serem observados nas restaurações adesivas de dentes submetidos ao clareamento dental, objetivando a obtenção de resultado estético e funcional com longevidade. A satisfação do paciente, com reflexo direto no sucesso profissional, demonstra que a realização de procedimentos estéticos pouco invasivos é importante aliada do Clínico na prática da Odontologia Estética
