ABSTRACT

Aortitis is one of the many possibilities of lesion caused by tertiary syphilis. Between all the cardiovascular lesions, the aorta's injury is the most common. We report a case of a 48-year-old patient diagnosed with syphilitic aortitis who had undergone surgery for the replacement of the aortic root and aortic valve. The diagnosis hypothesis was pondered because of the in situ aspect of the arterial damage. Although the rarity of the disease, it persists.

ABSTRACT

A aortite é uma das muitas possibilidades de lesão produzida pela sífilis terciária. Dentro das lesões cardiovasculares, o acometimento da aorta é o mais comum. Apresentamos o caso de um paciente de 48 anos, diagnosticado com aortite sifilítica, sendo realizada cirurgia para substituição de raiz da aorta e valva aórtica. A hipótese diagnóstica foi devido ao aspecto do dano arterial in situ. Apesar da raridade da doença atualmente, ainda é uma possibilidade.

ABSTRACT

The superior left vena cava with the absent superior vena cava is a rare abnormality, especially when associated with total heart block. We report a case of a patient in which the presence of superior left vena cava and the absence of the superior vena cava was detected during the implantation of a pacemaker for the correction of a total heart block, which led us to use a different technique for the fixation of the ventricular electrode. To confirm the supposed absence, several image exams were made showing the difficulty on the diagnosis of the described syndrome.

ABSTRACT

A persistência de veia cava superior esquerda com ausência da veia cava superior é uma anomalia rara, principalmente quando associada a bloqueio atrioventricular de 3º grau. Relatamos o caso de uma paciente, na qual durante implante de marca-passo definitivo, para a correção de bloqueio atrioventricular total, foi detectada presença de veia cava superior esquerda com suspeição de ausência de veia cava superior, o que levou ao emprego de técnica diferenciada para fixação do eletrodo ventricular. Para confirmação da provável agenesia, foram realizados diversos exames complementares de imagem, demonstrando-se a dificuldade no diagnóstico da síndrome aqui descrita.

The purpose of the paper is to demonstrate the case of a 62-year-old man, with spontaneous coronary dissection of the left circumflex artery, treated surgically by myocardial revascularization. The successfully accomplished surgery demonstrates, in this case, to be the only possible way of a cure.

O objetivo do trabalho é demonstrar o caso de um homem de 62 anos, com quadro de dissecção coronariana espontânea, localizada em 1/3 inicial de coronária circunflexa esquerda, tratado cirurgicamente com revascularização miocárdica. A operação realizada com sucesso demonstra, nesse caso, ser o único meio possível de cura.
