ABSTRACT

INTRODUCTION: This study aimed to evaluate whether there is discrimination in the dental educational environment, assess the main reasons for the discriminatory events, and if there is an association between discriminatory episodes and sociodemographic characteristics of undergraduate dental students. MATERIALS AND METHODS: This observational cross-sectional study was conducted with a self-administered questionnaire to students enrolled in three Brazilian dental schools. Questions included sociodemographic characteristics and the occurrence of discriminatory episodes in the dental academic environment. Descriptive analysis was performed in RStudio 1.3 (R Core Team, RStudio, Inc., Boston, USA) software and the associations were tested using Pearson's chi-square test, considering 95% confidence intervals. RESULTS: A total of 732 dental students were included, with a response rate of 70.2%. The vast majority of students were female (66.9%), with white/yellow skin colour (67.9%), and with a mean age of 22.6 (SD 4.1) years. Sixth-eight percent of students reported having experienced some discrimination in the academic environment and most reported feeling uncomfortable with the episode. The main reasons to have been discriminated against raised by students were specific behaviour/habit, have specific moral, ethical, and aesthetic values, gender, and socioeconomic status or social class. The occurrence of discriminatory episodes was associated with female gender (p = .05), non-heterosexual sexual orientation (p < .001), studying in public institutions (p < .001), receiving an institutional scholarship (p = .018), and being in the final undergraduate cycle (p < .001). CONCLUSION: The occurrence of discriminatory episodes was common in Brazilian dental higher education. Discriminatory situations generate traumas and psychological marks, causing a loss of diversity within the academic environment that leads to loss of productivity, creativity, and innovation. Thus, strong institutional policies against discrimination are crucial to create a healthy dental academic environment.

ABSTRACT

Aim: To assess the associations between sociodemographic and undergraduate-related characteristics and empathy scores among dental students in a school in Southern Brazil. Materials and methods: Cross-sectional study developed from self-administered online questionnaires to undergraduate dental students at the Federal University of Pelotas. Outcomes were the total score of empathy obtained through the Interpersonal Reactivity Index (IRI) and the scores obtained in four domains: perspective-taking (PT), fantasy (FA), empathic concern (EC), and personal distress (PD). In RStudio version 4.1.3 simple and adjusted linear regression models with robust residual standard errors were performed. Results: Eighty-seven students were included (response rate 24.4%). Considering the total IRI score, there was a mean score of 3.62 (SD=0.64) by item, and dental students had a mean score of 94.07 (SD=16.62). In adjusted analysis, being a woman increased the IRI (0.505;95%CI 0.187;0.823), EC (0.494;95%CI 0.168;0.819), and PT (0.822; 95%CI 0.329;1.315) scores compared to men. Adjusted associations were found between the EC domain and skin color and between the PT domain and family income. FA scores increased with age and family income and, decreased with dissatisfaction with undergraduate studies, only in the bivariate analysis. Discussion: Dental professionals' empathy is essential in daily practice, improving the patient-professional relationship in a patient-centered care approach. Thus, it is relevant to recognize predictors of empathy among dental students to promote strategies to increase empathy in dental practice. Conclusion: Gender, skin color, family income, age, and satisfaction with undergraduate studies were associated with empathy scores among dental students.

ABSTRACT

OBJECTIVE: To describe the temporal trend of notifications of physical, sexual and emotional violence and neglect against children in Brazil between 2011 and 2019. METHODS: This was an ecological time-series study based on notifications of violence against children aged 0-9 years held on the Brazilian Notifiable Health Conditions Information System. Age-adjusted notification rates were calculated for Brazil as a whole, by national macro-region and by sex. Trends were assessed using Joinpoint Regression. RESULTS: We analyzed 88,820 notifications of physical violence, 87,141 notifications of sexual violence, 52,359 notifications of emotional violence and 166,664 notifications of neglect. A rising trend was identified for notifications of physical, sexual and emotional violence and neglect for Brazil as a whole and for both sexes. Neglect accounted for the highest rate (95.24 notifications per 100,000 children in 2019). The Northeast and Southeast macro-regions had rising trends for all forms of violence. CONCLUSION: Notifications of physical, sexual and emotional violence and neglect showed rising trends in Brazil in the period studied. IMPLICATIONS FOR SERVICES: Increasing trends in notifications of violence against children highlight the continued need for capacity building in health services, crucial for early detection, effective prevention and coordinated intervention, taking regional variations into account. PERSPECTIVES: Notification of violence against children by health services requires greater commitment by health workers. Future studies could combine multiple national databases and surveys to increase the accuracy of rates and trends.

ABSTRACT

PURPOSE: This study aimed to evaluate whether discriminatory episodes occurring in the university environment were associated with dental students' self-perceived overall quality of life and assess the cumulative effect of perceived discriminatory experiences on the overall quality of life. METHODS: In this cross-sectional study, all students enrolled in three Brazilian dental schools were invited to participate in a survey between August and October 2019. The outcome was students' self-perceived quality of life, measured through the overall quality of life item of the World Health Organization-Quality of Life Abbreviated Version (WHOQOL-BREF). Descriptive, bivariate, and multivariable logistic regression analyses (95% confidence intervals and α of 5%) were conducted using RStudio software. RESULTS: The sample was composed of 732 students (70.2% response rate). The great part was female (66.9%), white or yellow skin color (67.9%), and were children of highly educated mothers. About 68% of the students reported having experienced at least one of the seven discriminatory experiences presented in the questionnaire and 18.1% reported neutral or negative quality of life. In multivariable analyses, it was estimated that students who experienced at least one episode of discrimination were 2.54 times (95% CI: 1.47-4.34) more likely to report worse quality of life than their counterparts who reported no experience of discrimination. There was also a 25% (95% CI: 1.10-1.42) increase in the odds of reporting poorer quality of life for each additional discriminatory experience reported. CONCLUSION: Reporting at least one discriminatory situation in the academic environment was associated with worse quality of life among dental students, and a cumulative effect was also noted.

ABSTRACT

OBJECTIVES: Obtaining robust evidence about the local mortality levels, trends and impact of oral cavity/base of tongue cancers and lip cancer, especially for women, is imperative in the fight against cancer. This descriptive retrospective ecological time-series study explored trends in oral cavity/base of tongue cancers and lip cancer mortality rates for women in Brazil from 1980 to 2018, by geographic region and anatomical location. METHODS: The crude and age-adjusted annual mortality rates were obtained by sex, anatomical location and macro-regions of Brazil. The number of deaths from oral cavity/base of tongue cancers and lip cancers in Brazil was based on official population counts and estimates. The annual percentage change was calculated based on age-adjusted rates. Data set were analysed using the Joinpoint Regression program. RESULTS: A total of 81,918 individuals died of oral cavity/base of tongue cancers and lip cancer between 1980 and 2018 in Brazil. The age-adjusted mortality rate for women was 0.47 and 0.57 per 100,000 in 1980 and 2018, respectively. The cumulative female mortality rates standardized by age were 0.01/100,000 for lip cancer and 0.5/100,000 for oral cavity and base of tongue cancers. A decrease in deaths related to oral cavity and base of tongue cancers was identified in the 1980s; however, over the last two decades, there has been an increase in the number of deaths of women with cancer at the base of tongue and neighbouring areas and on the floor of mouth. Importantly, Brazilian regions showed wide variability in trends of oral cavity, base of tongue and lip cancers rate and, in 2018, the regions with the highest rates were the Southeast, South and Northeast for both sexes and specifically for women. The North region showed the greatest recent significant upward trend. CONCLUSIONS: During the last 38 years, Brazil has shown a significant increase in the trend of the mortality rate due to oral cavity/base of tongue and lip cancers in women. Preventive strategies with control of risk factors should be strongly emphasized in order to improve the survival rates of individuals with oral cavity/base of tongue and lip cancers.

ABSTRACT

Objective: To investigate the clinical management of deep occlusal carious lesions in permanent teeth by Brazilian dentists. Material and Methods: This cross-sectional study included a sample of 732 Brazilian dentists who responded to an electronic questionnaire composed of 20 questions addressing socio-demographic information, training and professional activity characteristics, and clinical management of deep carious lesions. Descriptive analysis was performed, considering relative and absolute frequencies and 95% confidence intervals. The association between the dentists' variables of interest (age group, type of higher education institution, years in practice, highest academic degree completed and main sector of professional activity) and the mean number of incorrect answers regarding deep carious lesions management was analyzed by Analysis of Variance (ANOVA), followed by the Bonferroni post-test (p<0.005). Results: Stepwise removal was the strategy pointed out by most dentists (42.8%). However, 16% of the dentists selected nonselective carious tissue removal. Furthermore, 45.6% of the dentists disagreed with maintaining carious tissue over the pulp in deep lesions with a risk of pulpal exposure. When using instruments to remove carious dentine, 17% of the dentists chose a diamond burn while 13% preferred tungsten carbide burs. Dentists who graduated from public institutions had a lower mean of non-conservative decisions than those from private institutions. Dentists with master's or doctoral degrees were more conservative, as well as dentists from public service compared to those from the private sector. Conclusion: The dissemination of conservative approaches in the management of deep carious lesions needs to be strengthened and increasingly based on updated scientific literature.(AU)

ABSTRACT

Aim: The aim of the present study is to assess the perception of undergraduate students on the impact of the COVID-19 pandemic on Brazilian dental education. Methods: This crosssectional study was conducted using an online questionnaire hosted in Google Forms platform and publicized on Instagram® and Facebook®. The questionnaire was available between July 8-27, 2020. Absolute and relative frequencies were obtained for variables of interest using Pearson's chi-squared and considering 95% confidence intervals. Prevalence of learning resource variables according to self-reported skin color, educational institution, and Brazilian region were presented using equiplots. Results: A total of 1,050 undergraduate dental students answered the questionnaire. Most students reported being in full-distance learning mode. Among the undergraduate students, 65.4% reported perceiving a very high impact in dental education, and 16.6% of students reported not being able to follow distance learning. In addition, 14% reported not having a personal laptop or desktop to study with, with this condition being more prevalent among non-white than white individuals. Conclusion: We conclude that Brazilian dental students perceived a high impact of COVID-19 on dental education, as well as one-sixth of the students reported not having adequate resources to continue with distance learning. It is important that different policies are developed at the institutional and governmental levels to reduce the impact of the pandemic on dental education

ABSTRACT

Resultados: Compuseram a amostra final 271 respondentes.

Aim: To describe the potential consequences of social distancing and the COVID-19 pandemic on the behavior, sleep quality, screen time, and diet of Brazilian chil-dren aged 0 to 12 years. Materials and methods: This descriptive observational study was conducted using a self-administered online questionnaire distributed to parents, caregivers or guardians of children aged 0-12 years. The same questionnaire was distributed to two groups between August 19, 2020 and September 19, 2020. Users of Pelotas Public Health Services made up one group, while the general Brazilian population made up the other. The data was analyzed descriptively considering α=5%. Results: The total sample consisted of 271 respondents. According to caregivers, 59.4% of children showed behavioral changes and 48% reported or demonstrated COVID-19 fear. Moreover, improve-ment in sleep quality was reported for children up to 2 years, and 33.3% of caregivers in both groups reported a decline in sleep quality among children aged 9 to 12. Eighty-five percent of children increased their screen time. The intake of healthy foods remained the same for most of the children, but the intake of ultra-processed food increased for over 40% of them, primarily between 3 to 5 years old and 9 to 12 years old. Discussion: The results should be a focus of attention in order to maintain proper child development following a pandemic. Conclusion: The social distancing caused by the COVID-19 pandemic had a significant impact on the children's routine, with negative effects on their behavior, diet and screen time. In addition, a decline in sleep quality was observed among older children, while better sleep quality was reported for younger children.

ABSTRACT

The present study aimed to estimate the impact of the COVID-19 pandemic on dental care procedures in the Public Health System in Brazil. A retrospective, ecological study was carried out, comprising 5,564 Brazilian municipalities. The number of dental procedures (per 100,000 inhabitants) performed in the Brazilian Public Health System (SUS) was the outcome. The impact of the pandemic was estimated by comparing the rate of procedures for each month of 2020 with the rates observed in the equivalent months in 2019, considering the period from March to August. Regulatory legislation regarding restrictions on dental care, issued by the state governments of 27 Brazilian federative units, was used for mediation analysis. A 55% reduction was observed in March for all procedures (IRR = 0.45, 95%CI 0.39-0.51), while in the other months, the rate of reduction remained ≥ 88%. The biggest decrease was observed in collective preventive procedures (reduction ≥99%) between April and August. Procedures that were less likely to generate aerosols (tooth extraction and prosthetic) and emergencies showed the smallest reduction. The reduction was not mediated by the restrictions imposed by state governments (p>0.05). The impact of the COVID-19 pandemic was noteworthy in terms of the decrease in the number of dental procedures carried out in the Brazilian Public Dental Service. The lack of access, the delay of elective procedures and the lack of preventive collective procedures could result in an overburdened system, post-Pandemic.

ABSTRACT

OBJECTIVE: To assess the participation of women in publications of dental journals with a high impact factor. METHODS: Articles published in 2016, 2011, and 2006 in 10 dental journals chosen for their highest impact factors in each dental fields were included in this research. Articles-related variables collected included the country of origin of each author, the type of study, and gender of the researchers. Gender was examined through PubMed, Scopus, ResearchGate, and respective affiliated institutions. Furthermore, a website designed to discover the gender of names was used when the previous alternatives were not conclusive. Forward stepwise Poisson regression models were used for data analysis. RESULTS: A total of 3365 studies were included in the first authorship analysis and 3398 in analysis related to last authorship. The prevalence of women as first authors was 37.2% (confidence interval (CI) 95% 34.5-37.5) and as last authors was 22.6% (CI 95% 21.3-23.9). Having a woman as the last author increased the presence of women in the first author position in scientific dental articles by 16% (prevalence ratio = 1.16, CI 95% [1.04-1.29]). The year of publication, journal, and region of the author were associated with an increase in the prevalence of women as last authors. From 2006 to 2016, the prevalence of women as last authors increased by 61%. Despite these trends, women were still underrepresented in science in the evaluated period. CONCLUSIONS: There are meaningful gender inequalities in publications of scientific dental papers. Encouraging women to lead research groups can reduce the inequities observed in the present study.

ABSTRACT

Objective: to investigate in the dental literature studies addressing different aspects of LGBTQI+ (Lesbians, Gays, Bisexuals, Transsexuals, Queer Population, Intersex and others) education topics and the approaches used to reduce inequalities and prejudices in dental care. Methods: a search was conducted in the PubMed, Scopus, Web of Science and BVS/Bireme databases, until July 2017. After removing duplicates, two independent reviewers conducted the selection of articles. Results: the initial search resulted in 924 titles. After complete reading, eight studies completed the inclusion criteria. The articles included in this review demonstrated that students, student leaders and administrators recognize support and inclusion measures. Some institutions presented topics related to the LGBTQI+ population in their curriculum, but often related to diseases. This approach was identified as weak and insufficient for a qualified service. Only two studies presented methods for teaching LGBTQI+, but only one assess qualitative results. Conclusions: there is a lack of studies that approach the education of LGBTQI+ topics mainly related to teaching methodologies.(AU)

ABSTRACT

The aim of presented systematic scoping review was to investigate the actual and future clinical possibilities of regenerative therapies and their ability to regenerate bone, periodontal and pulp with histological confirmation of the nature of formed tissue. Electronic search was conducted using a combination between Keywords and MeSH terms in PubMed, Scopus, ISI-Web of Science and Cochrane library databases up to January 2016. Two reviewers conducted independently the papers judgment. Screened studies were read following the predetermined inclusion criteria. The included studies were evaluated in accordance with Arksey and O'Malley's modified framework. From 1349 papers, 168 completed inclusion criteria. Several characterized and uncharacterized cells used in Cell Therapy have provided bone regeneration, demonstrating bone gain in quantity and quality, even as accelerators for bone and periodontal regeneration. Synthetic and natural scaffolds presented good cell maintenance, however polyglycolid-polylactid presented faster resorption and consequently poor bone gain. The Growth Factor-Mediated Therapy was able to regenerate bone and all features of a periodontal tissue in bone defects. Teeth submitted to Revascularization presented an increase of length and width of root canal. However, formed tissues not seem able to deposit dentin, characterizing a repaired tissue. Both PRP and PRF presented benefits when applied in regenerative therapies as natural scaffolds. Therefore, most studies that applied regenerative therapies have provided promising results being possible to regenerate bone and periodontal tissue with histological confirmation. However, pulp regeneration was not reported. These results should be interpreted with caution due to the short follow-up periods.

Dentes submetidos à revascularização apresentaram aumento do comprimento e largura do canal radicular.

ABSTRACT

Objective: The literature has indicated a low level of awareness and training of health professionals in welcoming and serving the LGBT population. The present study aimed to report a clinical case of a trans user and discuss important aspects about the care and reception of transsexuals and transvestites. Case report: The user, with a social name of "A", 25 years old, male biological sex and female gender, was referred from the Basic Health Unit with aesthetic complaints and supra/subgingival calculus. The user arrived at the service already presenting clear signs of discomfort and seclusion. Considering such a state, the user was asked at the beginning of the dialog interview whether she had another name in which she would like to be identified. Then, the patient promptly identified her social name. From that moment, J.F.C.P. became "A" throughout the service, as assured by her rights. After this, the user was already more comfortable and receptive to the conversation in the interview, strengthening the bond with the student. During the interview, the user also expressed doubts about some of her rights as a trans woman, such as the use of the social name on her public health system card. Final considerations: The main role of health professionals in the reception of transsexuals and transvestites is to create a bond free of prejudices. Thus, professionals have a duty to be updated on issues of gender and sexuality to prevent situations and propagations of prejudice, discrimination, and violence.(AU)
