ABSTRACT

Marginalised families faced significant challenges during the COVID-19 pandemic. This study explores inequalities in Japanese mothers' and children's health and well-being and family economic stability before and during the pandemic. Data sources were as follows: nationwide surveys in 2019 and 2021 of families with children using medical institutions across Japan; infant mortality and adolescent suicide rates between 2018 and 2021 from publicly available sources. Outcomes by poor and non-poor families were compared for 2019 and 2021 using simple descriptive statistics. Poor mothers' part-time working increased from 41% to 61% and regular employment was reduced by two thirds. The well-being of poor mothers worsened from 39% to 55%. Employment opportunities and well-being did not change for non-poor mothers. School subsidies among poor families increased from 23% to 55%. The infant mortality rate (IMR) among unemployed families increased significantly from 12.9/1000 to 18.2/1000 between 2018 and 2021 compared with a decreasing overall IMR from 1.9/1000 to 1.7/1000. Suicide rates in 10-19-year-olds increased over the same period although no socio-economic indicators were available. Inequalities in mothers' and children's health and well-being indicators and family economics increased between 2019 to 2021 in Japan. This study cannot attribute causes but suggests a possible role of the pandemic.

ABSTRACT

OBJECTIVE: CD14+ dendritic-shaped cells show a dendritic morphology under the electron microscopy and engage in a pseudoemperipolesis phenomenon with lymphocytes. CD90 has been used as a marker of a major subset of fibroblast-like synoviocytes in rheumatoid arthritis (RA). In this study, we investigated the significance of CD90 expression in CD14+ dendritic-shaped cells and its correlation with RA chronic inflammation. METHODS: Double immunofluorescence staining for CD14 and CD90 was performed in the collected tissues, including 12 active RA synovial tissues. The localization of CD14+ CD90+ cells, the percentages of CD14+ CD90+ cells and vascular areas, the degree of synovitis, and clinical data were investigated. Furthermore, CD14+ CD90+ cells analyzed by flow cytometry (CD14high CD90intermediate (int) cells) were sorted from RA synovial cells, and we examined their potential to differentiate into dendritic cells. RESULTS: Double immunofluorescence staining showed that CD14+ CD90+ cells were abundant in RA synovial tissues. The percentages of CD14+ CD90+ cells and vascular areas correlated with some of the Krenn synovitis scores, but neither showed a strong correlation with RA disease activity parameters. Flow cytometry analysis indicated that CD14high CD90int cells were more abundant in both peripheral blood samples and synovial tissues in patients with active RA. CD14high CD90int cells were more likely to differentiate into dendritic cells in vitro. CONCLUSION: CD14+ dendritic-shaped cells expressed CD90 in the perivascular areas of RA synovial tissues. These findings suggest that CD14+ CD90+ dendritic-shaped cells migrate from the peripheral blood to the synovial tissue, the site of inflammation, and may contribute to the chronic inflammation of RA as dendritic progenitor cells.

ABSTRACT

The histological morphology of oviduct epithelia have been described well, however, the expression pattern of the gap junction proteins in the cells, and the function related with the proteins, such as [Ca2+]i dynamics pattern of living oviduct epithelia at different ages have not been clarified. We used immunohistochemistry to compare the expression pattern of gap junction proteins in the cells of the young and adult groups. Moreover, we used real-time confocal microscopy to observe the spontaneous Ca2+ oscillation (spontaneous fluctuation) in freshly isolated epithelia (ciliated cells) in ampulla potion of oviduct from the two groups. The results show as demonstrated by immunohistochemistry the gap junction proteins (Cx26, Cx32 and Cx43) formed a well-regulated expression in the young animals, but not in the adult animals. In addition, the [Ca2+]i dynamics of ciliated cells in freshly oviduct epithelia have a spontaneous fluctuation pattern that occurs without any stimulation in the young animals, but this pattern was not observed in the adult animals. In conclusions, our findings suggest that gap junctions regulate the spontaneous fluctuation of [Ca2+]i dynamics in ciliated cells of oviduct epithelia in young animals.

ABSTRACT

Our patient was a 57-year-old male with a history of esophageal cancer. He was referred to our hospital for squamous cell lung carcinoma(SCC). Chest computed tomography identified a mass in the left lung field, which was suspected to be invading the reconstructed gastric tube, left subclavian artery, common carotid artery, and distal aortic arch. He was diagnosed as primary pulmonary squamous cell carcinoma(SCC)because six years had already passed since a previous surgery for early esophageal cancer. He received three courses of induction chemotherapy including S-1/CDDP. We evaluated the therapy as a partial response. He underwent an extended resection of distal aortic arch and left subclavian artery with left upper lobectomy, and those vessels were reconstructed using prosthetic grafts. Pathological findings showed the tumor as a well differentiated SCC of pT4N0M0 at stage III A, with a residual tumor on the reconstructed gastric tube, even though the effect of induction chemotherapy was Ef2. He received three courses of S-1/CDDP after surgery. The patient has been well without recurrence for 31 months after surgery.
