ABSTRACT

BACKGROUND: Non-communicable diseases (NCD) like hypertension, diabetes, cardiovascular and cerebrovascular diseases are the most common comorbidities among COVID-19 patients. The clinical presentation and mortality pattern of COVID-19 are different for patients with comorbidities and without comorbidities. OBJECTIVE: To determine the clinical presentation of COVID-19 and risk factors for COVID-19 mortality among diabetic patients in a tertiary care hospital in South India. METHODS: A record-based cross-sectional study was conducted by reviewing the case records of COVID-19 patients admitted for treatment from June 2020 to September 2020 in a tertiary care centre in South India. Potential risk factors for COVID-19 mortality were analysed using univariate binomial logistic regression, generalized linear models (GLM) with the Poisson distribution. Survival curves were made using the Kaplan-Meier method. RESULTS: Out of 200 COVID-19 patients with diabetes with a mean (SD) age of 56.1 (11.8) years, 61% were men. The median survival time was slightly lesser in male COVID-19 patients (15 days) as compared to female patients (16 days). The risk of mortality among COVID-19 patients with diabetes is increased for patients who presented with breathlessness (aRR = 4.5 (95% CI: 2.3-8.8)), had positive history of smoking (aRR = 1.9 (95% CI: 1.1-3.8)), who had CKD (aRR = 1.8 (95% CI: 1.1-2.8)) and who had cardiac illness (aRR = 1.6 (95% CI: 0.9-2.7)). CONCLUSION: Diabetes patients with COVID-19 need to be given additional care and monitoring especially if they present with breathlessness, positive history of smoking, cardiac illness and, CKD. Public health campaigns and health education activities to control smoking is needed to reduce the COVID-19 mortality in diabetes patients.

ABSTRACT

Spinel Co3O4nanospheres have been synthesized by the microwave-assisted hydrothermal method. The N-doped graphene nanosheets (NGN) were synthesized using Hummer's method. The prepared spinel Co3O4and NGN were mixed under certain proportions using an ultrasonication process and treated with microwave radiation to prepare a novel spinel Co3O4nanospheres infused NGN. The synthesized samples were characterized by x-ray diffraction, Raman spectroscopy, Zetasizer, scanning electron microscope/transmission electron microscopy and x-ray photoelectron spectroscopy for identifying crystal structure and phase, particle size, and the morphology of the nanostructure and the elemental configuration, respectively. The prepared spinel Co3O4/NGN were used as anode material and lithium metal as a reference electrode to fabricate half cell using Swagelok cell components. The electrochemical properties were studied and found to exhibit a larger specific capacity of 575 mAh g-1compared to traditional graphite electrodes, after 100 cycles under 0.1 C rate with a coulombic efficiency of ≈100%. The good electrochemical properties ascribe to the distinctive surface morphological nanostructures of nanoporous nanospheres of spinel Co3O4nanospheres and nanosheets of N-doped graphene that reduce the lithium-ion diffusion pathway. The developed anode material would be a potential electrode for lithium ion battery applications.

ABSTRACT

Viral nervous necrosis (VNN) is a serious disease of marine and brackishwater fishes caused by nervous necrosis virus (NNV) resulting in up to 100% mortality in early larval and juvenile stages. Adult fish when infected are asymptomatic and spread the virus vertically to the offspring through milt and eggs. Prevention of vertical transmission of the disease is by using disease free broodstock and vaccinating the brooders. Estimation of antigen content and virus titre is essential to determine antigen/virus concentration in VNN vaccine. A monoclonal antibody based indirect sandwich ELISA was developed to quantify the NNV antigen and to estimate the virus titre by TCID50 coupled ELISA. Mouse hybridoma clones secreting monoclonal antibodies (MAb) against the capsid protein of NNV was developed and characterised. The antibodies reacted specifically with the recombinant capsid protein and purified virus in western blot. Polyclonal antibodies against NNV were used as capture antibodies and MAbs were used as detection antibodies to optimise an indirect sandwich ELISA to detect and quantify capsid protein of NNV. The developed assay had a sensitivity of 390 ng/ml and could detect the virus in clinical samples. The assay coupled with TCID50 could be used to estimate the virus titre rather than by observing the CPE which is laborious and subjective.

ABSTRACT

COVID-19 is an emerging viral disease affecting more than 200 countries worldwide and it present with varied clinical profile throughout the world. Without effective drugs to cure COVID-19, early identification and control of risk factors are important measures to combat COVID-19.  This study was conducted to determine the clinical profile and risk factors associated with mortality among COVID-19 patients in a tertiary care hospital in South India. This record-based longitudinal study was conducted by reviewing the case records of COVID-19 patients admitted for treatment from June 2020 to September 2020 in a tertiary care centre in South India. The clinical details, discharge/death details, were collected and entered in MS Excel. Potential risk factors for COVID-19 mortality were analysed using univariate binomial logistic regression, generalized linear models (GLM) with Poisson distribution. Survival curves were made using the Kaplan-Meier method. Log-rank test was used to test the equality of survivor functions between the groups. Out of 854 COVID-19 patients, 56.6% were men and the mean (standard deviation) age was 45.3(17.2) years. The median survival time was significantly lesser in male COVID-19 patients (16 days) as compared to female patients (20 days). Increasing age, male gender, patients presenting with symptoms of fever, cough, breathlessness, smoking, alcohol consumption, comorbidities were significantly associated with mortality among COVID-19 patients. Patients with older age, male gender, breathlessness, fever, cough, smoking and alcohol and comorbidities need careful observation and early intervention.  Public health campaigns aimed at reducing the prevalence of risk factors like diabetes, hypertension, smoking and alcohol use are also needed.

ABSTRACT

Viral nervous necrosis (VNN) is a serious viral disease of several species of farmed and wild fishes. Adult fish are asymptomatic and become carriers of the virus when infected with nervous necrosis virus (NNV) and they transmit the virus to the offspring through eggs. ELISA is ideal for non-lethal screening of adult fish for VNN. Asian seabass (Lates calcarifer) IgM was purified using Protein A affinity column and hybridoma clones secreting monoclonal antibodies (MAb) specific to the heavy chain of IgM was developed. An Indirect ELISA using anti-seabass IgM MAb was developed by optimizing all the reagents. The assay was used to screen adult Asian seabass from grow-out farms in comparison to RT-PCR. The assay was also used to assess the immune response in Asian seabass immunized with inactivated Red-spotted grouper NNV (RGNNV). Seabass IgM on SDS-PAGE analysis revealed three heavy chain bands of size 96, 82 and 76 kDa and a single light chain of size 25 kDa. Out of 18 positive hybridoma clones, two selected clones reacted specifically with the 76 kDa heavy chain band. Out of 28 serum samples of Asian seabass from grow-out farms 26 were positive for NNV antibodies while 22 were positive by RT-PCR. Fish immunized with inactivated RGNNV showed immune response by one week post-immunization, and the peak immune response was observed four weeks post-immunization. The assay developed can be used for non-lethal screening of adult Asian seabass for VNN and to assess the immune response after vaccination.

BACKGROUND: Triple-negative breast cancer (TNBC) is one of the most invasive subtypes of breast cancer, with high rates of visceral metastases and recurrence. Choroid plexus metastasis from breast cancer is infrequent despite a high incidence of brain parenchymal metastasis. METHODS: We report a case of solitary metastasis to the choroid plexus from a TNBC that masqueraded as central neurocytoma, and we review the PubMed database for similar cases focusing on their diagnostic challenges and management strategies. RESULTS: A 28-year-old woman with a history of TNBC presented with recurrent seizures, headache, and vomiting. Imaging studies depicted a well-defined lesion in the right anterior lateral ventricle that was attached to the septum pellucidum. After an initial radiological diagnosis of central neurocytoma, she deteriorated rapidly with intraventricular hemorrhage requiring emergency transcallosal microsurgical tumor decompression. Histopathological examination and immunohistochemistry confirmed breast carcinoma as the origin of the intraventricular mass. A review of the PubMed database identified only 2 case reports of choroid plexus metastases from breast cancer reported thus far. CONCLUSIONS: Choroid plexus metastases are exceedingly infrequent and can be mistaken for the more common central neurocytoma. The intraventricular milieu is inhospitable suggesting some extracranial carcinomas develop traits that help them to thrive in the acellular cerebrospinal fluid. Intraventricular mass lesions with a history of primary neoplasm should raise suspicion for choroid plexus metastases. A high index of suspicion despite excellent control of the primary tumor and the absence of systemic metastases is indispensable.

ABSTRACT

Aging brain is characterized by a change in biomolecular composition leading to a diverse range of neurological diseases. Anti-aging research is of current interest, to lessen the burden of age-related macromolecular damage through antioxidant supplementation and caloric restriction. However, data concerning the effect of these anti-aging regimens on age-related biomolecular changes in rat brain is still lacking. In the present study, for the first time, we employed Fourier transform infrared (FTIR) spectroscopy, to investigate the effect of quercetin, caloric restriction (CR) and combination of both on alterations in the composition of lipids and proteins of aged rat brain cerebral cortex. Aged male Wistar rats (21 months old) were divided into four groups: Control (CONT), fed pellet diet; Quercetin (QUER), fed quercetin (50 mg/kg/day); CR (caloric restriction) (fed 40% reduced CONT), and CRQ (40% CR and 50 mg/kg/day QUER). Three-month-old rats served as young control (YOUNG). Our short-term study (45 days) shows decreased band area of unsaturated lipids, decreased area ratios of olefinic/lipid and CH2 antisymmetric stretching (2925 cm-1)/lipids in CONT group compared to young rats, suggesting age-associated lipid peroxidation in aged rats. A slight decrease in the frequency of CH2 antisymmetric mode of lipids (whereas no change in CH2 symmetric mode), but a decrease in bandwidths of both CH2 antisymmetric and symmetric modes of lipids was observed for CONT group compared to YOUNG. Further, a significant decrease in the peak area of infrared bands of proteins and an increase in the peak area of the CO band of lipids was observed in the CONT group. Our data also show that lower levels of α-helical structures and higher levels of random coils, representing altered protein secondary structure composition in the CONT group compared to YOUNG group. Reduction in neuronal cell density and shrinked nucleus was also observed in aged rats. Increase in the accumulation of oxidative mediated damage to macromolecules and diminished antioxidant levels, could be the possible reason for the age-related alterations in the composition of lipids and proteins. However, the combination of quercetin and CR, but not either treatment alone, significantly prevented the age associated alterations in the lipid and protein profiles in the rat cerebral cortex. Further, our results help to understand the mechanism of action of antioxidants under non-restriction and CR conditions, this might help in the development of novel anti-aging treatments to ameliorate oxidative stress in age-related disorders.

Background: Patient safety cannot be considered in isolation when organizational factors, both active and latent, influence patient outcomes. Methods: We did a cross-sectional mixed methods study using a convergent parallel design at a tertiary care public sector hospital in Hyderabad, Telangana (i) to qualitatively investigate the nature and determinants of patient safety incidents occurring in the hospital; (ii) to quantify the perception of hospital staff regarding factors affecting patient safety from an organizational perspective; and (iii) to triangulate the results to highlight areas in need of improvement. Results: The most common factors affecting patient safety were situational factors, working conditions and latent organizational factors including communication systems. Despite the relatively poor knowledge of paramedical staff regarding patient safety incidents, they perceived innovation and flexibility, outward focus, reflexivity, quality, pressure to produce, performance feedback and effort to be significantly higher compared to the heads of departments and clinical faculty. The strength of the dimensions: integration, involvement, training, welfare, supervisory support in the hospital was weak as perceived by all categories of staff. Conclusion: There is a need to build team work, improve trust and communication between various departments, invest more in training, and provide supervisory support along with structural and process improvements in issues such as drug procurement and developing patient-friendly physical environment.

ABSTRACT

Pristine α-MnMoO4 nanorods were facilely prepared via co-precipitation (Cp) and microwave hydrothermal (MH) methods. X-ray diffraction (XRD) patterns showed pure monoclinic crystalline phase α-MnMoO4 for the heat treated powder at 500 °C. Fourier Transform Infrared (FTIR) spectra showed that the chemical bond structure of α-MnMoO4 corresponds to the strong vibrational modes of Mo-O-Mo, Mo-O and Mo[double bond, length as m-dash]O. Raman spectra showed the structural bonding and crystalline nature of α-MnMoO4. Field Emission Scanning Electron Microscope (FE-SEM) images exposed the nanorod shape of the α-MnMoO4 powder, with diameters of ∼200 nm and lengths of ∼1.6 µm. Electrochemical studies of the Cp- and MH-MnMoO4 nanorods with 2 M NaOH as the electrolyte showed specific capacitances of 143 F g-1 and 551 F g-1, respectively, at a 1 A g-1 constant discharge current density. Cyclic voltammetric (CV) studies of the MH-MnMoO4 nanorods at various scan rates revealed the presence of redox pairs, suggesting a pseudocapacitive nature. The structural stability at different current densities demonstrated the high rate performances and good reversible capacity retention of the calcined MH-MnMoO4 nanorods. A cycling life stability study of MH-MnMoO4 demonstrated a good capacity retention of 89% of the initial specific capacitance at 5 A g-1 after 1000 cycles.

A large amount of construction and demolition waste is being generated owing to rapid urbanisation in Indian cities. A reliable estimate of construction and demolition waste generation is essential to create awareness about this stream of solid waste among the government bodies in India. However, the required data to estimate construction and demolition waste generation in India are unavailable or not explicitly documented. This study proposed an approach to estimate construction and demolition waste generation using waste generation rates and demonstrated it by estimating construction and demolition waste generation in Chennai city. The demolition waste generation rates of primary materials were determined through regression analysis using waste generation data from 45 case studies. Materials, such as wood, electrical wires, doors, windows and reinforcement steel, were found to be salvaged and sold on the secondary market. Concrete and masonry debris were dumped in either landfills or unauthorised places. The total quantity of construction and demolition debris generated in Chennai city in 2013 was estimated to be 1.14 million tonnes. The proportion of masonry debris was found to be 76% of the total quantity of demolition debris. Construction and demolition debris forms about 36% of the total solid waste generated in Chennai city. A gross underestimation of construction and demolition waste generation in some earlier studies in India has also been shown. The methodology proposed could be utilised by government bodies, policymakers and researchers to generate reliable estimates of construction and demolition waste in other developing countries facing similar challenges of limited data availability.

ABSTRACT

Novel hybrid (organic/inorganic) electrospun nanocomposite polymer blend electrolyte fibrous membranes with the composition poly(vinylidene difluoride-co-hexafluoropropylene) [P(VdF-co-HFP)]/poly(methyl methacrylate) [P(MMA)]/magnesium aluminate (MgAl2O4)/LiPF6 were prepared by the electrospinning technique. All of the prepared electrospun P(VdF-co-HFP), PMMA blend [90% P(VdF-co-HFP)/10% PMMA], and nanocomposite polymer blend [90% P(VdF-co-HFP)/10% PMMA/x wt % MgAl2O4 (x = 2, 4, 6, and 8)] fibrous membranes were characterized by X-ray diffraction, Fourier transform infrared spectroscopy, differential scanning calorimetry, and scanning electron microscopy. The fibrous nanocomposite separator-cum-polymer blend electrolyte membranes were obtained by soaking the nanocomposite polymer blend membranes in an electrolyte solution containing 1 M LiPF6 in ethylene carbonate (EC)/diethyl carbonate (DEC) (1:1, v/v). The newly developed fibrous nanocomposite polymer blend electrolyte [90% P(VdF-co-HFP)/10% PMMA/6 wt % MgAl2O4/LiPF6] membrane showed a low crystallinity, low average fiber diameter, high thermal stability, high electrolyte uptake, high conductivity (2.60 × 10(-3) S cm(-1)) at room temperature, and good potential stability above 4.5 V. The best properties of the fibrous nanocomposite polymer blend electrolyte (NCPBE) membrane with a 6 wt % MgAl2O4 filler content was used for the fabrication of a Li/NCPBE/LiCoO2 CR 2032 coin cell. The electrochemical performance of the fabricated CR 2032 cell was evaluated at a current density of 0.1 C-rate. The fabricated CR 2032 cell lithium battery using the newly developed NCPBE membrane delivered an initial discharge capacity of 166 mAh g(-1) and a stable cycle performance.

ABSTRACT

Generation of flash X-rays (FXRs) at less than 500 keV is described with emphasis on experimental investigation. The pulser is a Tesla transformer-Water transmission line based pulsed power generator operating in double resonance mode to power a rod-pinch diode. The configuration of aspect ratio reported here falls much below the normally reported ratios for the rod-pinch diode operation. Experimental investigation at such low pulsed voltage has revealed "flowering" of the anode tip and "pitting" of the perspex window. A possible explanation in terms of Lorentz body force is discussed rather than the pinch mechanism generally suggested in literature. The experimental investigation for the FXR generation is corroborated by measuring the radiation dose using CaSO4 (Dy) thermo luminescent dosimeters.

ABSTRACT

Electrospinning is the most versatile technology in use today, for the generation of polymer nanoscale fibers. The nano materials generated using this technology have a large surface area and are highly porous making it very useful in many applications in diverse fields such as energy storage, healthcare, biotechnology, environmental engineering, defense and security. The production of the fibers and the morphology can be easily controlled by modifications to the processing parameters. The relatively high production rate and simplicity of the setup makes electrospinning highly attractive. This review summarizes the effect of various processing parameters on the effective generation of nanofibers. By simple modifications to the electric field inside the electrospinning chamber the fiber collection can be easily controlled. In addition, the various applications of electrospun fibers in electronic devices, environmental sensors and filters, energy storage, and in biomedicine such as in tissue engineering, drug delivery and enzyme encapsulation are examined and the current research in each field is also explored in this review.

Study on chemical characteristics of groundwater and impacts of groundwater quality on human health, plant growth, and industrial sector is essential to control and improve the water quality in every part of the country. The area of the Varaha River Basin is chosen for the present study, where the Precambrian Eastern Ghats underlain the Recent sediments. Groundwater quality is of mostly brackish and very hard, caused by the sources of geogenic, anthropogenic, and marine origin. The resulting groundwater is characterized by Na(+) > Mg(2+) > Ca(2+) : [Formula: see text] > Cl(-) > [Formula: see text], Na(+) > Mg(2+) > Ca(2+) : [Formula: see text] > Cl(-) > [Formula: see text] > [Formula: see text], Na(+) > Mg(2+) > Ca(2+) : [Formula: see text] > Cl(-), and Na(+) > Mg(2+) > Ca(2+) : Cl(-) > [Formula: see text] > [Formula: see text] facies, following the topographical and water flow-path conditions. The genetic geochemical evolution of groundwater ([Formula: see text] and Cl(-)-[Formula: see text] types under major group of [Formula: see text]) and the hydrogeochemical signatures (Na(+)/Cl(-), >1 and [Formula: see text]/Cl(-), <1) indicate that the groundwater is of originally fresh quality, but is subsequently modified to brackish by the influences of anthropogenic and marine sources, which also supported by the statistical analysis. The concentrations of total dissolved solids (TDS), TH, Mg(2+), Na(+), K(+), [Formula: see text], Cl(-), [Formula: see text], and F(-) are above the recommended limits prescribed for drinking water in many locations. The quality of groundwater is of mostly moderate in comparison with the salinity hazard versus sodium hazard, the total salt concentration versus percent sodium, the residual sodium carbonate, and the magnesium hazard, but is of mostly suitable with respect to the permeability index for irrigation. The higher concentrations of TDS, TH, [Formula: see text], Cl(-), and [Formula: see text] in the groundwater cause the undesirable effects of incrustation and corrosion in many locations. Appropriate management measures are, therefore, suggested to improve the groundwater quality.

ABSTRACT

Mechanisms governing the tribological behavior of polymer-on-polymer sliding were investigated by molecular dynamics simulations. Three main mechanisms governing frictional behavior were identified. Interfacial "brushing" of molecular chain ends over one another was observed as the key contribution to frictional forces. With an increase of the sliding speed, fluctuations in frictional forces reduced in both magnitude and periodicity, leading to dynamic frictional behavior. While "brushing" remained prevalent, two additional irreversible mechanisms, "combing" and "chain scission", of molecular chains were observed when the interfaces were significantly diffused.

ABSTRACT

Myocardial infarction (MI) is uncommon in patients undergoing noncardiac surgery without a history of coronary artery disease. But, patients with compromised pulmonary function and coexisting anaemia superimposed by precipitating factors like prolonged hypotension and tachycardia can culminate in myocardial catastrophe even in the absence of risk factors. We are herewith reporting an unusual case of postoperative non-ST elevation MI without any pre-existing ischemic heart disease. A 39-year-old female patient who was submitted for diagnostic video-assisted thoracoscopy and chemical pleurodesis for recurrent pneumothorax developed postoperative MI. After review of all the factors, it was found that the patient developed Type 2 MI as a sequel to oxygen supply and demand mismatch secondary to hypoxia and prolonged hypotension. This was evident in the 12-lead electrocardiogram and was confirmed by elevated cardiac biomarkers and regional wall motion abnormality on echocardiography.

ABSTRACT

Congenital anomalies of the urinary tract system are common. The ectopic pelvic kidney is a rare anomaly about 1:2500 live births, left side being more common. Here we are reporting a case of left ectopic kidney with non rotation and morphologically normal right kidney. The case was found during routine dissection in the department of anatomy in an adult male cadaver. There were no other anomalies observed.

ABSTRACT

An extensive morphometric study of human trachea and principal bronchi was undertaken in the Department of Anatomy, Institute of Post Graduate Medical Education and Research, Kolkata, West Bengal, India, on 87 specimens procured from 51 male and 36 female relatively disease free fresh cadavers from Kolkata Police Morgue. The specimens were grouped into five age groups for both sexes. The length of trachea, principal bronchi, subcarinal angle was measured in each specimen. The external transverse diameter (width), internal transverse diameter and anteroposterior diameter (depth) were recorded at two different levels of trachea (junction of upper third and middle third; middle third and lower third) and middle of principal bronchi. Then the width-depth ratio was calculated. For each component of the measurements, standard error (SE), standard deviation (SD) and test of significance were calculated using independent sample't' test and multiple comparison test. The present investigation revealed a wide variation in different dimensions viz. length, transverse and anteroposterior diameters and width-depth ratio of upper and lower trachea and right and left principal bronchi and the subcarinal angle, in a same age group as well as in different age groups in both sexes. The study of these morphometric variations is of profound clinical importance as it may help the clinicians to understand the etiology of several pulmonary diseases and the surgeons to deal with resection and reconstruction of the tracheobronchial tree. This knowledge is also helpful for smooth conduction of some maneuvers like endotracheal intubation and bronchoscopic procedures.

ABSTRACT

Ovarian neoplasms have become increasingly important not only because of the large variety of neoplastic entities but more because they have gradually increased the mortality rate due to female genital cancers. A total of 120 cases of ovarian tumors were studied at the Department of Pathology, Konaseema Institute of Medical Sciences, Amalapuram, India, during the period of March 2005 to March 2010, to find out frequency of different histological patterns of ovarian tumors at Konaseema Region. Among 120 cases, majority 86 (71.6%) were benign, but alarming number 30 (25.0%) were malignant, remaining 4 cases were borderline. The commonest histological pattern observed in the study was epithelial tumors (61.6%). The commonest benign tumor was serous cyst adenoma, while; the commonest malignant tumors were granulosa cell tumor and endometrial carcinoma. Epithelial tumors were commonest variety of ovarian tumors followed by germ cell tumors. A relatively high number of malignancies were observed in this study.

ABSTRACT

A wide range of morphological varieties and developmental anomalies of the thyroid gland have been reported in literature such as hypoplasia, ectopy, hemi agenesis, and agenesis. Out of these the incidence of agenesis of the isthmus of thyroid gland is rare and very few cases have been reported. In the present case report a male cadaver was found with agenesis of isthmus of thyroid gland with pyramidal lobe and levator glandulae thyroidae arising from right lobe.
