ABSTRACT

This case report discusses a unique scenario in which a 19-year-old patient with a penetrating wound in the common femoral vein developed deep vein thrombosis in response to life-threatening bleeding. The report highlights our thoughts on managing an isolated truncal venous injury leading to deep vein thrombosis, emphasizing the significance of surgical exploration in vascular trauma and the feasibility of employing non-invasive imaging diagnosis in preoperative planning.

ABSTRACT

PURPOSE: To evaluate the association between increased intima-media thickness (IMT) and atherosclerotic carotid plaque (CP) with idiopathic overactive bladder syndrome (OAB) in women with metabolic syndrome (MetS). METHODS: This is a cross-sectional study, which included consecutive women aged 40-75 years with MetS, seeking medical assistance at a reference center between April and December 2016. OAB-V8 questionnaire was used to estimate the prevalence of OAB symptoms, which were defined by a score ≥8 points. All patients underwent bilateral carotid artery ultrasound to assess IMT and CP. Atherosclerosis was defined by the identification of CP during ultrasound, which was diagnosed according to the Mannheim carotid intima-media thickness and plaque consensus. RESULTS: Forty-five women were prospectively included. Mean age was 60±9.3 years (range 40-75 ys). Eighteen (40%) patients were diagnosed with OAB. IMT in the general population was 0.72 mm (SD = 0.20). Overall prevalence of atherosclerosis, defined by the presence of the carotid artery plaque, was 51%. OAB prevalence among women with atherosclerosis was higher than in those without atherosclerosis (56.52% versus 22.73%), with a prevalence ratio of 2.49 (p=0.04). Additionally, OAB was associated with degree of carotid stenosis (p = 0.029). CONCLUSION: In this cohort of female patients with MetS, there was an association between carotid atherosclerosis and OAB. Identification of carotid ultrasound abnormalities may lead to refined treatment decision-making among OAB patients.

ABSTRACT

Com o uso crescente do dispositivo Amplatzer® para diversos procedimentos endovasculares, dentre os quais a comunicação interatrial, complicações decorrentes de seu uso vêm sendo descritas. Relatamos um caso em que o dispositivo foi empregado para correção de comunicação interatrial e, seis meses depois, migrou para a bifurcação da aorta abdominal. A retirada do corpo estranho foi realizada por cirurgia convencional, após insucesso de tentativa por via endovascular.


Complications arising from use of the Amplatzer© device to correct endovascular conditions such as atrial septal defect have been described with increasingly frequency. We report on a case in which this device was used to correct an atrial septal defect, but 6 months later migrated to the abdominal aorta bifurcation. Removal of the foreign body was accomplished by conventional surgery after an endovascular attempt had failed.

ABSTRACT

Correct identification and early management of hypertensive disorders should be a part of the therapeutic repertoire of every professional working in hemodynamics units. Based on recent publications, this study aims to propose a practical approach to the identification and early management of these disorders in this type of service.


O reconhecimento correto e manejo inicial das síndromes hipertensivas devem fazer parte do arsenal terapêutico de todo profissional que trabalhe em unidade de hemodinâmica. Este trabalho tem por objetivo, baseado em publicações recentes, propor uma abordagem prática para identificação e manejo inicial destes agravos nessa unidade.

ABSTRACT

O rim em ferradura é uma das anomalias urológicas congênitas mais comuns e está presente em cerca de 0,12% dos pacientes com aneurisma de aorta abdominal. O reparo cirúrgico convencional está associado a dificuldades técnicas que provavelmente aumentam a morbidade e a mortalidade, mas que podem ser evitadas com o tratamento endovascular. Relatamos um caso de um paciente de 64 anos com rim em ferradura e aneurisma de aorta abdominal, que foi submetido ao reparo endovascular do aneurisma com sucesso.


The horseshoe kidney is one of the most common urologic anomalies and is present in about 0,12% of the patients with abdominal aortic aneurysm. The conventional surgical repair is associated with technical difficulties that probably increase morbidity and mortality, but can be averted by the endovascular treatment. We report a case of a 64-year-old patient with horseshoe kidney and abdominal aortic aneurysm, who underwent endovascular repair with success.

ABSTRACT

Descrevemos um caso de oclusão arterial aguda por projétil de arma de fogo. Devido à raridade do caso (23 relatos em 20 anos) e a importância do diagnóstico diferencial precoce, uma revisão bibliográfica foi realizada. Foram abordados aspectos do quadro clínico, diagnóstico diferencial e tratamento. Neste caso, o diagnóstico por meio de ultrassom vascular da complicação foi publicado pela primeira vez.


We describe a case of acute arterial occlusion caused by a firearm bullet. Since the case is rare (23 cases in 20 years), and due to the importance of early diagnosis, a bibliographic review was performed. Aspects of the clinical picture, differential diagnosis and treatment were approached. In this case, diagnosis by a vascular ultrasound was published for the first time.
