ABSTRACT

BACKGROUND: Between February and April 2016, a slight increase in mortality was observed in a colony consisting of 400 captive Seba's short-tailed bats (Carollia perspicillata). These animals cohabited with other nocturnal animal species in a dome of a private zoo in Switzerland. RESULTS: Gross and histological analysis of two (14.3%) out of the 13 animals submitted for necropsy within this period revealed a necrosuppurative pneumonia, hepatitis, splenitis, enterocolitis, and endometritis, with abundant intralesional colonies of Gram-negative rods. Yersinia (Y.) pseudotuberculosis serotype O:1 and biotype 1 belonging to the sequence type ST90 was isolated from the affected organs in both animals. Following this diagnosis, » of the colony (99 animals) was culled and submitted for gross and histopathological analysis, and a bacterial culture selective for Yersinia spp. of lung, liver, and spleen was performed. From these 99 animals, one gravid female was tested and found to be positive for Y. pseudotuberculosis in the absence of clinical symptoms and histopathological lesions. PCR analysis of altogether three bacterial isolates for virulence factors revealed the presence of the ail gene, and one isolate was also positive for the virF and yadA plasmid genes. CONCLUSIONS: These findings suggest that Carollia perspicillata are susceptible to lethal yersiniosis but do not represent a regular reservoir for Y. pseudotuberculosis. Culling of » of the population was sufficient to limit the spread of this infection among the colony. Moreover, no infections were detected in cohabitant nocturnal animals and caretakers, indicating that the zoonotic risk in this case was low.

ABSTRACT

INTRODUCTION: The following case report describes the clinical and diagnostic procedure for suspected brucellosis infection in a dog. A 21 month old intact male Border Collie was presented with an enlarged right testicle and epididymis. The dog was imported to Switzerland from Germany at the age of three months, but was never abroad since then. Clinical and laboratory diagnostic investigation included bacteriology and histology. An initial serological evaluation by means of rapid slide agglutination test (RSAT) was negative. Repeated examination of the same serum by a chromatographic immunoassay (ICT) revealed a positive result. Brucella canis infection was confirmed by culture. The present case is intended to underline the importance of the suspected diagnosis of 'brucellosis' in the presence of reproductive tract problems in dogs. In addition, Brucella canis has zoonotic potential and it is imperative to comply with strict hygiene management.


INTRODUCTION: Le rapport de cas suivant décrit la procédure clinique et diagnostique en cas de suspicion d'infection par la brucellose chez un chien. Un Border Collie mâle intact de 21 mois a été présenté avec un grossissement du testicule et de l'épididyme droits. Le chien avait été importé d'Allemagne en Suisse à l'âge de trois mois, mais n'avait si non jamais été à l'étranger depuis lors. Des examens diagnostiques cliniques et de laboratoire, notamment bactériologie et histologie ont été effectués. Une première évaluation sérologique au moyen du test d'agglutination rapide sur lame (RSAT) était négative. Un examen ultérieur du même sérum par une immunoanalyse chromatographique (ICT) a révélé un résultat positif. L'infection à Brucella canis a été confirmée par culture. Le présent cas souligne l'importance du diagnostic présumé de «brucellose¼ en présence de problèmes de l>appareil reproducteur chez le chien. De plus, Brucella canis a un potentiel zoonotique et il est impératif d'appliquer des mesures d'hygiène strictes.

ABSTRACT

BACKGROUND: Data about paclitaxel-eluting balloon (PCB) angioplasty to treat drug-eluting stents (DES) in-stent restenosis (ISR) were mainly collected in selected patient populations in the setting of randomized trials. The main goal of this prospective registry was to confirm the positive findings of these studies in an unselected population in clinical practice. METHODS: Consecutive patients with DES-ISR treated by PCB angioplasty were recruited in this prospective real-world registry. The primary endpoint was clinically driven target-lesion revascularization (TLR) at 9 months. Secondary endpoints included acute technical success, in-hospital outcomes, 9-month major adverse cardiac events (MACE) a composite of death, myocardial infarction (MI) and TLR and the occurrence of target vessel revascularization. RESULTS: A total of 206 patients (67.7 ± 10.2 years, 80.6% male, 41.3% diabetics) with 210 lesions were recruited. Unstable coronary artery disease was present in 55.3% of patients. The time from DES implantation to DES-ISR was 3.0 ± 2.4 years. Quantitative analyses revealed that patterns of treated DES-ISR were focal in 55.7% and diffuse in 44.3%. The reference diameter was 2.76 ± 0.64 mm. The 9-month follow-up rate was 90.8% (187/206). At 9 months, the TLR rate was 7.0% (13/187) whereas the rates for MACE, MI and cardiac death were 10.7% (20/187), 4.8% (9/187) and 2.1% (4/187) respectively. Results were consistent in patients with paclitaxel and non-paclitaxel-eluting stents (PES) ISR. CONCLUSION: This large prospective registry demonstrated acceptable rates of TLR and MACE at 9 months after treatment of DES-ISR by PCB angioplasty. PCB angioplasty was equally effective in patients with PES-ISR and non PES-ISR.

ABSTRACT

Yersinia enterocolitica 4/O:3 is the most important human pathogenic bioserotype in Europe and the predominant pathogenic bioserotype in slaughter pigs. Although many studies on the virulence of Y. enterocolitica strains have showed a broad spectrum of detectable factors in pigs and humans, an analysis based on a strict comparative approach and serving to verify the virulence capability of porcine Y. enterocolitica as a source for human yersiniosis is lacking. Therefore, in the present study, strains of biotype (BT) 4 isolated from Swiss slaughter pig tonsils and feces and isolates from human clinical cases were compared in terms of their spectrum of virulence-associated genes (yadA, virF, ail, inv, rovA, ymoA, ystA, ystB and myfA). An analysis of the associated antimicrobial susceptibility pattern completed the characterization. All analyzed BT 4 strains showed a nearly similar pattern, comprising the known fundamental virulence-associated genes yadA, virF, ail, inv, rovA, ymoA, ystA and myfA. Only ystB was not detectable among all analyzed isolates. Importantly, neither the source of the isolates (porcine tonsils and feces, humans) nor the serotype (ST) had any influence on the gene pattern. From these findings, it can be concluded that the presence of the full complement of virulence genes necessary for human infection is common among porcine BT 4 strains. Swiss porcine BT 4 strains not only showed antimicrobial susceptibility to chloramphenicol, cefotaxime, ceftazidime, ciprofloxacin, colistin, florfenicol, gentamicin, kanamycin, nalidixic acid, sulfamethoxazole, streptomycin, tetracycline and trimethoprim but also showed 100% antibiotic resistance to ampicillin. The human BT 4 strains revealed comparable results. However, in addition to 100% antibiotic resistance to ampicillin, 2 strains were resistant to chloramphenicol and nalidixic acid. Additionally, 1 of these strains was resistant to sulfamethoxazole. The results demonstrated that Y. enterocolitica BT 4 isolates from porcine tonsils, as well as from feces, show the same virulence-associated gene pattern and antibiotic resistance properties as human isolates from clinical cases, consistent with the etiological role of porcine BT 4 in human yersiniosis. Thus, cross-contamination of carcasses and organs at slaughter with porcine Y. enterocolitica BT 4 strains, either from tonsils or feces, must be prevented to reduce human yersiniosis.

ABSTRACT

OBJECTIVE: To compare the effectiveness of 100% oxygen therapy vs oxygen treatment with targeted pulse oximetry in the management of symptomatic small to moderate spontaneous pneumothorax (SP). In total, 100% oxygen treatment for SP has been a common practice in neonatology, albeit there is little evidence to validate its efficacy. STUDY DESIGN: A retrospective chart review of 83 neonatal records with the diagnosis of pneumothorax was conducted. Infants <35 weeks gestation, those with large pneumothoraces requiring chest tube drainage and/or ventilatory support were excluded. Data gathered included demographics, vital signs, treatment information and clinical indicators of resolution of symptoms. RESULT: In total, 45 neonates with SP were included in the study. Groups were similar for gestational age, birth weight, Apgar scores, gravidity, parity, gender, race, pneumothorax size and location. Patients in the 100% oxygen therapy group received a significantly longer oxygen treatment (21.3 vs 8 h, P < 0.001), required longer intravenous fluid treatment (48.6 ± 29.9 vs 31.3 ± 18.8 h, P = 0.03) and were delayed in reaching full feeds (44.1 ± 25.7 vs 29.5 ± 18.8 h, P = 0.03) compared with the oxygen-targeted treatment group. Time to first oral feeding, time to resolution of tachypnea and length of stay were similar in both groups. CONCLUSION: There are no clinically significant advantages to using 100% oxygen in the treatment of symptomatic small to moderate SP. In fact, it may result in longer exposure to unnecessary oxygen treatment and toxicity. Oxygen should be reserved for those who are hypoxic and adjusted to comply with accepted saturation levels in neonates.

ABSTRACT

Lepidopteran insects are major pests of agricultural crops, and mated female moths exploit plant volatiles to locate suitable hosts for oviposition. We investigated the heritability of odor-guided host location behavior and fecundity in the cosmopolitan oriental fruit moth Grapholita (Cydia) molesta, an oligophagous herbivore that attacks fruit trees. We used a full-sib/half-sib approach to estimate the heritability and the genetic correlation between these two traits. Results document a considerable genetic basis for olfactory attraction of females (h ( 2 ) = 0.37 ± 0.17) and their fecundity (h ( 2 ) = 0.32 ± 0.13), as well as a genetic trade-off between female attraction and fecundity (r ( g ) = -0.85 ± 0.21). These estimations were empirically corroborated by comparing two strains maintained in the laboratory for different numbers of generations. The long-term reared strain lost its olfactory discrimination ability but achieved significantly higher fecundity compared with the short-term reared strain. Our results highlight that genetic studies are relevant for understanding the evolution of odor-guided behavior in herbivore insects and for judging the promise of pest management strategies involving behavioral manipulation with plant volatiles.

The aim of this study was to assess the value of analysis of pulmonary venous flow in the evaluation of the haemodynamic status of patients with chronic renal failure with normal left ventricular function, treated by haemodialysis. Pulmonary venous flow was recorded immediately before and after haemodialysis in 27 patients with chronic renal failure and a mean age of 44 years. Three groups of patients were defined according to the change in mitral E/A ratio: Group I (E/A < 1 before and after dialysis), Group II (E/A > 1 before and < 1 after dialysis) and Group III (E/A > 1 before and after dialysis). There was a significant difference between these subgroups before dialysis with respect to age, S, D, VTI S, Total VTI, VTI S/Total (p < 0.05). However, because the values overlapped, only a VTI S/Total ratio greater than 59% differentiated patients in Group II from those in group III (p < 0.05). After dialysis, the change in S/D and VTI S/Total ratios increased in Groups I and II and decreased in Group III. The authors concluded that 63% of patients without LV dysfunction on haemodialysis have abnormalities of relaxation which are latent in 47% of cases due to increased filling pressures diagnosed by a VTI S/Total ratio > 59% or simply because the patients are over 50 year old.

ABSTRACT

Supraventricular arrhythmias are frequently observed in pneumonectomy surgery. We retrospectively studied a series of 100 consecutive patients undergoing pneumonectomy for cancer between 1994 and 1996. We found 24% of significant supraventricular arrhythmias, corresponding to atrial fibrillation in 75% of cases, occurring in 80% of cases until the third postoperative day. The only risk factor significantly associated with these arrhythmias was the patient's age. These arrhythmias are easily reduced, spontaneously in 25% of cases, and usually by amiodarone, alone or associated with digitalis alkaloids. While the mortality of the overall group was 12%, 8% of patients with arrythmia died. These deaths concerned patients whose arrythmias occurred after the fourth postoperative day in a context of a pulmonary infection.

Average daily gain between birth and 30 d of age of 42,644 lambs of Swiss Black-Brown Mountain Sheep were used in this analysis. The influence of amount of pedigree information on computing time and on REML estimates of population parameters was investigated on a subset of 7,848 lambs. If all available pedigree information was used, 89.4% of the lambs had at least four complete generations of known ancestors. For the reduced pedigree information, only parents and grandparents of a lamb were included. In the data set with complete pedigree information, 2,616 additional animals (without records) caused 19.4% more equations, 4.6 times the number of non-zero elements in the system of equations, and 21.2 times the computing time to reach convergence. The difference in amount of pedigree information had only a marginal influence on the estimates of direct heritability (h2), maternal heritability (m2), permanent environmental effects, and on the genetic correlation between direct and maternal effect (rAM). The complete data set of 42,644 recorded lambs was randomly split into four subsets to save computing time. In a fifth subset of 27,787 lambs (Set C) all combinations of recorded grandparents and grand-offspring were accumulated because they contain information on the covariance between direct and maternal effects (cov[AM]). Including cov(AM) in the model assumptions increased estimates of h2 and m2 in all subsets. Estimates from Set C were smallest but showed the same trend. Inbreeding coefficients were computed for 910,444 animals of the Swiss Braunvieh population. Of the animals born in 1984, 71.5% were inbred with 67.9, 3.4, and .2% having inbreeding coefficients between greater than 0 and 5%, greater than 5 to 10%, and greater than 10%, respectively. The average inbreeding coefficient was 1.14% but, for animals with both parents and at least one grandparent known, it was 1.67%. Breeding values for total milk, fat, and protein yields and for fat and protein percentages were predicted using a repeatability animal model including a regression on the inbreeding coefficient. Phenotypic performance was sizeably depressed for milk yield only (-26 kg/% of inbreeding or 2.4% of the phenotypic standard deviation). Adjusting for inbreeding increased the estimated genetic trend slightly. Inbreeding is only partially accounted for when it is ignored in the construction of the inverse of the numerator relationship matrix. This effect was investigated by comparing predicted breeding values from a model including the complete matrix with predicted breeding values from a model including a matrix constructed with inbreeding ignored. Only .8% of all predicted breeding values were affected by more than +/- 5.5 kg. The maximum difference observed was 55.3 kg. The observed average absolute differences between the breeding values of offspring predicted with the two models increased with inbreeding of parents.
