ABSTRACT

The computed tomography (CT) and ultrasound findings in 18 patients (35 kidneys) with non-Hodgkin lymphoma involving the kidneys are presented. Renal involvement was bilateral in 13 patients. The most common presentation was that of multiple intraparenchymal nodules (15 kidneys). Other types of involvement included direct invasion from contiguous retroperitoneal lymph node masses (nine kidneys), solitary renal masses (five kidneys), and diffuse infiltration (two kidneys). On CT most of the lesions were of homogeneous tissue density. Ultrasonographically the renal masses were hypoechoic. We believe that CT is the procedure of choice for the detection and follow-up of renal lymphoma.

ABSTRACT

The ultrasonic appearance of peripancreatic adenopathy in ten patients is described. The enlarged nodes may appear as well-defined, echo-poor, ovoid or rounded masses in a retropancreatic location, or they may surround and silhouette the region of the pancreatic head and body. The former appearance should be easy to differentiate from primary pancreatic neoplasm, but with a confluent mass of peripancreatic nodes it may be impossible to exclude pancreatic disease.

ABSTRACT

The posterior wall of the bronchus intermedius (PWBI) is outlined on most lateral chest radiographs by air in the lumen and in adjacent lung. In a prospective review over a 19-month interval, the authors found 36 patients whose PWBI was thick, defined as thickness greater than the normal upper limit of 3.0 mm (33 patients) or as increase of 1.5 mm in thickness compared with control radiographs (three patients). Uniform thickening was found in 23 patients as a sign of congestive heart failure. Nine subjects with neoplastic thickening of the PWBI demonstrated either uniform (six patients) or lobulated (three patients) enlargement. Two patients with sarcoidosis showed lobulated thickening of the PWBI. Inspection of lateral chest radiographs for thickening of the PWBI may be helpful in establishing bronchial involvement by edema, neoplasm, or enlarged lymph nodes.

ABSTRACT

Prolapsing and/or intussuscepting neoplasms of the terminal ileum are an uncommon cause of a prominent ileocecal valve as seen on a barium enema. Because of their varying position, these lesions require several spot films with reflux into the terminal ileum for their detection and accurate diagnosis. Five cases are described and illustrated, four lipomas and one carcinoid.

ABSTRACT

Three patients had primary hyperparathyroidism and monoclonal serum immunoglobulins. Although multiple myeloma was suspected in each case, subsequent evaluation was consistent with a "benign monoclonal gammopathy". Parathyroid adenomas were removed from two patients. The three patients are presented and compared to the four other cases that have been reported previously. The association between primary hyperparathyroidism and benign monoclonal gammopathy is discussed in terms of possible pathogenetic mechanisms. Primary hyperparathyroidism should be suspected in patients with hypercalcemia and benigh monoclonal gammopathy, as well as in other conditions, like multiple myeloma, that are known to be associated with hypercalcemia.

ABSTRACT

The presence of a low anion gap in patients with multiple myeloma has recently been emphasized. This has been related to the retention of chloride and bicarbonate anions to offset the cationic charge of the monoclonal immunoglobulins. We now report the presence of a low anion gap in 50 patients with asymptomatic plasma cell dyscrasias. No correlation was found between the size of the anion gap and the height of the paraprotein "spike" or the size of the anion gap and an index of the net cationic charge of the monoclonal immunoglobulins.
