ABSTRACT

Silicone chin implants are frequently used in cosmetic surgery to enhance a harmonic face. Obtaining an aesthetically pleasing face is increasingly becoming more important for people, and a considerable part of this goal can be achieved through different aesthetic modifications of the chin. The purpose of this study was to analyze the presence of bone resorption after the insertion of silicone chin implants, with lateral chin radiographs. Fifteen patients were studied, all of whom had a chin silicone implant inserted at least 1 year ago using the same surgical technique. The surgery was done intraorally with insertion of the silicone implant under the periosteum of the chin. Fourteen patients presented bone erosion, with the maximum of 2.0 mm erosion. However, none of them manifested any symptoms of this erosion. In conclusion, even though the majority of the patients presented with bone erosion, the results were minimal and completely asymptomatic; thus, this technique produced an excellent final result.

ABSTRACT

OBJECTIVE: The aim was to study whether the seasonal variation of vitamin D [25(OH)-D or calcidiol] is similar or different in younger and older women living in a southern country. METHODS: Measurement of serum 25(OH)-D concentration in 739 Chilean women aged 20 to 87 years, residents of Santiago (latitude: 33.4° South) who, during a routine gynaecological checkup, agreed to be evaluated. RESULTS: The mean serum concentration of 25(OH)-D for the group was 24.1 ±â€Š10.5 ng/mL. In women 20 to 39 years, the mean was significantly different from the mean of the ≥60 years old group (25.8 ±â€Š10.6 ng/mL vs 23.9 ±â€Š11.1 ng/mL; P < 0.02). Globally, 38.4% of participants had vitamin D deficiency and 36.1% insufficiency. A deficiency was present in 28.4% of the 20 to 39 years old, and in 43.9% in the ≥60 years old group (P < 0.004). In the whole group, a lower proportion (P < 0.0001) of vitamin D deficiency cases in the youngest women occurred during the summer (23.7%) in comparison to the winter (47.7%). It was observed that the proportion of participants in the 20 to 39 years old group with vitamin D deficiency fell from 48.9% in winter to 4.9% in summer (P = 0.0001). In the older groups, this change (less deficiency) is progressively smaller, 51.2% to 27.6% (P = 0.0020) in women 40 to 59 years old, and it does not happen in women ≥60 years (40% with vitamin D deficiency). CONCLUSIONS: Serum vitamin D deficiency [25(OH)-D or calcidiol] is highly prevalent in Santiago, especially in older women (≥60 y) throughout the year. In contrast, in younger women (<40 y), the vitamin D deficiency tends to disappear during summer. More epidemiological studies and targeted prevention actions on vitamin D deficiency are warranted.

ABSTRACT

BACKGROUND: Abdominal wall weakness is a consequence of rectus abdominis diastasis and flaccidity of the myofascial component. A degree of plicature of the rectus abdominis generates an increase of intra-abdominal pressure (IAP), which may result in an increase of intrathoracic pressure, thus affecting thoracic hemodynamics and leading to inadequate ventilation. OBJECTIVES: To assess changes generated by plicature of the rectus abdominis on IAP and pulmonary function in patients undergoing abdominoplasty. METHODS: A total of 10 female patients with abdominal ptosis were included. Chronic smokers and patients with respiratory co-morbidities were excluded. The IAP was measured using a modified Kron's trans-bladder technique. Pulmonary function was assessed by pulmonary compliance (P-Comp) and was calculated with parameters provided by the mechanical ventilator. Both were calculated before and after plicature. RESULTS: The mean values for IAP before and after plicature were 6.6 and 9.3 mmHg respectively. Before plicature, the mean P-Comp value was 38.97 mL/cm of water, and after it was 36.54 mL/cm. Both differences were statistically significant. CONCLUSIONS: Based on the results obtained, it is possible to conclude that plicature of the rectus abdominis generates significant physiological changes, such as an increase in IAP and a decrease of P-Comp, which do not have a clinically relevant impact on healthy individuals. Measuring IAP with the modified technique and the assessment of pulmonary function using P-Comp are both reliable and provide a more accurate correlation with such physiologic changes. ABSTRACT

El uso de la videoscopia en la obtención de colgajos libres está aún siendo desarrollada. Comunicamos la transferencia exitosa de 10 colgajos musculares libres, seis gracilis, dos latissimus dorsi y dos rectos abdominales levantados con asistencia videoscópica e instrumental de laparoscopia. Algunos tejidos permiten su disección endoscópica debido a su ubicación subcutánea y a su fácil acceso. Los músculos gracilis, latissimus dorsi, y recto abdominal reúnen estas características. En nuestra serie, este método representó una considerable disminución del tamaño de la cicatriz y no hubo morbilidad de la zona dadora. La asistencia videoscópica puede ser utilizada en un intento de disminuir la secuela cicatricial de la zona donante en microcirugía reconstructiva.

ABSTRACT

La apendicitis aguda es la intervención quirúrgica más frecuente efectuada en los servicios de urgencia. La clínica sigue siendo el método de elección para efectuar el diagnóstico, dado que hasta el momento los métodos de laboratorio e imágenes no han logrado superar los rendimientos de la clínica, si bien se han constituido en una importante ayuda. En este trabajo se evalúan los casos de pacientes operados por apendicitis aguda, pero con informe histopatológico negativo para esa enfermedad. Ellos corresponden a un bajo porcentaje del total de apendicectomías (4 por ciento), lo que probablemente obedece a un buen estudio clínico y el apoyo de laboratorio e imágenes disponible, además de que la laparoscopía diagnóstica ha disminuido la frecuencia de apendicectomías en blanco. Cuatro de los pacientes correspondieron a otras patologías graves con compromiso apendicular como ser cáncer o enfermedad de Crohn. La morbimortalidad de esta serie correspondió a estos últimos pacientes

ABSTRACT

La histiocitosis sinusal con linfoadenopatía masiva, también conocida como enfermedad de Rosai Dorfman, consiste en una proliferación benigna de tejido fibroso y hematopoiético ubicado generalmente en cabeza y cuello. Es una patología infrecuente, habiéndose publicado cerca de 400 casos en la literatura mundial. La clave del diagnóstico histológico consiste en el hallazgo de histiocitos anormales que contienen linfocitos (emperipolesis), positivos con tinciones immunohistoquímicas para S 100 y negativo para CD1. En los estudios de imágenes raramente se sospecha el diagnóstico, especialmente por la escasa prevalencia y conocimiento de esta patología. Se trata de una enfermedad benigna, autolimitada, cuyo tratamiento generalmente se limita a la resección quirúrgica. Esta permite extirpar el tumor en estudio, realizar el diagnóstico, y aliviar la sintomatolología producida por el efecto masa de la lesión. El pronóstico es excelente sin existir evidencia de recidiva, ni de transformación maligna. Presentamos el caso de un paciente con enfermedad de Rosai Dorfman intraparotídeo, ubicación de la cual no hemos encontrado descripción en la literatura mundial
