ABSTRACT

PURPOSE: To analyze the torque application on prosthetic abutment screws using different maintenance times, to determine an influence on the removal torque values. MATERIALS AND METHODS: A total of 40 external hexagon implants, 40 titanium screws, and 40 customized abutments were used. In group 1, the screws received a torque of 30 N·cm by instant torque application; in groups 2, 3, and 4, torque of 30 N·cm was applied and maintained for 10, 20, and 30 seconds, respectively. Removal torque was performed 10 minutes after torque application. Data were statistically analyzed using 1-way ANOVA and Tukey HSD test (α = 0.05). RESULTS: The mean and standard deviation (±SD) of removal torque values found were 11.61 ± 1.43 N·cm for group 1; 18.64 ± 1.71 N·cm for group 2; 21.62 ± 0.97 N·cm for group 3; and 21.48 ± 1.55 N·cm for group 4. Groups 3 and 4 exhibited statistically higher values than group 2, which demonstrated significantly higher values than group 1 (P < 0.05). CONCLUSIONS: A torque of 30 N·cm applied for 20 seconds seemed to be the best option when considering the removal torque values of external hexagon implants.

ABSTRACT

O sucesso na Implantodontia depende diretamente da harmonia entre o tecido ósseo, a prótese e o tecido mole. Este último, responsável pela arquitetura gengival agradável, é obtido mediante técnicas de manejo tecidual. Entre estas técnicas destacam-se os enxertos pediculados, que apresentam certas vantagens quando comparados com técnicas de enxertos livres. Este artigo busca, por meio de uma revisão da literatura, exemplificar diversos procedimentos de manejo de tecido mole baseados em retalhos pediculados, que visam obter excelência estética associada à função e homeostasia marginal, a fim de selar alvéolos de extração onde foram instalados implantes, ganhar quantidade e qualidade de tecido mole.


The complete success in oral implantology is related to hard tissue, prosthesis and soft tissue´s harmony. The latter can provide a pleasant gingival architecture by means of soft tissue augmentation techniques. These based in pedicle flaps show some advantages when compared to free soft tissue graft techniques. This aim of this paper is to review the great variety of pedicle flaps techniques for esthetics, function and marginal homeostasis, in procedures for sealing of extraction alveolar sockets with immediate implant placement, as well as to improve the quantity and quality of soft tissue.

ABSTRACT

A Implantodontia tem demonstrado que a colocação de implantes não é uma tarefa das mais difíceis. A dificuldade reside em posicionar os implantes adequadamente para instalação, de maneira que o clínico possa optar pelo melhor sistema de retenção, de acordo com sua opinião: parafusada, cimentada ou cimentada-parafusada. Os problemas gerados por implantes malposicionados têm trazido muitas complicações na hora de confeccionar as próteses. Na tentativa de ilustrar uma forma de resolução desses problemas, o presente trabalho descreve um caso clínico de prótese unitária parafusada por lingual. Na região do primeiro molar inferior esquerdo, um implante – que havia sido instalado com inclinação vestibular – teve sua posição corrigida por meio desse sistema de retenção de prótese. Assim, conferiu-se ao trabalho protético estética vestibular sem aparecimento de cinta metálica, estética oclusal semelhante à das coroas cimentadas e, ao mesmo tempo, reversibilidade.


Implantology has shown that the insertion of implants is not one of the most difficult task. The difficulty lies in properly positioning the implant to be installed so that the clinician can choose the best retention system, according to his opinion: screwed, cemented or screwed-cemented. The problems caused by misplaced implants have brought many complications at the time of manufacturing the prosthesis. In an attempt to illustrate one way of solving these problems, this paper describes a clinical case of a single crow prosthesis screwed by lingual. In the region of the lower left first molar, an implant - which had been installed with vestibular inclination - had its position adjusted by means of this prosthesis retention system. Thus, it was given to prosthetic work buccal aesthetics appearance without showing the metallic collar, occlusal esthetics similar to that of cemented crowns and, at the same time, reversibility.
