ABSTRACT

Examination of 65 postmenopausal women with urinary incontinence demonstrated that most of the patients had mixed urinary incontinence. Pathogenesis of urinary incontinence in postmenopausal women is strongly associated with estrogen deficiency. This dictates necessity of preoperative hormone replacement therapy. Surgical treatment of postmenopausal women aims at elevation of obturator pressure, prolongation of the urethra with narrowing of its orifice which can be managed with combination of urethrocystoplasty, levatoroplasty and sling operation.

ABSTRACT

Treatment outcomes are reviewed for 58 females aged 36 to 54 years with genital prolapse (GP) and urine incontinence (UI): colpoptosis (n=44), incomplete falling of the womb (n=14), UI type IIa (n=36), UI type IIb (n=22). All the patients received surgical treatment. Preoperative biological biofeedback-therapy was used. It resulted in a considerable reduction of detrusoric unstability. The operative treatment combined suburethral sling (TVT technique) with consolidation of the pelvic diaphragm. The operation reestablished anatomo-topographic vesico-urethral correlations providing transmission of high abdominal pressure on the urinary bladder and urethra. This stopped urine loss. Efficacy of the technique was proved by clinical and device tests.

ABSTRACT

156 women operated on for genital endometriosis received treatment for infection of the upper urinary tracts diagnosed by ultrasound. The treatment was conducted only in case of normal urodynamics achieved by drainage (on demand) and comprised antibacterial therapy, efferent hemocorrection, intravascular laser irradiation of blood, hyperbaric oxigenation. A response was obtained in 98% patients.

ABSTRACT

The authors analyze the results of multiple-modality treatment of 31 patients suffering from genital endometriosis combined with nephroptosis. Nephropexy with a muscular flap was carried out in all the cases, followed by therapy for genital endometriosis, including hormones, immunomodulators, performed physical factors, hyperbaric oxygenation, etc. Late results were followed up for up to 3 years. The results were found satisfactory, no nephroptosis recurrences were recorded. The authors have arrived at a conclusion on the usefulness of starting the treatment of this patient population by nephropexy.

ABSTRACT

The paper is concerned with the results of combined investigation of 157 women suffering from genital endometriosis, operated on for this disease. The involvement of the lower parts of the ureters in a pathological process was detected in 48 patients before operation and in 6 patients at operation. Comparison of x-ray and operative findings with histology specimens made it possible to define endogenous and exogenous forms of ureteral endometriosis and three variants of an x-ray picture in this pathology (the absence of abnormal changes of the urinary tract, hydroureter is over the narrowed part of the ureter, hydroureteronephrosis). The authors have shown a high informative value of combined x-ray investigation and the effectiveness of its use in clinical practice.

ABSTRACT

The data on the complicated character of endometriosis treatment, in females with the involved urinary system in particular, were discussed. The reported incidence of the affected urinary system in females with endometriosis ranged from 1-2 to 18.5 per cent. Out of 57 patients followed up by the authors, the ureter involvement was diagnosed in 52.6 per cent. For timely diagnosis, the authors recommended the performance of urological examinations prior to the nonoperative treatment. In case of urodynamic disorders, surgical intervention was advised to be the first, its extent depending on affection patterns and the performance of the upper urinary tract. A 1-3-year follow-up evidenced normal renal and urodynamic functioning in operated-on patients.
