ABSTRACT

PURPOSE: To evaluate the radial peripapillary capillary density using optical coherence tomography angiography in patients with and without Helicobacter pylori infection. METHODS: This prospective, cross-sectional study comprised 52 patients (52 eyes: Group 1) and 38 patients (38 eyes: Group 2) with and without H. pylori infections, respectively. The radial peripapillary capillary density and retinal nerve fiber layer thickness in 4 equal quadrants and 2 equal hemispheres in the peripapillary region were calculated using optical coherence tomography angiography. The optic nerve head parameters of the patients were also assessed. RESULTS: The groups were similar in terms of age, gender, and the optic nerve head parameters. The radial peripapillary capillary densities in the superior hemisphere and quadrant were significantly lower in Group 1 than in Group 2 (p=0.039 and p=0.028, respectively) and were positively correlated with the superior hemisphere's retinal nerve fiber layer thickness (p<0.001 and p<0.001, respectively). Similarly, the radial peripapillary capillary densities in the inferior hemisphere and quadrant were also significantly lower in Group 1 compared to Group 2 (p=0.03 and p=0.017, respectively) and were positively correlated with the inferior hemisphere's retinal nerve fiber layer thickness (p<0.001 and p<0.001, respectively). The retinal nerve fiber layer thickness in the nasal and temporal quadrants were significantly decreased in Group 1 when compared to Group 2 (p=0.013 and p=0.022) and were positively correlated with the corresponding radial peripapillary capillary densities of the 2 quadrants (p=0.002 and p=0.022). CONCLUSION: The decreased radial peripapillary capillary density in the H. pylori-positive patients suggests that H. pylori may play a role in the etiopathogenesis of glaucoma.

RESUMO Objetivos: Avaliar a densidade capilar peripapilar radial de pacientes com e sem infecção por Helicobacter pylori (H. pylori) por meio de angiotomografia de coerência óptica. Métodos: Cinquenta e dois olhos de 52 pacientes com infecção por H. pylori (Grupo 1) e 38 olhos de 38 pacientes sem infecções por H. pylori (Grupo 2) foram incluídos neste estudo prospectivo e transversal. A densidade capilar peripapilar radial (%) e a espessura da camada de fibra nervosa retiniana (μm) em 4 setores iguais e 2 hemisférios iguais foram calculados automaticamente na região peripapilar por angiotomografia de coerência óptica. Os parâmetros da cabeça do nervo óptico dos pacientes também foram avaliados. Resultados: Os grupos foram semelhantes em relação aos parâmetros: idade, sexo e cabeça do nervo óptico. As densidades capilares peripapilares radiais no hemisfério superior, hemisfério inferior, quadrante superior e quadrante inferior foram significativamente menores no Grupo 1 do que no Grupo 2 (p=0,039, p=0,03, p=0,028 e p=0,017 respectivamente). As densidades capilares peripapilares radiais, tanto no hemisfério superior quanto no quadrante superior, foram correlacionadas positivamente com a espessura da camada de fibra nervosa da retina do hemisfério superior (p<0,001 e p<0,001). As densidades capilares peripapilares radiais no hemisfério inferior e no quadrante inferior foram positivamente correlacionadas com a espessura da camada do nervo retiniano do hemisfério inferior (p<0,001 e p<0,001). A espessura da camada da fibra nervosa retiniana nos quadrantes nasal e temporal diminuiu significativamente no Grupo 1 quando comparado ao Grupo 2 (p=0,013 e p=0,022), e esses valores foram positivamente correlacionados com as densidades capilares peripapilares radiais correspondentes nos quadrantes nasal e temporal (p=0,002 e p=0,022). Conclusão: A diminuição das densidades capilares peripapilares radiais nos olhos de indivíduos positivos para H. pylori sugere que H. pylori pode desempenhar um papel na etiopatogenia do glaucoma.

ABSTRACT

PURPOSE: To investigate the relationships between (i) thickness of the retinal nerve fiber layer, optic nerve head topography, and visual field parameters and (ii) corneal biomechanical properties in normal controls and patients with ocular hypertension and primary open-angle glaucoma. METHODS: This observational, cross-sectional study included 68 eyes with primary open-angle glaucoma, 99 eyes with ocular hypertension and 133 control eyes. Corneal biomechanical properties, optic nerve head topographic features, retinal nerve fiber layer thickness, and visual fields were assessed in all cases. Corneal biomechanical properties, retinal nerve fiber layer thicknesses, and optic nerve head topographic features were compared among the groups. The associations between structural and functional measures of glaucomatous damage and corneal biomechanical factors were also evaluated. RESULTS: Significantly lower corneal hysteresis and corneal resistance factor values were observed in the primary open-angle glaucoma and ocular hypertension groups as compared with the control group, but there were no significant differences between the primary open-angle glaucoma and ocular hypertension groups. In the ocular hypertension group, no associations were observed between the corneal hysteresis and corneal resistance factor with values and the structural and functional parameters. In the primary open-angle glaucoma group, positive correlations were observed between the corneal hysteresis values and the global retinal nerve fiber layer thickness (p<0.01, r=0.27), mean retinal nerve fiber layer thickness (p<0.01, r=0.33), and mean deviation (p<0.01, r=0.26), and negative correlations were observed between the corneal resistance factor values, and the cup area (p<0.01, r=-0.39), cup-to-disk ratio (p=0.02, r=-0.28), linear cup-to-disk ratio (p=0.02, r=-0.28), and cup shape (p=0.03, r=-0.26). In the control group, weak correlations were detected between the corneal hysteresis and the cup area (p=0.03, r=0.19), cup-to-disk ratio (p=0.01, r=0.21), and linear cup-to-disk ratio (p=0.01, r=0.22). CONCLUSIONS: Distinct correlations were identified between the corneal hysteresis and corneal resistance factor values and the functional and structural parameters in the primary open-angle glaucoma and control groups. Corneal hysteresis and corneal resistance factor may have different roles in the pathophysiology of glaucoma.

ABSTRACT

PURPOSE: To determine the clinical characteristics and seasonal distribution of patients admitted to the ocular emergency department of a tertiary ophthalmology care center. METHODS: The study cohort includes 27,120 patients who were admitted to ocular emergency room between November 2013 and November 2014. The age, sex, reason for admission, diagnosis, and complete ocular examination reports were recorded for each patient. X-ray and ultrasonographic examinations were performed if necessary. RESULTS: The mean patient age was 32.83 ± 17.62 years (range, 0-95). The number of males was nearly two times the number of females, with 18,808 (69.4%) males and 8312 (30.6%) females. The diagnoses included viral conjunctivitis (7,859 patients; 29.0%), corneal foreign body (5,286 patients; 19.5%), bacterial conjunctivitis (3,892 patients; 14.4%), corneal abrasions (2,306 patients; 8.5%), and allergic conjunctivitis (1,433 patients; 5.3%) (Table 1). Other frequent diagnoses included subconjunctival hemorrhage, photo keratopathy, chemical eye injury, and penetrating and blunt eye injuries. Allergic conjunctivitis, ocular trauma, and corneal foreign body were more frequent in spring, whereas keratitis and chemical eye injury were more common in winter (chi-square test). The most common reasons for emergency room admission, in order of frequency, were viral conjunctivitis, corneal foreign body, bacterial conjunctivitis, and corneal abrasions. CONCLUSION: This study is the first long-term prospective study to evaluate the seasonal distribution and diagnosis of all adult and pediatric patients admitted to the emergency room for ocular conditions. The frequency of ophthalmological conditions seen in the emergency room may vary according to the season.

PURPOSE: To compare the anterior segment parameters of patients with pseudoexfoliation syndrome, patients with pseudoexfoliation glaucoma, and normal subjects. METHODS: This prospective, controlled, comparative study included 150 eyes of 150 patients. The patients were divided into the pseudoexfoliation syndrome group, the pseudoexfoliation glaucoma group, and the control group (50 patients in each group). Axial length, central corneal thickness, aqueous depth, anterior chamber depth, lens thickness, K1 and K2 keratometry values, and white to white distance measurements were obtained by optical biometry and compared between the groups. RESULTS: The mean ages of the pseudoexfoliation syndrome, pseudoexfoliation glaucoma, and control patients were 62.18 ± 6.21, 61.80 ± 6.62, and 59.40 ± 6.89 years, respectively. There were no statistically significant differences between the groups in mean age or sex ratio (p>0.05). Mean central corneal thickness was statistically significantly greater, mean aqueous depth and anterior chamber depth were statistically significantly greater, and mean lens thickness was statistically significantly less in the control group than in the pseudoexfoliation syndrome and pseudoexfoliation glaucoma groups (p<0.05). Pairwise comparisons of the pseudoexfoliation syndrome group and the pseudoexfoliation glaucoma group revealed that there were no significant differences between these two groups in central corneal thickness, aqueous depth, anterior chamber depth, and lens thickness (p>0.017). CONCLUSIONS: Patients with pseudoexfoliation glaucoma and pseudoexfoliation syndrome had greater lens thickness, shallower aqueous depth and anterior chamber depth, and less central corneal thickness than normal subjects. None of the anterior segment parameters differed between patients with pseudoexfoliation syndrome and patients with pseudoexfoliation glaucoma.

RESUMO Objetivos: Comparar os parâmetros do segmento anterior de casos de síndrome de pseudo-esfoliação, de glaucoma pseudo-esfoliação e de indivíduos normais. Métodos: O presente estudo prospectivo comparativo controlado incluiu 150 olhos de 150 pacientes. Os pacientes foram divididos em três grupos: grupo síndrome de pseudo-esfoliação, grupo glaucoma pseudo-esfoliação e grupo controle (50 em cada grupo). O comprimento axial, a espessura corneana central, a profundidade aquosa, a profundidade da câmara anterior, a espessura da lente, os valores de ceratometria K1 e K2 e as medidas branco a branco, obtidas por biometria óptica, foram comparados entre os grupos. Resultados: As idades médias dos indivíduos do grupo síndrome de pseudo-esfoliação, glaucoma pseudo-esfoliação e controle foram 62,18 ± 6,21, 61,80 ± 6,62 e 59,40 ± 6,89 anos, respectivamente. Entre os grupos, não houve diferenças estatisticamente significativas quanto às idades e ao gênero dos pacientes (p>0,05, para todos). A espessura da córnea central média foi significativamente mais espessa, a profundidade média aquosa e a profundidade da câmara anterior foram significativamente mais profundas e a espessura média da lente foi significativamente mais fina no grupo controle do que nos grupos síndrome de pseudo-esfoliação e glaucoma pseudo-esfoliação (p<0,05, para todos). As comparações por pares do grupo síndrome de pseudo-esfoliação e do grupo glaucoma pseudo-esfoliação (p<0,05, para todos). As comparações por pares do grupo síndrome de pseudo-esfoliação e do grupo glaucoma pseudo-esfoliação não revelaram diferenças significativas entre esses dois grupos quanto à espessura corneana central, à profundidade aquosa, à profundidade da câmara anterior e aos valores de espessura da lente (p>0,017, para cada um). Conclusões: Os casos de glaucoma pseudo-esfoliação e de síndrome de pseudo-esfoliação apresentaram lente mais espessa, menor profundidade aquosa, menor profundidade da câmara anterior e espessura corneana central mais fina do que os indivíduos normais. No entanto, nenhum dos parâmetros do segmento anterior foi diferente entre os indivíduos do grupo síndrome de pseudo-esfoliação e do grupo glaucoma pseudo-esfoliação.

RESUMO Objetivo: Determinar as características clínicas e a distribuição sazonal dos pacientes admitidos no departamento de emergências oculares de um centro terciário de cuidados oftalmológicos. Métodos: Um total de 27.120 pacientes, admitidos no pronto atendimento ocular entre novembro de 2013 e novembro de 2014, foram incluídos neste estudo prospectivo. Idade, gênero, causa da admissão, diagnóstico e relatórios completos dos exames oculares dos pacientes foram registrados e exames de raios X e de ultrassonografia foram realizados quando necessários. Resultados: A idade média do paciente foi de 32,83 ± 17,62 anos (intervalo, 0-95). O número de homens era quase duas vezes maior que o número de mulheres, com 18.808 (69,4%) do sexo masculino e 8.312 (30,6%) do sexo feminino. Os diagnósticos incluíram conjuntivite viral (7.859 pacientes, 29,0%), corpo estranho corneano (5.286 pacientes, 19,5%), conjuntivite bacteriana (3.892 pacientes, 14,4%), abrasões corneanas (2.306 pacientes, 8,5%) e conjuntivite alérgica (1.433 pacientes, 5,3%) (Tabela 1). Outros diagnósticos freqüentes incluíram hemorragia subconjuntival, queratopatia fotográfica, lesões oculares químicas e lesões oculares penetrantes e contundentes. A conjuntivite alérgica, o trauma ocular e o corpo estranho da córnea foram mais frequentes na primavera, enquanto que a queratite e lesões oculares químicas foram mais comuns no inverno (teste qui-quadrado). Os motivos mais comuns para a admissão na sala de emergência, em ordem de frequência, foram conjuntivite viral, corpo estranho da córnea, conjuntivite bacteriana e abrasões da córnea. Conclusão: Este é o primeiro estudo prospectivo de longa duração avaliando as causas e a distribuição sazonal de todos os casos de admissão em um pronto atendimento oftalmológico para pacientes em idade adulta ou pediátrica. A frequência das causas de encaminhamento ao pronto atendimento pode variar em função da estação do ano.

ABSTRACT

PURPOSE: To compare the conjunctival, lacrimal sac, and nasal flora cultures and conjunctival normalization time following external (EX-), endoscopic (EN-), and transcanalicular multidiode laser (TC-) dacryocystorhinostomy (DCR) and to evaluate the relationship between culture positivity and surgical success. We further performed antibiotic sensitivity analyses for lacrimal sac culture samples. METHODS: A total of 90 patients with primary acquired nasolacrimal duct obstruction were recruited and divided into EX-DCR (n=32), EN-DCR (n=28), and TC-DCR (n=30) groups. Conjunctival, nasal, and lacrimal sac cultures and antibiograms were analyzed. RESULTS: In all three groups, coagulase-negative Staphylococcus (CNS) was predominantly isolated preoperatively from the conjunctiva, nose, and lacrimal sac and postoperatively from the conjunctiva. Preoperative and postoperative conjunctival culture positivity rates were similar between all the groups (p>0.05). A statistically significant difference in the growth rate of culture in the lacrimal sac was observed between the three groups (p=0.001). CNS and Staphylococcus aureus cultures were predominantly sensitive to linezolid, teicoplanin, tigecycline, vancomycin, and mupirocin. Conjunctival normalization times were similar between the three groups (p>0.05). Anatomical and functional success rates were not found to be significantly correlated with preoperative conjunctival and lacrimal sac culture positivity (p>0.05). CONCLUSIONS: Similar rates of preoperative and 1-week postoperative conjunctival culture positivity were observed in all the groups; a significantly lower bacterial growth rate was observed in postoperative conjunctival cultures. CNS was the most commonly isolated organism. Bacterial growth rates in the lacrimal sac samples were significantly higher in the EN-DCR group. Bacterial growth rates obtained preoperatively from the conjunctival and lacrimal sac culture samples were not correlated with DCR success.

ABSTRACT

ABSTRACT Purpose: To compare the conjunctival, lacrimal sac, and nasal flora cultures and conjunctival normalization time following external (EX-), endoscopic (EN-), and transcanalicular multidiode laser (TC-) dacryocystorhinostomy (DCR) and to evaluate the relationship between culture positivity and surgical success. We further performed antibiotic sensitivity analyses for lacrimal sac culture samples. Methods: A total of 90 patients with primary acquired nasolacrimal duct obstruction were recruited and divided into EX-DCR (n=32), EN-DCR (n=28), and TC-DCR (n=30) groups. Conjunctival, nasal, and lacrimal sac cultures and antibiograms were analyzed. Results: In all three groups, coagulase-negative Staphylococcus (CNS) was predominantly isolated preoperatively from the conjunctiva, nose, and lacrimal sac and postoperatively from the conjunctiva. Preoperative and postoperative conjunctival culture positivity rates were similar between all the groups (p>0.05). A statistically significant difference in the growth rate of culture in the lacrimal sac was observed between the three groups (p=0.001). CNS and Staphylococcus aureus cultures were predominantly sensitive to linezolid, teicoplanin, tigecycline, vancomycin, and mupirocin. Conjunctival normalization times were similar between the three groups (p>0.05). Anatomical and functional success rates were not found to be significantly correlated with preoperative conjunctival and lacrimal sac culture positivity (p>0.05). Conclusions: Similar rates of preoperative and 1-week postoperative conjunctival culture positivity were observed in all the groups; a significantly lower bacterial growth rate was observed in postoperative conjunctival cultures. CNS was the most commonly isolated organism. Bacterial growth rates in the lacrimal sac samples were significantly higher in the EN-DCR group. Bacterial growth rates obtained preoperatively from the conjunctival and lacrimal sac culture samples were not correlated with DCR success.


RESUMO Objetivo: Comparar a flora conjuntival, do saco lacrimal e nasal com o tempo de normalização após dacriocistorrinostomia (DCR) externa (EX-), endoscópica (EN-) e transcanalicular a laser de multi diodo (TC-) para correlacionar a positividade da cultura com o sucesso cirúrgico, assim como identificar a sensibilidade aos antibióticos em amostras de saco lacrimal. Métodos: Neste estudo prospectivo, 90 pacientes com obstrução do canal nasolacrimal adquirida primária foram incluídos e divididos em grupos EX-DCR (n=32), EN-DCR (n=28) e TC-DCR (n=30). Culturas e antibiogramas conjuntivais, nasais e do saco lacrimal foram analisados. Resultados: Staphylococcus coagulase-negativo (CNS) foi o organismo predominante isolado no pré-operatório (conjuntiva e nariz), no transoperatório (saco lacrimal) e pós-operatório (conjuntiva), nos 3 grupos. Taxas de positividade de cultura da conjuntiva pré- e pós-operatórias nos três grupos foram semelhantes (p>0,05). A diferença nas taxas de crescimento do saco lacrimal dos três grupos foi estatisticamente significativa (p=0,001). CNS e S. aureus foram mais sensíveis a linezolida, teicoplanina, a tigeciclina, vancomicina e mupirocina. O tempo de normalização conjuntival foi semelhante nos três grupos (p>0,05). Não houve relação estatisticamente significativa entre as taxas de sucesso anatômicas e funcionais e a positividade da cultura conjuntival e de saco lacrimal pré-operatória (p>0,05). Conclusões: Pacientes submetidos a EX-DCR, EN-DCR, e TC-DCR apresentaram positividades de cultura conjuntival semelhantes no pré-operatório e na 1a semana pós-operatória. Houve uma redução significativa na taxa de crescimento das culturas da conjuntiva pós-operatórias. O organismo mais comumente isolado foi o CNS. A taxa de crescimento de bactérias a partir do saco lacrimal foi significativamente maior no grupo PT-DCR. O crescimento bacteriano da conjuntiva no pré-operatório e de amostras do saco lacrimal no transoperatório não se relacionaram com o sucesso da DCR.
