ABSTRACT

In a series of articles spanning 8 years, Ed Ellis reviewed the clinical results of the treatment of 478 mandibular angle fractures managed by eight different techniques. During a series of benchtop investigations employing polyurethane synthetic mandible replicas, Rich Haug investigated the biomechanical behavior of approximately 15 different techniques designed to reconstruct mandibular angle fractures. This article reviews these two series of investigations in an attempt to gain insight into the biomechanical and biological factors that affect the successful reconstruction of mandibular angle fractures. It appears that the current techniques used to reconstruct mandibular angle fractures are sound from the standpoint of biomechanics within a range of forces encountered during clinical function. It also appears that an unsuccessful reconstruction is based on a biological result of a behavioral issue such as noncompliance, substance abuse, and/or nutritional or immune compromise.

ABSTRACT

Relapse in orthognathic surgery is multifactorial and can be attributed to posttreatment growth, condylar changes, lack of rigid fixation, and muscle pull and function. Consideration of these factors can aide the surgeon in the decision-making process with regards to treatment options and alternatives. This article reviews the stability of various orthognathic movements using traditional osteotomies and fixation, and compares them to what is currently in the literature regarding distraction osteogenesis.

ABSTRACT

Diabetes is a chronic disease characterized by the body's inability to process blood glucose properly. It is generally classified as insulin-dependent diabetes mellitus (IDDM), or type 1, and non-insulin-dependent diabetes mellitus (NIDDM), or type 2. Type 1 is characterized by a defect in insulin secretion by the beta cells of the pancreas, usually secondary to autoimmune destruction of those cells. Type 2 is characterized by peripheral insulin resistance with an insulin-secretory defect that varies in severity. Diabetes is a common medical condition that affects 6% of Americans younger than 50 years and approximately 10% to 15% of those older than 50 years. Increasing numbers of patients who have diabetes are presenting to the oral surgeon's office for care. Patients who have diabetes have a 50% chance of undergoing a surgical procedure in their lifetime.
