ABSTRACT

PURPOSE: Conjunctival nevi in young individuals can correspond to the entity named Inflammatory Juvenile Conjunctival Nevus (IJCN), presenting clinically as a rapid growth lesion, and showing at the histopathological study an inflammatory infiltrate surrounding the lesion. All these findings can suggest a diagnosis of malignancy. Due to a case of IJCN diagnosed in our Pathology department, we realized that this entity is rarely reported in the literature and histopathological diagnostic criteria are not well defined. The aim of our study is to compare the histopathological characteristics of conjunctival nevi in patients aged thirty years or less to those in patients above 30 years, looking for the findings described in IJCN. METHODS: All the excisional specimens of resected conjunctival nevus in a tertiary hospital from 2000 to 2018 were retrieved from the Pathology department archives. Demographic data were recorded, and histopathological variables (histological type of nevus, lymphocytic infiltration, eosinophilic infiltration, presence of lymphoid follicles, stromal nevomelanocytic component, intraepithelial nevomelanocytic component, epithelial inclusions, quantity of goblet cells in epithelial inclusions, cellular atypia, mitoses and maturation of the lesion) were evaluated by three independent observers. Statistical analysis was performed comparing the two age groups. RESULTS: The study determined a significant predominance of the lymphocytic and eosinophilic infiltration in the group of patients aged thirty years or less respect to the elderly group. The percentage of stromal component of the lesion is larger in patients over thirty years compared to the younger group. There was no correlation between epithelial inclusions, maturation or cytological atypia and age groups. CONCLUSION: We found some histopathological differences in conjunctival nevi related to young age, some of them coincident with the ones described in IJCN, which histopathologically could lead to a misleading diagnosis. However, we did not find significant differences related to age in many of the described histopathological findings described in IJCN. Larger series with a greater number of cases would be of interest to characterize more precisely this lesion.

ABSTRACT

An accurate preoperative evaluation of the vascular anatomy of the abdominal wall is extremely valuable in improving the surgical strategy in abdominal perforator flaps. The multidetector-row computer tomography offers thin slice coverage of extended volumes with an extremely high spatial resolution. From October 2003 to December 2004, 66 female patients had breast reconstruction surgery in our department using the deep inferior epigastric artery perforator flap. Our multidetector-row computer tomography studies were performed using a 16-detector-row computer tomography scanner. The image assessment was carried out using the following protocol: we first identified the best three perforators from each side of the abdomen. Then we conducted a three-dimensional reconstruction of the abdomen by identifying exactly where the three best perforators emerged from the rectus abdominis fascia. We then transferred the data obtained from the image to the patient using a coordinate system. In addition, we also placed the dominant perforators in the patient by using a conventional hand-held Doppler. During the operation we compared intra-operative findings, Doppler results and computer tomography outcomes. Neither false positive nor false negative results were found in the computer tomography outcome. Multidetector-row computer tomography provides us with an easy method of interpreting the virtual anatomic dissection in three dimensions. It has high sensitivity and specificity and provides a good quality evaluation of the perforator vessels. This information allows reduction of operating time and safer performance of surgery. The multidetector-row computer tomography is a highly effective tool in the preoperative study of abdominal perforator flaps.

Se realizó un estudio descriptivo, longitudinal de 133 pacientes con el diagnóstico de cicatrices inestéticas vistos en consulta de Cirugía Plástica del policlínico de Especialidades "Saturnino Lora" de Santiago en el período de febrero de 1995 a enero de 1996.Se realizó un segumiento con evoluciones periódicas, de acuerdo a la coloración de la piel, se realizaron biopsias antes del tratamiento con el peloide del tejido afectado y piel sana, la medición de las características de las lesiones después del tratamiento se hizo en base a los criterios de evaluación de eficacia de la cicatrización así como por las declaraciones de los propios pacientes tratados

ABSTRACT

The complications of the pectoralis major myocutaneous flap (PMMF) placed at cervical level, carried out in oncologic patients during the period 1984-1992 are reviewed. A total of 33 PMMF for reconstruction at the cervical level were carried out in 30 patients. Data referred to the age, sex, indication, technics, complications and evolution of the patients were analysed. The global rate of patients affected by complications was 67%, with a 17% of failures. Considering the 33 PPMF carried out, in 15 occasions there appeared fistulas (45%) and in other 8 cases dehiscence or partial necrosis (24%). In only one case a total necrosis of the flap appeared (3%). The appearance of complications significatively increased the hospital stay.
