ABSTRACT

We conducted validation of a scale to measure nursing workloads, previously designed using NIC interventions within the four nursing functions (patient care, teaching, management, and research). METHODS: This is an analytical, descriptive, prospective, and observational study using qualitative methodology (focus groups and in-depth interviews) with a quantitative and qualitative section (committee of experts and real application of the scale through a validation pilot and with multicentric application, including hospitalization units of internal medicine and surgery of four hospitals). Qualitative analysis was performed with Atlas.ti8 and quantitative analysis with R. RESULTS: Qualitatively, all the participants agreed on the need to measure workloads in all nursing functions with standardized terminology. The expert committee found greater relevance (91.67%) in "prevention" and "health education" as well as consistency with the construct and adequate wording in 99% of the selected items. In the pilot test and multicenter application, the nurses spent more time on the caring dimension, in the morning shift, and on the items "self-care", "medication", "health education", "care of invasive procedures", "wounds care", "comfort", and "fluid therapy". Cronbach's alpha 0.727, composite reliability 0.685, AVE 0.099, and omega coefficient 0.704 were all acceptable. Construct validity: KMO 0.5 and Bartlett's test were significant. CONCLUSIONS: The scale can be considered valid to measure nursing workloads, both qualitatively in obtaining the consensus of experts and health personnel and quantitatively, with acceptable reliability and validity superior to other similar scales.

ABSTRACT

Objectives: The study aimed to the multicenter application of a nursing workload measurement scale in the internal medicine and surgery adults hospitalization units. Methods: The study design was a multicenter, observational, and descriptive study. A multicenter application of the MIDENF® nursing workload measurement scale was carried out, which consists of 21 items, and covers the four nursing functions (patient care items, teaching, manager, and researcher), in units of hospitalization of adults of internal medicine and surgery of four different hospitals. Each item contains one or more of the nursing interventions of Nursing Interventions Classification (NIC) and has an assigned time, after comparing the real time it takes to perform each intervention with the North American Nursing Diagnosis Association (NANDA) standardized time. The study was carried out during nine months of the year 2020, measuring two days each month in the three work shifts (morning, evening, and night) to all patients admitted on the days of measurement in the indicated units. Results: The descriptive and inferential analysis of 11,756 completed scales, 5,695 in general surgery and 6,061 in internal medicine, showed a greater care load for the two units during the morning shift (227,034 min in general surgery, 261,835 min in internal medicine), especially in the items of "self-care", "medication", "common invasive procedures", "fluid therapy", and "patient and family support", while the managerial function was similar during the three work shifts in the two units studied, getting values between 57,348 and 62,901 min. In the analysis by shift and unit, statistical significance was obtained both for the total workload and the four nursing functions(P < 0.001). Conclusions: It is shown that the use of validated scales with the standardized language of nursing functions, adapted to the units, provides objective information to adjust the nursing staff to the real situation of care in any hospital and unit where it is applied, improving quality and patient safety.

Among Nursing and Psychology professionals, the job satisfaction of those in Adult Inpatient Units is analyzed, with a new scale to measure nursing workloads validated. Objective: The objective of this study was to relate nursing workloads to professional job satisfaction. Methods: This is an observational, analytical, descriptive, concurrent and quantitative study, which used the Overall Job Satisfaction scale and subscales therein, to identify global satisfaction-intrinsic or related to motivational factors and extrinsic or associated with hygienic factors-in nursing professionals (n = 104) from eight Inpatient Units of Internal Medicine and Surgery, in four hospital centers, to describe job satisfaction in the professionals studied and to find statistically significant associations between job satisfaction and workload (measured with the scale MIDENF®) in the inpatient units where they work. Results: There were higher levels of satisfaction in the variables "relationship with immediate boss" and "relationship with fellow workers", and lower levels in "relationship with senior management" and "organizational system of the unit". In the inferential analysis, the scores were 75.63 for overall satisfaction, 35.28 for intrinsic satisfaction, and 40.36 for extrinsic satisfaction. Conclusions: There is a close relationship between workload and job satisfaction, showing more dissatisfaction regarding organizational aspects and professional recognition.

ABSTRACT

OBJETIVO: Analizar el nivel y la relación entre habilidades de comunicación, autoeficacia percibida y síndrome de Burnout en profesionales de la salud del servicio de urgencias hospitalario y unidad de cuidados críticos de un hospital comarcal. MÉTODO: Se plantea un estudio observacional y transversal en una muestra de 90 profesionales sanitarios de un Hospital Comarcal, 19 médicos (21,1%), 47 enfermeros (52,2%), 24 auxiliares de enfermería (26,7%). Se han usado la Escala sobre Habilidades de Comunicación en Profesionales de la Salud, el Maslach Burnout Inventory Human Services Survey y la escala de la Autoeficacia percibida. RESULTADOS: Se obtiene una correlación negativa entre las diferentes dimensiones de la Escala sobre Habilidades de Comunicación en Profesionales de la Salud, la realización personal en el trabajo del Maslach Burnout Inventory y la autoeficacia percibida con las dimensiones agotamiento emocional y despersonalización del Maslach Burnout Inventory. Por otro lado, se observa una correlación positiva y estadísticamente significativa entre las dimensiones de las habilidades de comunicación con la dimensión realización personal en el trabajo del Maslach Burnout Inventory y la autoeficacia percibida. CONCLUSIONES: Las habilidades de comunicación y la autoeficacia percibida de los profesionales sanitarios en los servicios de urgencias hospitalarios y las unidades de cuidados críticos están relacionadas con un menor agotamiento emocional y despersonalización, y una mayor realización personal en el trabajo


AIM: To analyze the level and relationship between communication skills, perceived self-efficacy and Burnout syndrome in health professionals from the hospital emergency department and the critical care unit of a regional hospital. METHOD: An observational and cross-sectional study was conducted with a sample of 90 health professionals from a Regional Hospital, 19 physicians (21.1%), 47 nurses (52.2%), 24 nursing aides (26.7%). The Communication Skills Scale for Health Professionals, the Maslach Burnout Inventory Human Services Survey and the Scale of Perceived Self-Efficacy were utilized. RESULTS: A negative correlation was obtained between the different dimensions of the Communication Skills Scale for Health Professionals, the personal accomplishment at work of the Maslach Burnout Inventory and the perceived self-efficacy with the emotional exhaustion and depersonalization dimensions of the Maslach Burnout Inventory. On the other hand, a positive and statistically significant correlation was observed between the dimensions of communication skills with the personal accomplishment dimension in the Maslach Burnout Inventory work and the perceived self-efficacy. CONCLUSIONS: The communication skills and the perceived self-efficacy of the health professionals in the emergency department and critical care unit are related to less emotional exhaustion and depersonalization, and greater personal accomplishment at work

ABSTRACT

Introducción: la musicoterapia se entiende por el uso de la música o sus elementos con fines terapéuticos. Su empleo en la enfermería es escaso más allá del uso de la intervención NIC 4400. Objetivo: conocer y analizar el grado de aceptación por parte de los estudiantes de enfermería ante la posible inclusión de la musicoterapia como asignatura en los programas educativos de los estudios de Grado en Enfermería. Método: se ha realizado un estudio observacional descriptivo de corte transversal con una muestra n=346 participantes. Resultados: los resultados muestran que el 93,93% de los alumnos sí incluirían la musicoterapia como una asignatura Optativa en los estudios de Grado en Enfermería, haciendo uso de ella en su futuro profesional


Introduction: music therapy is understood by the use of music or its elements for therapeutic purposes. Its use in nursing is scarce beyond the use of the NIC 4400 intervention. Objective: the research aims to know and analyze the degree of acceptance by nursing students regarding the possible inclusion of music therapy as a subject in the educational programs of the Nursing Degree studies. Method: a cross-sectional, observational, descriptive study was carried out with a sample of n=346 participants. Conclusion: the results show that 93.93% of the students did include music therapy as an optional subject in Nursing Degree studies, making use of it in their professional future

ABSTRACT

Objetivo: Identificar los factores que influyen en cambios en la ansiedad de los cuidadores tres meses después del alta hospitalaria por exacerbación aguda de la Enfermedad Pulmonar Obstructiva Crónica (EPOC). Metodología: Estudio longitudinal. Participaron 87 cuidadores de pacientes hospitalizados por exacerbación aguda de EPOC. Se midió la ansiedad en el momento de la hospitalización y tres meses después del alta. Además, se midieron potenciales factores asociados a su cambio en cuatro dominios: Contexto del cuidado, demandas del cuidado, recursos y características del paciente. Utilizamos regresiones logísticas múltiples univariadas y multivariadas para determinar los cambios en la ansiedad tres meses después. Resultados: Presentaron ansiedad en el momento de la hospitalización el 57,5% de los cuidadores. De ellos, el 44% había remitido su ansiedad tres meses después del alta hospitalaria. Sin embargo, el 22% de los cuidadores quienes no habían presentado ansiedad en el momento de la hospitalización se mostraron ansiosos a los 3 meses. La gravedad de la EPOC y no recibir apoyo de otro cuidador disminuyó las probabilidades de remisión de la ansiedad. La sobrecarga moderadamente alta incrementa las probabilidades de presentar nuevos síntomas de ansiedad. Conclusiones: La percepción de los síntomas de la ansiedad es dinámica. Los cuidadores pueden recuperarse si reciben ayuda de otro cuidador o si el paciente al que cuidan no está en un estado severo de EPOC (AU)


Objective: To identify the factors that influence changes in caregivers anxiety status three months after discharge for acute exacerbation of Chronic Obstructive Pulmonary Disease (COPD). Methodology: Longitudinal study. Participants included 87 caregivers of patients hospitalized for acute exacerbation of COPD. Anxiety was measured at the time of hospitalization and three months after discharge. We measured factors from four domains: context of care, caregiving demands, caregiver resources, and patient characteristics. We used multiple univariate and multivariate logistic regressions to determine changes in anxiety three months later. Univariate and multivariate multiple logistic regressions were used to determine changes in anxiety three months later. Results: A total of 57.5% of caregivers reported anxiety at the time of hospitalization. Of these, 44% had a remission of their anxiety three months after discharge. However, 22% of caregivers who had not experienced anxiety at the hospitalization became anxious at 3 months. The severity of COPD and not receiving help from another caregiver decreased the likelihood of remission of anxiety. Moderately high overload increases the likelihood of experiencing anxiety symptoms. Conclusions: The perception of anxious symptoms is dynamic. Caregivers are likely to recover from anxiety when they receive help from another caregiver and if the patient they are caring for does not have severe COPD (AU)

Objetivo: Determinar a associação entre variáveis sociodemográficas e de trabalho da equipe de enfermagem, e o nível de satisfação no uso das escalas NEMS e NAS em uma UTI, observando qual a mais adequada. Método: Estudo descritivo, incidental, quantitativo e concorrente. Avaliação das escalas NAS e NEMS por enfermeiros/as de UTI através de um questionário elaborado e validado para uma amostra. Análise descritiva e associação das variáveis sociodemográficas e de trabalho com cada pergunta do questionário. Análise estatística usando SPSS versão V.18.0. Resultados: A escala NAS obteve melhor avaliação global (7,20 pontos), e em todos os itens (de 5,66 a 7,36 pontos), que a escala NEMS (3,93 pontos). Conclusão: Profissionais mais antigos no hospital avaliam melhor a escala NEMS, e os mais antigo na UTI consideram a NAS mais apropriada para medir cargas de trabalho para a unidade, pacientes e profissionais, em comparação com NEMS .

Rating NAS and NEMS scales by nurses / as ICU through a questionnaire designed and validated for a sample. Descriptive analysis, and association of sociodemographic and labor variables with each question in the questionnaire. Statistical analysis using SPSS version V.18.0. Results: The NAS gets better overall assessment scale (7.20 points) and all items (from 5.66 to 7.36 points), the NEMS (3.93 points) scale. Conclusion: Older professionals in the hospital better value NEMS scale, and the oldest in UCI considered the most appropriate NAS workloads to measure for the unit, patients and professionals, compared with NEMS .

Conclusión: Profesionales más antiguos en el hospital valoran mejor la escala NEMS, y los más antiguos en UCI consideran la NAS más adecuada para medir cargas de trabajo para la unidad, pacientes y profesionales, en comparación con la NEMS .

ABSTRACT

Objetivo: Sintetizar la producción de conocimiento relacionado al uso de Prácticas Integradas y Complementarias (PIC) en el tratamiento del estrés laboral. Método: Revisión integradora, que tuvo como pregunta guía: ¿cuál es la producción del conocimento de las PIC relacionada al tratamiento del estrés laboral en el período de 2003 a 2013?. Para recolectar los datos se utilizaron las bases de datos Lilacs y Medline, utilizando la combinación de los descriptores "estrés" con "terapias complementarias", "medicamentos florales", "terapias mente-cuerpo", "terapias sensoriales a través de las artes", "terapia espiritual" y "terapias en estudio." Resultados: Se seleccionaron 48 artículos. Posteriormente se realizó por etapas la selección por los títulos de los artículos, evaluación de los resúmenes y el análisis y lectura de los textos completos. Del total, 07 artículos fueron analizados y construídas las categorías: Las PIC como tratamientos para reducir el estrés y otras enfermedades; El uso de las PIC en el tratamiento del estrés y La credibilidad en el uso de las PIC. Conclusiones: Las PIC vienen siendo utilizadas no sólo para reducir el estrés, sino también en la reducción de los niveles de ansiedad, síntomas depresivos, alivio del dolor y en la compulsión alimentaria. Las principales PIC mencionado fueron: MSBR, CBRS, Kouk Sun Do, la acupuntura, la meditación, respiración controlada, el yoga y la terapia de polaridad. Citadas como las principales razones de su uso: el alcance del bienestar espiritual, serenidad, mejor estado de ánimo, la compasión por los demás y para mejorar el sueño (AU)


Objetivo: sintetizar a produção do conhecimento relacionada à utilização das Práticas Integrativas e Complementares (PIC) no tratamento de estresse ocupacional. Método: revisão integrativa, que teve como questão norteadora: qual é a produção do conhecimento relacionada à utilização das PIC no tratamento de estresse ocupacional no período 2003 a 2013?. Para a coleta de dados utilizou-se as bases de dados: Lilacs e Medline, utilizando a combinação dos descritores "estresse" com "terapias complementares", "medicamentos florais", "terapias mente-corpo", "terapias sensoriais através das artes", "terapias espirituais" e "terapias em estudo". Resultados: 48 artigos foram selecionados. Posteriormente realizada, por etapas, a triagem dos títulos dos artigos, a avaliação dos resumos e a leitura e avaliação dos textos completos. Do total, 07 artigos foram analisados e construídas as categorias: As PIC como terapias redutoras de estresse e outras doenças; O uso das PIC no tratamento de estresse ocupacional e a Credibilidade no uso das PIC. Conclusões: as práticas integrativas complementares vêm sendo utilizadas não só na redução do estresse, mas também na redução dos níveis de ansiedade, de sintomas depressivos, do alivio da dor e da compulsão alimentar. As principais PIC mencionadas foram: MSBR, CBRS, Kouk Sun Do, acupuntura, meditação, respiração controlada, yoga, e a terapia da polaridade. Foram citadas como principais motivos para sua utilização: o alcance do bem-estar espiritual, da serenidade, da melhora do humor, da compaixão pelo próximo e da melhoria do sono


Objective: To synthesize the knowledge production related to the use of Integrative and Complementary Practices (PIC) in the treatment of occupational stress. Method: Integrative Review, which had as its guiding question: what is the production of knowledge related to the use of PIC in the treatment of occupational stress published between 2003 and 2013?. The data was collected in the Lilacs and Medline databases using combinations of the following descriptors: "stress" with "complementary therapies", "floral medicines", "mind-body therapies", "sensory therapies through arts", "spiritual therapies, and "therapies under study". Results: 48 articles were selected. The sorting of titles, summaries, reading assessment, and evaluation of complete texts were subsequently performed in stages. Out of all selected articles, 07 articles were analyzed and the following categories built: PIC as stress reducing therapies and other diseases; the use of PIC in the treatment of occupational stress; and Credibility in the use of PIC. Conclusions: The integrative complementary practices have been used not only to reduce stress but also in reducing levels of anxiety and depressive symptoms, and relief of pain and binge eating. The main PIC mentioned were: MSBR, CBRS, Kouk Sun Do, acupuncture, meditation, controlled breathing, yoga, and polarity therapy. The major cited reasons for its use were: reaching spiritual well-being and serenity, mood and sleep improvement, and compassion for others (AU)

Conhecer a opinião dos enfermeiros sobre a realização do Relatório de Enfermagem na alta hospitalar. Métodos Um estudo sociológico-exploratório e transversal incidental em uma amostra de conveniência de 400 enfermeiros em hospitais na Região de Múrcia (Espanha) através da aplicação de um questionário. Resultados Foram encontradas associações significativas entre as variáveis unidade de trabalho e redação do relatório de enfermagem (p= 0,000) e entre educação continuada sobre este (p= 0,027). O mesmo se aplicou às variáveis centro de saúde (p= 0,001) e educação universitária (p = 0,049). Conclusão Os enfermeiros afirmam não estarem preparados teoricamente nem metodologicamente para enfrentar a redação do Relatório de Enfermagem na alta hospitalar...


To know the opinion of nurses report on the realization of nursing report at hospital discharge. Methods A sociological study - exploratory and cross-sectional incidental in a convenience sample of 400 nurses in the hospitals of the Region of Murcia (Spain), by applying a questionnaire. Results Significant associations between the variable unit of work and writing of the paper (p= 0.000) and continuing education about it (p= 0.027) was obtained. The same applies to the health center variable (p= 0.001) and higher academic education (p = 0.049).
