ABSTRACT

BACKGROUND: Consecutive droughts and quantitative and qualitative reduction of surface and underground water resources have caused an increase in greenhouse and hydroponic cultivation for most garden crops, including strawberries, in Iran. On the other hand, most of the inputs of greenhouse crops in Iran are imported. To possibility of replacing vermicompost with peat moss under hydroponic cultivation, an experiment was done in a split plot based on randomized complete blocks design in three replications in Isfahan (Iran) Agricultural and Natural Resources Research Center in 2019. The main treatment was substrate at four levels included different levels of vermicompost (30 and 50%) and peat moss (30 and 50%) in combination with perlite and sub-treatment were Selva and Camarosa cultivars. RESULTS: The results showed that Camarosa cultivar and Selva cultivar in (perlite/ peat moss 50:50) and Selva cultivar in (perlite / vermicompost 70:30) had maximum yield. Leaf number and chlorophyll index were maximum in Camarosa cultivar in peat moss substrates. Strawberry cultivars had the highest root fresh weight, the content of vitamin C and total soluble solids (TSS) in substrates containing vermicompost. Camarosa cultivar in (perlite / peat moss50:50) and Selva cultivar in (perlite /vermicompost 50:50) had maximum root dry weight. Also, the highest number of inflorescences was related to substrates containing peat moss and (perlite /vermicompost 70:30). Maximum amount of fresh and dry weight of shoots were observed in (perlite/ peat moss70:30). Selva cultivar had more inflorescences (16.5%) than Camarosa cultivar and Camarosa cultivar produced more fresh and dry weight of shoots (16.5%, 23.01%) than Selva cultivar. CONCLUSION: Expriment results highlighted the importance of considering both main and sub-treatments in agricultural research, as they interacted to influence various growth and yield parameters. 50% vermicompost treatment combined with perlite had a positive impact on plant growth and in quality index such as vitamin C content and TSS was highest. while the choice of cultivar affected different aspects of plant development. Selva cultivar was known to be more tolerant to salinity caused by vermicompost. Vermicompost is local and more economical, also salt resistant cultivars are recommended in a controlled (30%) amount of vermicompost.

ABSTRACT

PURPOSE: There is a growing body of evidence suggesting that patients with a diagnosis of depression suffer worse outcomes after total joint arthroplasty (TJA) procedures. It is unclear whether depression treatment is a modifiable risk factor that can be targeted to improve suboptimal outcomes. We conducted a systematic review to understand the role that various interventions have on outcomes of TJA in patients with diagnosed depression (PDDs). METHODS: PubMed, Ovid MEDLINE, Scopus, and EMBASE were searched systematically from inception until November 2022. Studies of PDDs who underwent TJA that compared any intervention/treatment of depression with a control group and reported pain, functional outcomes, depression scores, and/or revision rates after TJA were relevant for this review. RESULTS: Ten relevant studies were included in the final systematic review, with a total of 33,501 patients included. Two studies reported lower revision rates for patients receiving selective serotonin reuptake inhibitor treatment and psychotherapy. Two studies showed no difference in functional outcomes for patients receiving pharmacologic treatment. One study reported improved functional outcomes for patients receiving cognitive behavioral therapy and another reported improved postoperative functional outcomes for patients receiving alprazolam. CONCLUSION: Interventions targeted at PDDs may improve short-term pain and functional outcomes, and there may be an association between selective serotonin reuptake inhibitor use and implant survival. The current literature is limited and inconclusive, with important gaps in understanding regarding the optimization and treatment of this modifiable risk factor. Surgeons should consider depression treatment as a method to improve outcomes in this cohort.

ABSTRACT

Mesenchymal tumors harboring GLI1 gene fusions are a rare new entity that typically occur in the head and neck region of young to middle aged adults, with a particular predilection for the tongue. We report herein a case of epithelioid mesenchymal tumor with PTCH1-GLI1 gene fusion of the right submental region in an 82-year-old male never smoker. Ultrasound-guided fine needle aspiration (FNA) with concomitant core needle biopsy was performed. Cytology smears revealed a hypercellular, monotonous aspirate comprised of epithelioid to plasmacytoid cells with round regular nuclei and moderate amounts of cytoplasm. There were admixed granulomata. The patient underwent surgical resection with limited neck dissection and subsequent pathologic examination with performed next generation sequencing confirmed the presence of epithelioid mesenchymal tumor with PTCH1-GLI1 gene fusion. To our knowledge, this is the first reported example of a mesenchymal tumor harboring GLI1 gene fusion initially evaluated by FNA.

ABSTRACT

CONTEXT.­: Appendiceal goblet cell adenocarcinoma is an uncommon appendiceal neoplasm that is typically diagnosed incidentally to a presentation of acute appendicitis. Over the years, there have been many changes to the histologic evaluation and classification of this neoplasm, culminating in the most recent (2019) World Health Organization recommendations. OBJECTIVE.­: To understand the evolution of the concept of "goblet cell" neoplasia in the appendix, we explore a series of cases of goblet cell adenocarcinoma encountered at a county hospital over a 20-year period. We performed a historically informed review of these cases to highlight the significance and blind spots of our current definition and approach to this disease entity. DATA SOURCES.­: The cases were recovered from the archives of a county hospital (1999-2019). The literature was assembled through PubMed searches on the various names given to "goblet cell" neoplasms since the 1970s. We also refer to standard reference books of gastrointestinal pathology. CONCLUSIONS.­: Frequent and numerous changes in the nomenclature of "goblet cell" lesions of the appendix resulted in considerable confusion regarding the approach to this rare entity, especially pertaining to its grade, as well as to its management in the clinical setting. The newest approach, recommended by the World Health Organization, provides clarity to its pathologic and clinical management. Adherence to these recommendations will improve communication between pathologists, surgeons, and oncologists about the natural history and prognosis of this malignant neoplasm.

ABSTRACT

AIMS AND OBJECTIVE: In this work, the performance of a sodalite membrane reactor (MR) in the conversion of methanol to olefins (MTO process) was evaluated for ethylene and propylene production with in situ steam removal using 3-dimensional CFD (computational fluid dynamic) technique. METHODS: Numerical simulation was performed using the commercial CFD package COMSOL Multiphysics 5.3. The finite element method was used to solve the governing equations in the 3- dimensional CFD model for the present work. In the sodalite MR model, a commercial SAPO-34 catalyst in the reaction zone was considered. The influence of key operation parameters, including pressure and temperature on methanol conversion, water recovery, and yields of ethylene, propylene, and water was studied to evaluate the performance of sodalite MR. RESULTS: The local information of component concentration for methanol, ethylene, propylene, and water was obtained by the proposed CFD model. Literature data were applied to validate model results, and a good agreement was attained between the experimental data and predicted results using CFD model. Permeation flux through the sodalite membrane was increased by an increase of reaction temperature, which led to the enhancement of water stream recovered in the permeate side. CONCLUSION: The CFD modeling results showed that the sodalite MR in the MTO process had higher performance in methanol conversion compared to the fixed-bed reactor (methanol conversion of 97% and 89% at 733 K for sodalite MR and fixed-bed reactor, respectively).

ABSTRACT

Non-neural granular cell tumor was first described in 1991 as an unusual primitive, polypoid variant of the conventional granular cell tumor. To date, this neoplasm remains a rare entity and the cell of origin is uncertain. While the histological features are similar to the conventional granular cell tumor, it represents a distinct entity that is negative for S100 and lacks true nerve sheath differentiation. Here, we describe a case of a 4-year-old male who presented with a painless, soft nodule on his right chest wall that was slowly increasing in size. The mass was excised and sent for pathologic analysis. Microscopic examination reveals spindle and epithelioid cells with vesicular nuclei and prominent granular eosinophilic cytoplasm. Immunohistochemical analysis shows negative staining for S100 and AE1/AE3/PCK26 but is positive for CD68. A diagnosis of a non-neural granular cell tumor was made. We report a rare and diagnostically challenging case in a pediatric patient.

ABSTRACT

Epidermotropic metastasis of head and neck squamous cell carcinoma is rare. We report a case of a 56-year-old white man with a history of oral squamous cell carcinoma who presented with multiple papules on the face, eyelid, and chest wall. The histopathologic examination of the skin lesions revealed nests of malignant cells with basaloid features and zones of cytoplasmic vacuolization predominantly within the dermis but focally involving the epidermis. Central necrosis, focal papilla formation, and lymph-vascular invasion were also seen. Two of these lesions, each read by different dermatopathologists, were initially diagnosed as sebaceous carcinoma. However, further evaluation, including adipophilin immunohistochemistry, confirmed the metastatic nature of the skin lesions. Cytoplasmic vacuolization in metastatic deposits of squamous cell carcinoma may mimic primary sebaceous carcinoma. This presentation highlights the need to consider the possibility of a metastatic deposit when making a diagnosis of primary sebaceous carcinoma.

ABSTRACT Objective Diabetes affects the erectile function significantly. However, the penile alterations in the early stage of diabetes in experimental animal models have not been well studied. We examined the changes of the penis and its main erectile components in diabetic rats. Materials and methods Male Sprague-Dawley rats were divided into 2 groups: streptozotocin (STZ)-induced diabetics and age-matched controls. Three or nine weeks after diabetes induction, the penis was removed for immunohistochemical staining of smooth muscle and neuronal nitric oxide synthase (nNOS) in midshaft penile tissues. The cross-sectional areas of the whole midshaft penis and the corpora cavernosa were quantified. The smooth muscle in the corpora cavernosa and nNOS in the dorsal nerves were quantified. Results The weight, but not the length, of the penis was lower in diabetics. The cross-sectional areas of the total midshaft penis and the corpora cavernosa were lower in diabetic rats compared with controls 9 weeks, but not 3 weeks after diabetes induction. The cross-sectional area of smooth muscle in the corpora cavernosa as percentage of the overall area of the corpora cavernosa was lower in diabetic rats than in controls 9 weeks, but not 3 weeks after diabetes induction. Percentage change of nNOS in dorsal nerves was similar at 3 weeks, and has a decreased trend at 9 weeks in diabetic rats compared with controls. Conclusions Diabetes causes temporal alterations in the penis, and the significant changes in STZ rat model begin 3-9 weeks after induction. Further studies on the reversibility of the observed changes are warranted.
