ABSTRACT

Purpose The aim of the current study was to assess and compare Doppler ultrasound findings, especially the resistivity index (RI), among and between patients with vertebral artery hypoplasia (VAH) and normal populations. Material and methods Fifteen consecutive patients with VAH (mean age 54 ± 21 years) and 15 sex-matched controls without VAH (mean age 54 ± 22 years) were selected for the study. The vertebral arteries (VA) were examined with Doppler ultrasound. We also explored each group for sex and age differences (young: age ≤ 50, old: age >50). Results The mean RI (MRI), right RI (RRI), left RI (LRI), non-dominant-side RI, and dominant-side RI were significantly higher in the Case Group than the Control Group. In the Case Group, the affected-side RI (A.RI) was significantly higher than the normal side, while the normal side peak systolic velocity was significantly higher than the affected side. The MRI and A.RI were significantly higher in older patients. We also found a significant negative correlation between the mean diameter (MD) and MRI. MRI and A.RI both correlated positively with age in the Case Group, while left peak systolic velocity decreased significantly with age in the Control Group [p-values < 0.05]. Conclusion The dominant VA had a higher RI in the Case Group than the Control Group. It can therefore be inferred that the dominant VA in patients with VAH does not work completely normally, thus making these patients even more susceptible to vertebrobasilar insufficiency and possible strokes.

ABSTRACT

Nonalcoholic fatty liver disease (NAFLD) is one of the most common noncommunicable diseases worldwide. The present study aimed to investigate the effects of oleoylethanolamide (OEA) supplementation combined with calorie restriction on inflammation, body composition, and hepatic fibrosis among obese patients with NAFLD. In this 12-week randomized clinical trial, 76 obese patients newly diagnosed with NAFLD were randomly allocated into either OEA or placebo group. The weight-loss diet was also designed for both groups. Pre- and postintervention messenger RNA expression levels of the transcription factor nuclear factor-κB (NF-κB), interleukin-6 (IL-6) and IL-10, body composition, and NAFLD fibrosis score were assessed. At the end of the study, the OEA group showed lower NF-κB and IL-6 expression levels compared to the placebo (p < .01). However, IL-10 expression level was approximately twofold higher in the OEA group compared to the placebo group (p = .008). A significant reduction was observed in the fat mass of the OEA group compared to the placebo (p = .044) postintervention. In addition, OEA supplementation led to a significant increase in fat-free mass in the OEA group compared to the placebo (p = .032). A remarkable increase was observed in resting metabolic rate (RMR) in the OEA group (p = .009); however, it was not found in the placebo group. There were no significant between-group differences in RMR postintervention. In addition, no significant within-and between-group differences were observed in the NAFLD fibrosis score at the end of the trial. Treatment with OEA along with weight-loss intervention could significantly improve inflammation and body composition in patients with NAFLD.

ABSTRACT

Introduction: The aim of this study was to evaluate the mechanism and type of upper limb arterial trauma in Iranian population. Methods: Fifty-one patients with upper limb trauma were evaluated over a 4-year period with conventional angiography at the Tabriz Imam Hospital, Iran. Results: Twenty-four patients (19 men, 5 women with a mean age of 27.5 ± 11.8 years) had arterial injuries. Blunt trauma was more frequent than penetrating trauma (87.5%). The most cause of trauma was traffic accidents and the brachial artery was the most frequently affected artery. In 87.5% cases associated bone injuries were observed. Conclusion: Patients with blunt upper limb injuries must be evaluated for vascular integrity timely, especially in traffic accidents because vascular injuries after traffic accidents need to be referred to vascular centers. The results of this article are of potential use and clinical importance because precise diagnosis of vascular insults are essential to restore injured extremities.

ABSTRACT

At the present time, imaging guided renal biopsy is used to provide diagnoses in most types of primary and secondary renal diseases. It has been claimed that renal biopsy can provide a link between diagnosis of renal disease and its pathological conditions. However, sometimes there is a considerable mismatch between patient renal outcome and pathological findings in renal biopsy. This is the time to address some new diagnostic methods to resolve the insufficiency of conventional percutaneous guided renal biopsy. Nanotechnology is still in its infancy in renal imaging; however, it seems that it is the next step in renal biopsy, providing solutions to the limitations of conventional modalities.

Background The intrarenal resistive index (RI) is a promising tool for predicting renal graft outcomes. Due to the complexity of graft function and the presence of diverse contributing factors, however, the available data are inconclusive. Purpose To examine the performance of the RI 1 week after transplant in predicting allograft function at week 12, with an emphasis on the type of intrarenal artery used in the examination. Material and Methods A total of 58 first-time living-donor kidney-allograft-transplantation patients aged less than 50 years underwent Doppler ultrasonography (US) of the intrarenal arteries 1 week after engraftment. The RI was calculated for both the segmental (RI-C) and interlobar-arcuate (RI-P) arteries. Serum creatinine level and the creatinine reduction ratio (CRR) were determined at weeks 1 and 12 post transplantation. Results While the RI did not correlate with serum creatinine level and CRR at week 1, significant correlations were present between the RI and serum creatinine level (r = 0.28, P = 0.03 for both RI-C and RI-P) and CRR (r = -0.25, P = 0.05 for both RI-C and RI-P) at week 12. The sensitivity, specificity, positive predictive value, and negative predictive value of using RI-C in predicting abnormal 12-week serum creatinine level were 51.2%, 52.9%, 72.4%, and 31.3%, respectively; and 53.7%, 47.1%, 70.9%, and 29.6% for RI-P, respectively ( P > 0.99 for all comparisons). Conclusion Early post-transplantation RI correlates significantly with both serum creatinine level and creatinine reduction ratio 12 weeks after engraftment with intermediate predictive accuracy.

ABSTRACT

INTRODUCTION: Sesamoids are ovoid bones with variable size and shape in the course of tendons, can be found in different parts of skeletal system. CASE PRESENTATION: We report a case of 61-year-old woman in whom we observed a large accessory bone located in the anterior aspect of the left ankle joint. Since such accessory bones are found very infrequently, their presence may cause some diagnostic confusion. CONCLUSION: Regarding complaints in foot area one has to be familiar with such bones in order to make a correct diagnosis.
