ABSTRACT

The aim of the present study was to compare the sensitivity between Alvarado and RIPASA score for the diagnosis of acute appendicitis patients. A comparative study was conducted in the Dhaka National Medical College Hospital, Bangladesh from July 2015 to December 2015. The study was approved by the institutional ethical committee. Most of the appendicitis patients belonged to the between 21-30 years which was 64(32%). Male appendicitis patients (52%) are more than the female patients (48%). Majority of the patients (69%) complains pain occurs in the Right iliac fossa. Sensitivity of Alvarado scores was 81.60%, specificity 74.58%, accuracy 79.35%, positive and negative predictive values were 87.18% and 65.67% respectively. RIPASA score's sensitivity was 85.6%, specificity 69.49%, accuracy 80.40%, positive and negative predictive values were 85.60% and 85.60% respectively. This study reported that RIPASA score is significantly shown higher diagnostic accuracy then that Alvarado score.

ABSTRACT

The aim of this study is to asses the result of fenestration and discectomy for the treatment of PLID. This Prospective quasi experimental study was conducted on 29 Patient of PLID with different ages at Dhaka Medical College Hospital (DMCH), Dhaka, Bangladesh from July 2014 to June 2016. Post Operative outcome was assessed through Modified Macnab Criteria & Visual Analogue Score (VAS). The mean age of patient were 38.14±9.20 years and ranging from 24 to 55 years. The mean age of male was 37.21±9.72 years and female patient was 39.90±8.32 years. Overall subjective assessment of this study revealed that 79.2% patients had excellent functional outcome, 13.8% good and 6.9% fair and there was no poor functional outcome a according to modified Macnab criteria.

ABSTRACT

Migration behavior of radionuclide is one of the most important factors to be considered for the long-term safety assessment of a radioactive waste disposal facility in a wet geological formation. In the present study, laboratory based column experiments have been carried out to assess the radionuclide migration behavior of ¹³7Cs and 6°Co and to evaluate the retardation factor through a clay soil layers using gamma spectrometry. Investigation was performed for a range of particle sizes and fixed column lengths to determine an appropriate value of migration rate of ¹³7Cs and 6°Co. The distribution pattern of particle size in soil samples were measured by sieved method. Two different particle sizes (≤ 90 µm and mixed size) were used in the column experiments. The migration rate in the clay type soil layer of particle size ≤ 90 µm was found by the order of 6°Co > ¹³7Cs. The maximum migration length of 6°Co in the soil layer was found to be 0-25 cm, however in the case of ¹³7Cs it was found at a maximum length of 0-10 cm. The distribution coefficient of 6°Co was found nearly same as that of ¹³7Cs. The retardation factor was found to be 1.79 and 1.94 for 6°Co and ¹³7Cs, respectively. The experimental breakthrough from this study indicates that the amount of radioactive cesium and cobalt released depends upon the composition of the soils.

ABSTRACT

A 56 years old farmer from Churkhai, Mymensingh was admitted in Cardiology unit of Mymensingh Medical College Hospital 24 October, 2004 with the complaints of progressive breathlessness on exertion with the repeated respiratory tract infection. He had 3 episodes of multiple large joints swelling involving knee, ankle, wrist, during his childhood with spontaneous recovery without any residual deformity. At the age of 45 years, he was incidentally diagnosed as enlarged heart by a medical board, when applied for Foreign Service. In cardiology unit he was diagnosed as a case of Lutembacher's syndrome on the basis of history, physical examination and it was confirmed by X-Ray, ECG and Echocardiography study. As the patient developed pulmonary hypertension with calcified mitral valve leaflet so percutaneous transseptal mitral commissurotomy and or surgery is not indicated. So the patient was managed by medical therapy alone.
