ABSTRACT

AIM: To analyze activity of chymotrypsin-like plasma proteinases (CTP) in patients with various cardiovascular diseases. MATERIAL AND METHODS: CTP activity was studied in 82 patients with various cardiovascular diseases: 13 coronary heart disease patients with normal arterial pressure, 49 patients with essential hypertension (EH) and secondary arterial hypertension, 20 patients with nonspecific aortic arteritis. 28 donors served control. RESULTS: CTP activity in plasma of patients with EH rose 4 times compared to donors. If EH patients had concurrent diseases (CHD, chronic pyelonephritis, atherosclerosis of extracranial arteries), CTP activity may increase by 30-300%. In patients with nonspecific aortic arteritis CTP activity in blood plasma is 17.5 times higher than in donors. CONCLUSION: High CTP activity in cardiovascular patients may be explained by chymase and cathepsin G release into blood flow indicating activation of alternative to ACE pathway of angiotensin II production or the presence of the inflammatory process.

ABSTRACT

An examination was made of 22 hypertensive patients with clinical and echoCG symptoms of left ventricular hypertrophy. All the patients underwent coronary angiography, transvenous multiphase left ventriculography and acute captopril (11 patients), nifedipine (11 patients) tests. A single oral 25 mg dose of captopril increased energetic efficacy of cardiocycle, restored diastolic function of the left ventricle, aroused myocardial contractility.

ABSTRACT

In 181 patients with arterial hypertension of various genesis clinical-laboratory data were compared with those obtained by computed tomography of kidneys and adrenal glands for etiology verification. Computer tomography is proved to be a valuable noninvasive method for elucidation of retroperitoneal space organs pathology which can cause arterial hypertension. Hormonal-active tumors are better identified when patients are purposely selected according to the catecholamine and VMA levels as well as the state of renin-angiotensin-aldosterone system. Computer tomography allowed determination of alterations in kidneys including those not identifiable by other methods (small cysts in kidneys, tumors, concrements).

ABSTRACT

It was found that hypertension in patients with stenosis of the renal artery is of a renopressor character which is maintained by increased renin secretion by the kidney with the stenosed artery. High arterial hypertension is conducive to the maintenance of the function of the kidney with the stenosed artery at the most optimum level and has an effect on the contralateral kidney. After surgery (nephrectomy), in addition to normalization of arterial pressure the blood flow in the remaining kidney becomes almost equal to the blood flow in two kidneys, i.e. the remaining kidney compensates for the function of the removed kidney. The incidence of malignant course of arterial hypertension in combination of an occlusive lesion of the renal artery and pyelonephritis of this kidney is twice that in isolated stenotic lesion of the renal artery.

ABSTRACT

The article discusses the results of the examination of 40 patients with symptomatic renal hypertension (in 15 of whom arterial hypertension followed a malignant course) and 19 patients with IIB-IIIA stages of hypertensive disease (6 of whom had the malignant form). Marked activation of the renin-angiotensin system along with reduced renal blood flow was found in patients with symptomatic renal hypertension. Despite the reduced renal blood flow, patients with the malignant form of hypertensive disease had diminished activity and secretion of renin as well as a considerably increased prostaglandin F2 alpha content and reduced sodium content in the plasma of renal venous and, particularly in the plasma of aortic blood.
