ABSTRACT
Results of treatment of 238 patients, suffering an acute gastrointestinal hemorrhage, occurring on background of application of medicinal preparations, were adduced. The incidence rate for an acute medicinal ulcers of foregut was analyzed, as well as possibilities for their treatment.

The possibility of intraarterial access for infusion in patients with severe acute pancreatitis (AP) (initial severity score using the APACHE II assessment scale 16 points) is discussed. The results of the intraarterial crystalloid and colloid solutions infusion and its influence into the state of systemic hemodynamic in patients with organ dysfunction are demonstrated. Positive influence of the proposed method for recovering of central hemodynamic parameters in patients with cardiac and/or respiratory dysfunction and the outcome of early phases of AP were ascertained.

The results of the affected esophagus substitution, using a large intestine segment, in postburn stenosis in 42 patients were analyzed. Complete cicatricial esophageal impassability, impossibility or dangerous bougienage, rapid occurrence of the stricture recurrence, anamnesis data about perforation occurrence, presence of esophageal fistula have constituted the indications for the intervention performance. The best results were achieved while application of isoperistaltic retrosternal coloesophagoplasty, using left half of large intestine. The early postoperative complication was partial insufficiency of the neck anastomosis sutures and the late one--formation of its cicatricial stricture. Three patients died (postoperative lethality was 7.1%).

The results of treatment of 334 patients, operated on for cholelithic disease, were analyzed. In 164 of them intraoperative transillumination (IT) was applied and in 170--the generally accepted methods of the hepatoduodenal ligament content exposure. There was established the expedience of the IT application for the extrahepatic biliary ducts (EBD) and vessels verification as well as for the choledocholithiasis diagnosis during performance of laparoscopic and open cholecystectomy. Application of IT and the laparoscopic cholecystectomy method proposed have ad permitted to reduce trustworthy the frequency of the EBD and the vessels iatrogenic injury.

The methods of diagnosis and operative treatment of internal (in 19 patients) and external (in 18) biliary fistulas, which appeared due to destructive and complicated forms of cholecystitis were analyzed.

Results of treatment of 48 patients with postoperative peritonitis using programmed relaparotomy are adduced. Indications for application of the method are determined. New antiseptic agent "Palisan" was used for the abdominal cavity sanation. Analysis of complication is conducted. Lethality was 18.75%.

Treatment of postoperative peritonitis includes correction of enteral insufficiency, application of programmed relaparotomy, sanation and drain of abdominal cavity, antibiotic therapy, methods of detoxication. Complex approach promotes improvement of results treatment and diminution of postoperative lethality.
