ABSTRACT

The development of the system of informational analytical support based on expertise data is one of most important stage of increasing of effectiveness of management of regional health care. The study was organized to substantiate formation of the system of informational analytical support of management of regional health care on the basis of expertise data. The study was carried out on the basis of expertise data from subjects involved in informational analytical support of management of regional health care (health care management executives, chief specialists and directors of medical organizations in the subjects of the Russian Federation situated in the Siberian federal okrug). The study established that alongside with statistical information the expertise is enough important, objective and informative information to be applied in developing of managerial decisions. The highest integral estimated value of importance, objectiveness and informativeness has the information concerning competence of medical personnel, proportions of medical care of population and conditions of material technical base of health institutions. The most foreground issues concerning expertise are population health condition, pharmaceutical and medical equipment support of medical institutions, level and quality of population medical care. The degree of impact of expertise information on managerial decision making is highest in such directions as support of population with medical care, increasing of availability of medical care and degree of organization of medical care rendering. The probability of increasing of degree of impact of expertise information on managerial decision making is the highest in such directions as population provision with medical care, competence of medical personnel, level and quality of medical care, level of organization of medical care support, that is to be considered during implementation of expertise. The study data was used in developing the major principles of elaboration of system of informational analytical support of management of regional health care. The unified expertise questionnaire was developed to be used as a document basis of expertise system. The study results can be implemented in practical activities of regional health administration and medical organizations. The results of study made it possible to substantiate the development of the system of informational analytical support of regional health care and to determine priority directions of its formation.

ABSTRACT

The most problematic spheres in the resource support of emergency medical care to rural residents are the completeness of staff of physicians in rural medical surgeries, community hospitals and departments of emergency medical care in central district hospitals. The provision of feldsher obstetrics posts with sanitary motor transport and medical equipment is yet another problematic sphere. The main troubles during provision of emergency medical care at feldsher obstetrics posts are related to surgery treatment. The organization of emergency and urgent medical care suffers of many unresolved problems related to informational program support at feldsher obstetrics posts, polyclinics of central district hospitals.

ABSTRACT

The results of complex treatment analysis in 147 patients with acute destructive pancreatitis from 2006 to 2012 was made. In membrane plasmapheresis application the reduction of pain and improvement of general condition observed rapidly. By the end of the second day the activity of amylase decreased by 53% urea and creatinine level--by 62 and 56% compared with baseline. Decreasing of the necrotic suppurative complications rate in the main group--15.4%, in control--32.8%, mortality--accordinty 14.1 and 27.6%.

ABSTRACT

The most problematic indicators in functioning of emergency medical care are relatively preventable mortality of population outside medical institutions and relatively preventable lethality in hospital. The fundamentally low influence on survival of patients and amelioration of results of treatment is made by organization of emergency medical care at feldsher obstetrics medical unit and acute and urgent medical care in central district hospitals.

ABSTRACT

THE PURPOSE OF RESEARCH: studying of features of clinical current, diagnostics, treatment chronic opisthorchiasis at persons of elderly and senile age. 117 patients are surveyed. In bile in 100% of cases, a sladg-syndrome in 66.6%, infringement of motor function of a bilious bubble in 92.3%, macrobiocenosis in 98.3%, an allergic syndrome--microscopic changes are found out in 93%. The diagnostic importance of triad Paltseva at observable patients is confirmed. It is shown, that chronic [Russian characters: see text] is system disease at which leading value the pathology hepatobiliar and pancreatic system has.

ABSTRACT

In 61 patients pancreaticoduodenal resection (PDR) was performed: for pancreatic gland head cancer-in 29, periampullar zone cancer--in 20, chronic pseudotumoral pancreatitis--in 12. In 10 patients, suffering pancreatic gland and periampullar zone cancer, complicated by solitary hepatic metastases, PDR was added by cryodestruction of metastases. Cryoaffection on pancreatic gland stump during PDR performance have resulted in lowering of early postoperative complications frequency (pancreatic fistula, parapancreatic septic inflammation and an acute pancreatitis) and of chronic pancreatitis acute phase evolvement risk in the late postoperative period. Cryodestruction of hepatic metastases have promoted the patients survival median increase.

ABSTRACT

Non-alcoholic fatty liver disease, including non-alcoholic steatohepatosis (NSH) and non-alcoholic steatohepatitis (NASH) is considered to be a wide spread disease. Such reasons as metabolic, toxic, infections, alimentary and cryptogenic cause this disease. Pathogenesis of the disease is complex. If the necessary medical preventive measures are absent the disease develops as follows, first steatosis, then steatohepatitis, fibrosis, liver cirrhosis, hepatocarcinoma. The aim of the investigation was to study influence of Metadoxil in patients with NSH and NASH. The conducted investigation have shown high efficiency of the drug at combined treatment of a patient.
