ABSTRACT

The notion "venous thromboembolism" combines two processes: thrombosis of deep veins and pulmonary artery thromboembolism. The necessity in long-term adequate anticoagulant therapy as the basic treatment of patients with deep vain thrombosis does not give rise to doubts, it should be started prior to instrumental verification of the diagnosis. The past decade was marked by the appearance on the pharmaceutical market of a new oral anticoagulant: selective blocker of the Xa factor of blood coagulation system - rivaroxaban. According to the data of large-scale randomized clinical trials, the efficacy and safety of rivaroxaban are comparable to those of traditional conventional anticoagulant therapy (non-fractionated/low-molecular-weight heparin NFH/LMW + warfarin). Indisputable advantages of new agents are as follows: the lack of the necessity in laboratory monitoring, no interaction with foodstuff, minimal interaction with other drugs, minimal risk for the development of haemorrhage, and simplicity of dosing.

ABSTRACT

Three selected alkali-based organosolv pulps (alkali-sulfite-anthraquinone-methanol (ASAM), alkali-anthraquinone-methanol (organocell) and ethanol-soda) from agrofibre crop giant reed (Arundo donax L.) were bleached by an ozone-based TCF (totally chlorine- free) bleaching sequence AZE(R)QP (where A is an acidic pulp pre-treatment, Z is an ozone stage, (E(R)) is an alkaline extraction in the presence of reducing agent, Q is a pulp chelating, P is a hydrogen peroxide stage) without oxygen pre-bleaching, and compared with a conventional kraft pulp used as a reference. The different response on bleaching conditions within each bleaching stage was noted for all tested pulps. The pulp bleachability, in terms of brightness improvement or lignin removal per unit of applied chemicals, was found higher for the organocell pulp. The ASAM and ethanol-soda pulps showed the highest bleaching selectivity, expressed by viscosity loss per unit of lignin removed or brightness improved. The overall bleaching results of organosolv pulps were superior to kraft.

ABSTRACT

Role of venous reflux in evolution and prediction of acute varicotrombophlebitis (AVTP) has been evaluated with coloured duplex scanning performed as daily monitoring. It is demonstrated that venous reflux determines the direction and rate of thromb formation, it potential of embolism, promotes formation of floating thrombi. Study of venous reflux permits to predict the evolution of AVTP. It is one of the key elements which influences the thrombus formation and determines policy of treatment of varicose disease complicated with AVTP.

ABSTRACT

A comparative study on TCF (totally chlorine-free) bleachability of organosolv pulps from the annual fibre crop Arundo donax L. (giant reed) was carried out using a simple three-stage peroxide bleaching sequence without oxygen pre-bleaching. ASAM (alkali-sulfite-anthraquinone-methanol), Organocell (alkali-anthraquinone-methanol) and ethanol-soda organosolv pulps were bleached and compared with kraft pulp, as a reference. The final brightness of 76-78% ISO was attained for all tested pulps. The chemical charge required to reach this level of brightness varied for different pulps (despite the equal initial content of the residual lignin) and directly related to starting brightness values. No direct correlation between brightness improvement and lignin removal during bleaching was found, indicating the influence of the specific pulp properties introduced by pulping process on bleaching chemistry. The general higher bleaching response of organosolv pulps from A. donax was noted in comparison with kraft.

ABSTRACT

Problems of strategy and policy of surgical treatment of acute thrombophlebitis in the territory of saphena magna vein are discussed. Results of urgent radical combined venectomy in 76 patients are presented. A gas technique of subfascial dissection of perforant veins (SEPS) was used in 52 patients for the first time both for diagnosis and adequate treatment of thrombosed perforants. Spread of thrombosis from superficial veins to perforants was revealed in 14.3% patients with C3-C5 class of venous dysfunction (CEAP classification). Isolated and local forms of crus varicothrombophlebitis that must be urgently operated are distinguished. The mean hospital stay was 8.7 bed-days.

ABSTRACT

An unusual heteroxylan composed of galactosyl, 4-O-methyl-glucuronosyl and xylosyl residues with molar ratio 1:3:30 was isolated from the wood of Eucalyptus globulus Labill. The results of linkage analysis, supported by data of 1H, 2D 1H-1H COSY and 13C NMR spectroscopy, revealed that the polysaccharide is a (2-O-alpha-D-galactopyranosyl-4-O-methyl-alpha-D- glucurono)-D-xylan with a (1-->4)-linked beta-D-xylopyranosyl backbone branched at O-2 by short side chains composed of terminal 4-O-methyl-alpha-D-glucuronic acid and of 4-O-methyl-alpha-D-glucuronic acid substituted at O-2 with alpha-D-galactose.
