ABSTRACT

BACKGROUND: Distant metastasis from differentiated thyroid carcinoma at presentation is rare and isolated liver metastasis on presentation is almost unknown. We report a case of primary follicular carcinoma of the thyroid with isolated liver metastasis at presentation. CASE PRESENTATION: A 65-year-old man of Kashmiri origin presented to our tertiary referral center with obstructive jaundice; he was evaluated with magnetic resonance cholangiopancreatography and positron emission tomography-computed tomography. Positron emission tomography-computed tomography documented a lesion in his liver in addition to a metabolically active thyroid nodule. Fine needle aspiration cytology of the liver lesion supplemented with immunohistochemical analysis using thyroid transcription factor 1 confirmed the lesion as being an isolated metastasis from the primary thyroid lesion (which on fine needle aspiration cytology showed follicular architecture). CONCLUSIONS: To best of our knowledge, this is first reported case of primary differentiated thyroid carcinoma presenting with isolated liver metastasis manifesting as obstructive jaundice.

ABSTRACT

Neuroendocrine tumor (Merkel cell carcinoma-MCC) of the vulva is a very rare entity with less than 15 cases reported in the English literature. It is known for its aggressive behaviour and propensity for early dissemination. The actual cell of origin and etiology of this disease is controversial. In absence of any definite guidelines for management (due to its rarity), extrapolation of data from extra-vulvar MCC seems logical. We present a case of vulvar neuroendocrine tumor who presented at a locally advanced stage.

ABSTRACT

Buschke Lowenstein tumour or giant condyloma acuminata is a rare entity with only less then 50 cases reported in English literature so far. No such case has been reported from the Kashmir valley. They are considered as intermediate lesions between simple condyloma acuminata and invasive squamous cell carcinoma. A 57-year-old heterosexual male presented with a giant perianal condyloma. The lesion was surgically excised completely. Postoperatively patient was put on topical 5-FU ointment. Patient is recurrence free 6 months after surgery. The giant condyloma acuminate is an aggressive tumour with propensity for recurrance and malignant transformation. Surgical excision is the treatment of choice. One such rare case is discussed with review of literature.
