ABSTRACT

Objective: To understand the relationship between secondhand smoke exposure and dyslipidemia among adults in Beijing and to provide a scientific basis for relevant intervention. Methods: Data were from Beijing Adult Non-communicable and Chronic Diseases and Risk Factors Surveillance Program in 2017. A total of 13 240 respondents were selected by multistage cluster stratified sampling method. The monitoring contents include a questionnaire survey, physical measurement, collection of fasting venous blood, and determination of related biochemical indicators. SPSS 20.0 software was used for the chi-square test and multivariate logistic regression analysis. Results: The prevalence of total dyslipidemia (39.27%), hypertriglyceridemia (22.61%), and high LDL-C (6.03%) were the highest among those exposed to daily secondhand smoke. Among the male respondents, the prevalence of total dyslipidemia (44.42%) and hypertriglyceridemia (26.12%) were the highest among those exposed to secondhand smoke daily. Multivariate logistic regression analysis after adjustment for confounding factors showed that compared with no exposure to secondhand smoke, the population with an average exposure frequency of 1-3 days per week had the highest risk of total dyslipidemia (OR=1.276, 95%CI: 1.023-1.591). Among the patients with hypertriglyceridemia, those exposed to secondhand smoke daily had the highest risk (OR=1.356, 95%CI: 1.107-1.661). Among the male respondents, those exposed to secondhand smoke for 1-3 days per week had a higher risk of total dyslipidemia (OR=1.366, 95%CI: 1.019-1.831), and the highest risk of hypertriglyceridemia (OR=1.377, 95%CI: 1.058-1.793). There was no significant correlation between the frequency of secondhand smoke exposure and the risk of dyslipidemia among female respondents. Conclusions: Secondhand smoke exposure in Beijing adults, especially men, will increase the risk of total dyslipidemia, especially hyperlipidemia. Improving personal health awareness and minimizing or avoiding exposure to secondhand smoke is necessary.

ABSTRACT

Objective: To investigate the sodium intake, potassium intake, sodium- potassium ratio and influencing factors of family cooks and members in six regions of China. Methods: Using the multistage random sampling method, a total of 1 576 family cooks and their family members were recruited from 6 regions in China. Questionnaire survey and physical examination were conducted to collect basic information. 24 h urine was collected to test 24 h urinary sodium and potassium levels. Results: After excluding unqualified urine samples, a total of 1 530 people were included in the study. For all participants, 24 h urine sodium value was (4.39±1.93) g, 24 h urine potassium value was (1.59±0.62) g and sodium-potassium ratio was 5.02±2.18. 24 h urinary sodium was negatively correlated with age, female, junior high school education or above, and annual family income (P<0.05), and positively correlated with perceived salty taste, BMI and SBP (P<0.05). The 24 h urine potassium value was negatively correlated with eating out almost everyday (P<0.05), but positively correlated with BMI and unwillingness to reduct salt (P<0.05). The sodium-potassium ratio was negatively correlated with age, female and junior high school education or above annual family income (P<0.05), and positively correlated with perceived salty taste, SBP and eating out 3-5 d/week (P<0.05). Conclusions: The 24 h urinary sodium value and sodium potassium ratio of family cooks and family members in six regions of China are still at high level, while the 24 h urine potassium value is low, with multiple factors. Therefore, it is necessary to carry out long-term comprehensive salt reduction intervention for family cooks and their members.

ABSTRACT

Objective: To assess the relationship between body mass index (BMI) and death risk of cardiovascular disease (CVD) in rural male population. Methods: 22 282 men aged 40 years older in Tanghe county and Fenghuang county from the cohort of the "Prospective Study on Adult Behavior and Health Risk Factors in China" were selected as subjects of this study. Cox regression model was used to calculate the hazard ratios (HRs) of the death of CVD during the follow-up period with different BMI groups at baseline. Results: The average follow-up period in the two counties was (19.1±8.7) years and 10 828 (48.6%) people died during the follow-up period. 4 504 deaths were attributed to CVD. Among the deaths of CVD, 1 279 cases died of ischemic heart disease (IHD), ischemic stroke (IS) died in 1 201, cases died of died of 1 317 hemorrhagic stroke (HS), other 707 cases. Compared to population with BMI<18 kg/m(2), Cox regression model (adjusting factors of region, age, nationality, education level, occupation, smoking, drinking, blood pressure, blood pressure, etc.) showed that people with BMI between 20-22 kg/m(2) had the lowest risk of CVD death (HR=0.95, 95%CI: 0.83-1.09). But the difference was not statistically significant among each BMI group (P>0.05). The risk of IHD death was the lowest in the population with BMI between 20-22 kg/m(2) (P<0.05) (HR=0.64, 95%CI: 0.52-0.80). There was no statistically significant difference between the risk of IHD death in the population with BMI≥24 kg/m(2) and that in the population with BMI<18 kg/m(2) (P>0.05). There was no statistically significant difference between the risk of IS death and BMI (P>0.05). The death risk of HS in the population with BMI between 18-24 kg/m(2) was higher than that in the population with BMI<18 kg/m(2) (P<0.05). The death risk of the population with BMI between 26-28 kg/m(2) was the highest (HR=1.88, 95%CI:1.18-2.99). Conclusions: The mortality risk of CVD and IHD was the lowest in lean or normal weight group, and HS was higher in overweight group. Maintaining a reasonable weight can reduce the risk of death in patients with CVD.

ABSTRACT

Objective: To analyse the long-term efficacy in childhood T-cell acute lymphoblastic leukemia (T-ALL) cases enrolled in the national protocol of childhood leukemia in China-acute lymphoblastic leukemia (NPCLC-ALL) 2008. Methods: Clinical data of 96 patients diagnosed as T-ALL and treated with NPCLC-ALL2008 protocol between January 2009 and December 2017 in the Department of Hematology-Oncology, the Children's Hospital, Zhejiang University School of Medicine were analyzed retrospectively. Predictive value of minimal residual disease (MRD) monitored by flow cytometry was analyzed. Kaplan-Meier method was used for long-term survival analysis. Results: A total of 96 evaluable patients with newly diagnosed T-ALL were analysed, including 72 males and 24 females. The age was 9.5 (ranged from 1.0 to 16.0) years. The follow-up time was 5.7 (ranged from 1.0 to 9.7) years. Among 96 patients, 92 (96%) achieved complete remission. The 5-year event free survival (EFS) and overall survival (OS) rates were (61±6) % and (70±5) %, respectively. Relapse occurred in 18 cases and the 5-year cumulative incidence of relapse was (27±6) %. Twenty-four patients died. The 5-year OS rates of patients with MRD>5% on day 15 of induction therapy was significantly worse than those with MRD≤5% ((60±12) % vs. (72±6) %, χ(2)=3.904, P=0.048) . The 5-year EFS and OS rates were obviously lower in patients with MRD>10% before the consolidation therapy ((50±35) %). The 5-year OS rates of patients with relapsed disease was significantly worse than those without ((26±13) % vs. (81±5) %, χ(2)=18.411, P<0.01). The earlier the relapse, the worse the prognosis. The 5-year OS rates for patients relapsed within 6 months, within 3 years and more than 3 years, were (25±22) %, (30±14) % and (50±35) % respectively (χ(2)=13.207, P<0.01). Conclusions: NPCLC-ALL2008 protocol is effective for childhood T-ALL. The MRD guided accurate risk stratification and individualized treatment can reduce the relapse and improve the survival rate of pediatric T-ALL.

ABSTRACT

Objective: To investigate the effect of optimized preoperative dietary management on enhanced recovery in patient with consecutive operation of robot-assisted radical prostatectomy(RARP) at night. Methods: Forty patients undergoing consecutive operation of robot-assisted radical prostatectomy at night in the department of urology in our hospital from Jun 2018 to March 2019 were divided into two groups, 23 patients in the study group and 17 patients in the control group. The control group followed the traditional fasting program at 24∶00 p.m. the day before the surgery. In the study group, the preoperative fasting procedure was optimized. The fasting time, water deprivation time, intravenous infusion volume, scores of hunger and thirst, blood glucose level, length of postoperative hospital stay and adverse reactions were compared between two groups. The level of hunger and thirst were evaluated using the Likert score. Results: The preoperative fasting time and water deprivation time of the study group and the control group were (11.9±4.4 vs 19.3±4.8) h and (6.0±2.9 vs 19.3±4.8) h, respectively (P<0.01). The infusion volume of study group was (406.5±310.5) ml and that of control group (744.1±443.0) ml, the difference was statistically significant (P<0.01). The hunger and thirst scores of the study group were lower than those of the control group before surgery, and the postoperative hospital stay was shorter than the control group (P<0.05). Conclusion: The optimized preoperative dietary management shortens fasting and water deprivation time, reduces the intravenous infusion volume, relieves the hungry and thirsty in patients with consecutive operation of robot-assisted radical prostatectomy at night.

ABSTRACT

OBJECTIVE: Long non-coding RNA (LncRNA) has been reported to play an important role in type 2 diabetes (T2D). We investigated the role of LncRNA maternally expressed gene 3 (MEG3) and its potential interaction with miR-185-5p in palmitate-induced hepatocyte insulin resistance. PATIENTS AND METHODS: High-fat diet (HFD) mice and insulin resistant hepatocyte were employed. Relative mRNA expressions of MEG3, miR-185-5p, and early growth response proteins-2 (Egr2) were measured by qRT-PCR. Western blot was performed to evaluate Egr2 protein expression levels. Glycogen contents and plasma insulin levels were tested by the corresponding assay. RESULTS: MEG3 and Egr2 were upregulated, but miR-185-5p was downregulated in palmitate-treated insulin resistance hepatocytes and HFD mice. MEG3 knockdown alleviated the influence of palmitate on insulin resistance in vitro and in vivo. miR-185-5p expression was upregulated upon MEG3 knockdown. Expression of Egr2 was positively correlated with MEG3 knockdown or overexpression, which could be negatively managed by abnormal expression of miR-185-5p. CONCLUSIONS: Our data demonstrated that LncRNA MEG3 aggravated palmitate-induced insulin resistance by regulating miR-185-5p/Egr2 axis, providing new insights into T2D therapeutic strategies.

ABSTRACT

We report the experimental observation of coherently coupled dark-bright vector solitons in single mode fiber lasers with either normal or anomalous cavity dispersion. The properties of these vector solitons were found to agree well with theoretical predictions based on the coherently coupled nonlinear Schrödinger equations. The experimental results clearly confirmed the existence of a fundamentally new form of optical solitons in coupled nonlinear systems.

ABSTRACT

We report on the experimental observation of vector and bound vector solitons in a fiber laser passively mode locked by graphene. Localized interactions between vector solitons, vector soliton with bound vector solitons, and vector soliton with a bunch of vector solitons are experimentally investigated. We show that depending on the soliton interactions, various stable and dynamic multiple vector soliton states could be formed.

ABSTRACT

We report results of numerical simulations on the various initial conditions for dark soliton generation in an all-normal-dispersion fiber laser. All the dark solitons generated are odd dark solitons. Differently from the dark soliton generation in fibers, where an arbitrary dip could evolve into a dark soliton, it is found that the dark soliton can originate only from an initial dip with a certain parameter requirement. A bright pulse with either a hyperbolic secant square, Gaussian, or Lorentz profile can be developed into a dark soliton, provided that the parameters of the initial bright pulse are selected. Dark solitons can be generated in fiber lasers only if there is a phase jump, and this phase jump can be maintained and evolve to π during the pulse evolution.

ABSTRACT

We report on an ultrahigh repetition rate dark soliton fiber laser. We show both numerically and experimentally that by taking advantage of the cavity self-induced modulation instability and the dark soliton formation in a net normal dispersion cavity fiber laser, stable ultrahigh repetition rate dark soliton trains can be formed in a dispersion-managed cavity fiber laser. Stable dark soliton trains with a repetition rate as high as ∼280 GHz have been generated in our experiment. Numerical simulations have shown that the effective gain bandwidth limitation plays an important role on the stabilization of the formed dark solitons in the laser.

ABSTRACT

We report on the efficient operation of a high-power erbium-doped polycrystalline Er:YAG ceramic laser at 1617 nm resonantly pumped by a high-power 1532 nm Er,Yb fiber laser. Lasing characteristics of Er:YAG ceramics with different Er3+ concentrations are evaluated and compared. With an output coupler of 15% transmission and 0.5 at. % Er3+-doped YAG ceramic as the gain media, the laser generates 14 W of output power at 1617 nm for 28.8 W of incident pump power at 1532 nm, corresponding to a slope efficiency with respect to incident pump power of 51.7%.

ABSTRACT

We report on high-power operation of a fiber distributed-feedback (DFB) laser fabricated from Tm-doped photosensitive alumino-silicate fiber and in-band pumped by an Er/Yb fiber laser at 1565 nm. The fiber DFB laser yielded up to 875 mW of single-ended output at 1943 nm on two orthogonally polarized modes for 3.5 W of absorbed pump power. Further scaling of the DFB laser output power was achieved with the aid of a simple Tm-doped fiber amplifier stage spliced directly to the DFB fiber without the need of an optical isolator. The maximum output power from the DFB laser and fiber amplifier was >3 W for a combined absorbed pump power of 8.1 W. The influence of thermal loading, owing to quantum defect heating in the Tm-doped core, on the output power and longitudinal mode behavior is discussed, and the prospects for further improvement in performance are considered.

ABSTRACT

High-power operation of a cladding-pumped Tm-doped broadband superfluorescent fiber source in the two-micron wavelength regime is described. Predominately single-ended operation was achieved using a simple all-fiber geometry without the use of a high reflectivity mirror or fiber Bragg gratings. The source produced >11 W of single-ended amplified spontaneous emission output spanning the wavelength range from approximately 1930 nm to 1988 nm for a launched diode pump power of approximately 40 W at approximately 790 nm, corresponding to a slope efficiency of 38% with respect to launched pump power. The wavelength spectrum of the superfluorescent source spanned the range from approximately 1650 to 2100 nm with a bandwidth (FWHM) of > 100 nm for output power levels of < 20 mW.

ABSTRACT

High-power room-temperature operation of an Er:YAG laser at 1617 nm in-band pumped by a cladding-pumped Er,Yb fiber laser at 1532 nm is reported. The Er:YAG laser yielded 31 W of continuous-wave output in a beam with M(2) approximately = 2.2 for 72 W of incident pump power. The threshold pump power was 4.1 W and the slope efficiency with respect to incident pump power was approximately 47%. The influence of erbium doping level and resonator design on laser performance is discussed, and the prospects for further increase in output power and improvement in lasing efficiency are considered.

ABSTRACT

A high-power Er:YAG laser that is in-band pumped by a high-power cladding-pumped erbium-ytterbium codoped fiber laser operating at 1532 nm is reported. The Er:YAG laser produced 60.3 W of continuous-wave output at 1645.3 nm in a beam with M2 approximately equal to 3 for 82 W of incident pump power and 20 W of TEM00 output with M2 < 1.2 for 32.4 W of incident pump power. The slope efficiency with respect to incident pump power at pump powers of >20 W was approximately 81%. In the Q-switched mode of operation, a slightly modified resonator configuration incorporating an electro-optic Q switch produced pulses of approximately 4 mJ energy and approximately 100 ns (FWHM) duration, corresponding to a peak power of approximately 42 kW at a repetition rate of 1 kHz for an incident pump power of 16.8 W. The prospects for further improvement in continuous-wave and Q-switched performance are discussed.

ABSTRACT

High-power and widely tunable Tm-doped silica fibre lasers cladding-pumped and core-pumped by a 1565 nm Er,Yb fibre laser are reported. Output power up to 19.2W was generated from the cladding-pumped cavity configuration for ~38.2W of launched pump power and with slope efficiency up to ~72% with respect to absorbed pump power. Wavelength tuning was realized by use of an external cavity containing a diffraction grating. A maximum output power of 17.4 W at 1941 nm was generated for 38.2 W of launched pump power and the operating wavelength could be tuned over 202 nm from 1859 to 2061 nm. In the core-pumped configuration, a maximum output power of 12.1 W was generated at 1851 nm for 23.1 W absorbed pump power using a simple free-running cavity configuration with only ~24 cm of Tm-doped fibre. By employing a tunable cavity configuration, the operating wavelength of the core-pumped Tm:fibre laser could be tuned over 250 nm from 1723-1973 nm at multiwatt power levels.

ABSTRACT

Efficient high-power operation of an erbium-ytterbium co-doped fiber laser cladding-pumped by two spatially-multiplexed and polarization combined 975 nm diode-stacks is reported. Up to 188 W of continuous-wave output at 1.57 microm was generated with a beam-quality factor (M;2) of 1.9 and an overall slope efficiency with respect to launched pump power of 41% (and 43% for output powers <120W). Tunable operation was demonstrated by use of an external cavity containing a diffraction grating and a maximum output power of 108 W at 1538 nm was generated for a launched pump power of ~ 336 W. The operating wavelength was tunable from 1531 to 1571 nm, with >100W output power over a tuning range of 36 nm from 1532 nm to 1568 nm.

ABSTRACT

Efficient single-frequency operation of a Ho:YAG ring laser at room temperature with a traveling-wave TeO2 acousto-optic modulator to enforce unidirectional operation is reported. By use of a 2-at. % Ho3+-doped 10-mm-long Ho:YAG rod, end pumped by a cladding-pumped tunable Tm-doped silica fiber operating at 1.9 microm, the Ho:YAG ring laser yielded 3.7 W of single-frequency output at 2.1 microm in a diffraction-limited TEM00 beam with M2 < 1.1 for an incident pump power of 8.8 W. The rf power required for unidirectional operation was 0.3 W and corresponded to an increase in cavity loss for the lasing direction (due to diffraction) of only 0.5%. The prospects for further improvement in efficiency are discussed.

ABSTRACT

Multienergy Cu(+) ions with total dose of ~10(14) ions/cm(2) were implanted into LiNbO(3) crystals to form a nearly homogenous damage profile from the sample surface to the end of the ions' track. The extraordinary refractive-index enhancement was found in the damage region by observation of the dark mode. The extraordinary index showed an increasing trend until the ion dose reached a critical value. The largest index enhancement was ~0.0132. By use of low-dose ion implantation, we determined that the decreased spontaneous polarization caused the extraordinary index increase.

ABSTRACT

What is believed to be the first planar optical waveguide was formed in BiB(3)O(6) (BIBO) crystal by 2.8-MeV He(+)-ion implantation with a dose of 2x10(16)ions/cm (2) and 2.8-MeV P(+)-ion implantation with a dose of 1x10(14)ions/cm (2) at room temperature. We observed 21 darks modes for the He(+)-ion-implanted BIBO waveguides and four dark modes for the P(+)-ion-implanted waveguides. The refractive-index profile of the He(+)-implanted BIBO waveguide was analyzed. The data also suggest that the BIBO waveguides formed by MeV He(+)-ion and P(+)-ion implantation differ in their developing mechanisms.
