Disturbed motility of the upper gastrointestinal tract presenting with phasic derangement (the absence of all motor phases or peristaltic duodenal contraction, abnormal duration of phases) has been found out in all cholelithiasis patients examined. The findings should in no way be viewed as isolated chronic duodenal insufficiency. Impairment of esophageal, gastric and duodenal motility needs identification and correction in combined treatment, rehabilitation and prevention of cholelithiasis.

A study of the structure of recurrences of ulcer disease was made on the basis of long-term follow-up results in 257 patients with selective proximal vagotomies. Real (with the presence of an ulcer) and false (without ulcer) recurrences are distinguished. The clinical aspects of the course and diagnosis of real and false recurrences are described. The principles of prophylaxis and treatment of ulcer disease recurrences are outlined.

The comparative assessment of the clinico-economical efficiency of the conservative and surgical treatment of the patients with duodenal ulcer disease was carried out. Direct expenses for surgical treatment were 2.5-fold higher than those for conservative treatment. For the five-year period, the expenses for the treatment of recurrences after the use of a conservative method exceeded 2.5-fold those for primary treatment and 4.5-fold--for the late treatment of patients after the operation.
