ABSTRACT

Echocardiographic examination of 56 patients with lymphogranulomatosis involving lymphatic mediastinal nodes showed in four cases a paradoxical excursion of the interventricular septum and systolic thickening ("isosceles triange") of the posterior left ventricular wall. Clinico-instrumental examination of the patients made it possible to rule out in these subjects the presence of constrictive pericarditis, which had been formerly considered one of the causes of a disturbance of left ventricular kinetics.

ABSTRACT

Monolayer cultivation of the radiated bone marrow was performed in 39 lymphogranulomatosis patients with a long-term remission attained after radiotherapy according to the radical program. In 41 patients, the non-radiated bone marrow was also subjected to monolayer cultivation. Counts were made of the total number of myelokaryocytes and myelogram. Analysis of the punctates from the radiated areas has shown that the growth of fibroblast colonies occurred where complete or partial recovery of the cellularity was detected and the total focal dose per bone marrow was less than 35 Gy. With the use of large doses the growth of the colonies was detected in none of the cases. In the radiated areas, the patients commonly manifested bone marrow aplasia. Investigation of the punctates from intact areas pointed to the growth of the colonies in the majority of cases whereas the myelogram count to the increased percentage of the reticular cells, lymphoid and erythroid elements.

ABSTRACT

The function of the respiratory organs using the results of ventilation, gas exchange and roentgenopneumopolygraphy, was analysed in 98 Hodgkin's disease patients in a state of a prolonged remission (5-12 yrs.) after radiation therapy of the mediastinum. Pulmonary ventilation disorders were revealed in 43.9% of the patients. The degree of ventilation disorders depended on the degree of expression of radiation fibrosis in the paramediastinal regions of the lungs. Usually there were no clinical signs of the revealed changes.

ABSTRACT

The paper is concerned with some data on the pregnancy and delivery of 78 patients with Hodgkin's disease as well as on the condition of 89 infants born to them following radiation and combined treatment. It was established that pregnancy and delivery in most of the women were normal, the infants' psychomotor development (at the age of 1 to 14) was in accordance with age.

ABSTRACT

The cardiovascular system was analysed in 157 patients with Hodgkin's disease in a prolonged remission after radiation therapy including irradiation of the mediastinum. The revealed myocardial changes were equally often noted in different irradiation volumes of the heart within the range of 31 to 45 Gy. The number of changes was growing with time. Functional disorders of the pulmonary, hepatic and capillary vessels were also revealed.

ABSTRACT

Secondary tumors were noted in 8 out of 675 patients with Hodgkin's disease during remission following treatment. The following diseases were diagnosed:acute myeloblastic leukemia (1), lung cancer (2), gastric cancer (3), cervical cancer (1), and basalioma (2). The incidence of secondary tumors among patients with Hodgkin's disease turned out to be much higher than in general population. Complications were more frequently observed in men after combined therapy (radio- and drug therapy).

ABSTRACT

A study was made of 200 patients with Hodgkin's disease in a prolonged remission (5-12 yrs.) resulting from radiation therapy. In a long-term period after treatment the patients' height and body mass usually correspond to those obtained at examination of the entire population. Late radiation fibrosis of the soft tissues was noted in 26 patients. It was caused by repeated irradiation for recurrences of Hodgkin's disease, excess single focal doses prior to surgical intervention in the zone of irradiation. In some cases (0.9%) while using wide irradiation fields late radiation fibrosis is also observed when radiation exposure did not exceed values that are considered normal for the treatment of patients with Hodgkin's disease. In some of the patients (at the moment of treatment their age ranged between 14-16) at later time pronounced hypotrophy of the neck soft tissues (the "thin neck" syndrome) was observed, accompanied by weakness, boring pain, convulsive muscular twitching in this area, causing cosmetic discomfort.

ABSTRACT

Some data on 239 patients with Hodgkin's disease in a long-term (5-12 yrs.) remission achieved with radiotherapy, have been analysed. The number of married persons and the number of children in a family were less, and the number of divorces equal to indices determined during mass screening of population. The rates of morbidity with a loss of capacity to work were consistent with control values. Most of the patients (90.3%) after therapy resumed their work the nature of which was mainly the same (88.6%). In expert medical evaluation of the working capacity there are no reliable criteria. It should proceed from the completeness of remission and the nature of the pathological process.

ABSTRACT

In a group of 180 patients with Hodgkin's disease who received irradiation of the mediastinum, 23 (12.7%) developed shortly after that (up to 6 mos.) the signs of radiation pulmonitis, later on 171 (95%) local paramediastinal fibrosis. Radiation changes revealed were either subclinical or asymptomatic. The frequency of early radiation changes and the extent of local fibrosis depend on the volume of irradiated pulmonary tissue. The summary focal doses within the range of 31-45Gy are less important.

ABSTRACT

Cases of radiation pericarditis among 707 patients with Hodgkin's disease after irradiation of the mediastinum were analysed. In 100 patients irradiation was combined with polychemotherapy in cycles, SORR scheme. Radiation exudative pericarditis was noted in 2.6% of patients. Later on an adhesive process was diagnosed in some of patients. Radiation pericarditis was accompanied either by feebly marked signs or no signs at all. In cases of a lingering course (over 24 mos.) of the disease surgical intervention is indicated. The frequency of radiation complications caused by pericarditis depends on the volume of irradiation and, to a lesser degree, on the value of a summary focal dose delivered to the heart within the range of 31-45 Gy. For prevention the heart should be shielded with lead blocks.
